    Abstract Malignant melanoma of unknown primary is rare. Cystic malignant melanoma is again rare. Metastatic malignant melanoma with unknown primary presenting as a cystic mass in the anteromedial compartment of the thigh masquerading as soft tissue sarcoma has not been reported so far. We report one such rare occurrence in a 63-year-old male and review pertinent literature to increase awareness regarding such unusual presentations of malignant melanoma among radiologists and clinicians.

    Abstract Papillary thyroid carcinoma (PTC) is a common malignancy with multiple variants, some of which are rarely encountered in routine surgical pathology practice. PTC with exuberant nodular fasciitis-like stroma or PTC with fibromatosis-like stroma is one such variant. This tumor is characterized by an abundant stromal component with an intervening epithelial component with the typical morphologic features of PTC. We describe gross and histopathological features of this rare variant of papillary carcinoma in a 38-year-old female and review the literature. We also discuss the importance of a thorough search for epithelial components within any fibroproliferative lesion of the thyroid and address the diagnostic difficulties created by the tumor's extensive stromal component. Given the rarity of this condition, the experience gained from the present case is a useful addition to the current knowledge on disease prognostication and management.

    Abstract Plasmacytoid urothelial carcinoma (PUC) of the urinary bladder is an uncommon and aggressive variant of urothelial carcinoma associated with late presentation and poor prognosis. Immunohistochemical examination showing expression of epithelial markers, CD138, and losing the membranous expression of E-cadherin confirms evidence of PUC. Here, we report a case of bladder PUC in a 78-year-old male who presented with hematuria. A transurethral biopsy revealed urothelial carcinoma with plasmacytoid appearance. The diagnostic dilemmas of this unusual variant of urothelial malignancy and its clinical impact are discussed. The pathological diagnosis was PUC with diffuse muscle and vascular invasion, in which almost of the areas studied on the tissue section showed the plasmacytoid appearance of tumor cells. The tumor cells showed positivity for CK7 and CD138. The diagnostic dilemmas of this unusual variant of urothelial malignancy and its clinical impact are discussed.

    Abstract Background: The earliest hematological manifestation of HIV infection can occur at the time of primary infection. Several peripheral blood abnormalities are known in HIV. However, not many studies have reported morphological changes in white blood cells in antiviral naïve patients and even fewer have compared them with HIV seropositive patients. Material and methods: Peripheral blood smears from 100 patients were studied. 50 of them were newly diagnosed HIV seropositive cases and 50 were HIV seronegative. A peripheral blood smear was made to study morphology of various white blood cells A written informed consent was taken. Statistical analysis of the data obtained was done. Results: Morphological abnormalities in the lymphocytes had been the most significant finding, which included nuclear lobulation, nuclear convolution and plasmacytoid and monocytoid features. Neutrophils also showed various morphological abnormalities. Most common were hypogranulation, hypersegmented nuclei and cytoplasmic vacuolations in descending order of frequency. Monocytes also revealed few morphological abnormalities including nuclear irregularities and cytoplasmic vacuolations. Conclusion: The observations from this study will serve as a guide to clinicians as treatment of these patients should include supplements to monitor and improve these changes.
