Abstract Coronavirus disease 2019 (COVID-19) is an emerged pandemic disease caused by a new coronavirus known as SARS-CoV-2 (severe acute respiratory syndrome-coronavirus-2). Initially the mortality of this infection are related to severe acute respiratory distress syndrome, but several publications also showed that this infection caused an inflammatory response with severe systemic complications [1]. Venous thromboembolism has been shown to be an important cause of morbidity and mortality in patients with COVID-19, both in the general inpatient and in the intensive care unit (ICU) setting, and even in patients receiving therapeutic anticoagulation [2]. We report the case of an adult patient who presented a cavernous sinus thrombosis during his hospitalization for COVID-19.

Abstract Cavernous sinus thrombosis (CST) is a life-threatening condition occurring due to an active infection, coagulation abnormalities, and trauma. Thromboembolic events are known complications secondary to novel coronavirus disease-19 (COVID-19) due to dysfunction in endothelial function and clotting factor dysfunction. We report a case of cavernous sinus thrombosis and delayed ophthalmic vein thrombosis associated with a preceding COVID-19 infection confirmed with a facial computed tomography (CT) scan. Clinicians should have a broad differential in patients with hypercoagulable states, as complications secondary to COVID-19 can be life-threatening.

Abstract Direct carotid-cavernous fistula is a high-flow communication between the internal carotid artery and the cavernous sinus that requires early transarterial embolization for its resolution. We report a case of a patient with a direct carotid-cavernous fistula who subsequently developed a central retinal vein thrombosis due to a delay in treatment related to the health collapse experienced in the first months of the Covid-19 pandemic in Spain.

Abstract This paper describes a case of cavernous sinus thrombosis. The condition developed in a patient who had been treated for confirmed COVID-19. Left eye examination findings were as follows: amaurosis; absence of periorbital skin sensation; signs of orbital cellulitis; ptosis; chemosis; total ophthalmoplegia; exophthalmos; decreased corneal sensation; widened pupil; and absence of pupil light reflex. In addition, ophthalmoscopy showed diffused retinal and optic disc edema. The patient had an atypical clinical course which could not be clearly identified as septic or aseptic, and his elevated D-dimer level could indicate a potential relationship between this condition and COVID-19.

Abstract Rationale: Presenting this case report is of essential importance to shed the light on this rare clinical situation. Patient Concerns: A 68-year-old man presented with generalised weakness, shortness of breath, mild fever and loss of smell and taste for three days. Diagnosis: A filling deficiency of the cavernous sinus was detected by contrast-enhanced CT scanning of the brain. Interventions: He received intravenous injection of remdesivir with intravenous antibiotics (amoxicillin-A clavulanic 1.2 g/metronidazole 500 mg), and tocilizumab 400 mg intravenous once.He was also taken one unit of plasma transfusion with supportive care. Outcomes: After 1 week, the patient recovered from pneumonia and also recovered from features of cavernous sinus thrombosis. Lessons: COVID-19 infections are a serious cause of hypercoagulation condition which increases the chance of cavernous sinus thrombosis, which need early diagnosis and treatment.

Abstract Patients with COVID-19 have been reported to have elevated coagulation factors, which is a well-documented cause of venous thromboembolism events such as deep vein thrombosis and pulmonary embolism. Other venous thrombotic events, however, such as cavernous sinus thrombosis (CST) have been less commonly observed, specifically in combination with primary orbital cellulitis. Due to its unique anatomic location, the cavernous sinus is susceptible to thrombophlebitis processes including septic thrombosis and thrombosis most commonly from sinusitis. Many studies have shown that in the antibiotic era thromboembolic events of the cavernous sinus are less common due to infection spread from the orbit or facial region. This case report describes a 17-year-old COVID-19 positive male who presented with a left-sided primary orbital cellulitis with CST without radiographic evidence of ipsilateral sinus disease.
