    Abstract Eosinophilic colitis is a rare condition characterized by histologic findings of high eosinophilic infiltrate in the gut wall, typically presenting with diarrhea and abdominal pain. The etiology of this entity remains unclear because it can be primary or can occur secondarily to infections, drugs, or even in association with immune-mediated diseases. We present the case of a woman referred to our outpatient clinic for chronic diarrhea that had been worsening for months. Colonoscopy with biopsies was performed, and eosinophilic colitis associated with the use of clopidogrel was diagnosed. After clopidogrel discontinuation, a complete remission of the clinical and histological picture was observed.

    Abstract Rationale: Eosinophilic pulmonary disease (EPD) is a general term for a large group of diseases with complex etiology. Ulcerative colitis is an inflammatory bowel disease (IBD). Patients with IBD may have pulmonary involvement. We herein present a case of ulcerative colitis complicated with EPD.
    Patient concerns: A 34-year-old woman with ulcerative colitis presented with dry cough. She had peripheral eosinophilia and apical ground glass opacities on CT (computed tomography) of her chest. Antibiotic treatment was ineffective.
    Diagnoses: Lung biopsy revealed eosinophil infiltration in the alveolar space and interstitial space, so EPD was considered.
    Interventions: After oral administration of prednisone, the lung shadow on CT disappeared when the cough symptoms resolved. However, the symptoms recurred after drug withdrawal, and the lung shadow reappeared on imaging. The cough symptoms and lung shadow disappeared after oral prednisone was given again. Prednisone was slowly discontinued after 6 months of treatment.
    Outcomes: The patient stopped prednisone for half a year. No recurrence or abnormal CT findings were detected during the half-year follow-up.
    Lessons: The clinical manifestations of EPD are atypical, laboratory and imaging findings are not specific, and it is difficult to make a definite diagnosis before lung biopsy. The diagnosis depends on pathological examination. Glucocorticoid treatment is effective, but some patients may relapse after drug withdrawal. Active follow-up after glucocorticoid treatment is very important for identifying disease recurrence. Patients with IBD are relatively prone to developing EPD. The etiology of EPD is complex. In clinical practice, we need to make a diagnosis and differential diagnosis to clarify its etiology.

    Abstract Eosinophilic colitis (EC) is the most uncommon disease within the spectrum of eosinophilic gastrointestinal disorders. Currently, there are no well-established diagnostic criteria and no approved Food and Drug Administration therapies. We discuss a case of a 30-year-old man with refractory malignant EC leading to ischemic colitis. The patient was tried on multiple therapies, including a combination therapy of benralizumab and vedolizumab, which were unsuccessful. Unfortunately, the patient ultimately required a total proctocolectomy. More studies need to be performed to treat patients with severe refractory cases of EC.

    Abstract Eosinophilic colitis (EoC) is a pathological entity associated with abnormal infiltration of colonic mucosa by eosinophilic polynuclear cells (Eo). This is a relatively common pathology in infants and children under 2 years old, but is more rare and has been less studied in adults. EoC can be classified as primary or secondary. Primary EoC is, in the majority of cases, related to an allergic reaction, either IgE-mediated and capable of causing an anaphylactic-type food allergy, or not mediated by IgE and capable of giving rise to food enteropathy. The symptoms for adults with EoC are variable and non-specific, diarrhoea and abdominal pain being the most common signs. There is no histological consensus for the diagnosis of EoC. The presence of over 40 Eo per high-power field (×400) in at least two different colonic segments could be suggested as the criterion for the diagnosis. In adults with primary EoC, skin tests are of limited value and the response to a restrictive diet is less effective than in young children, given that IgE or non-IgE-mediated allergic reactions are rarely identified and EoC generally require medical treatment. There is no consensus on the treatment of EoC, but the potential efficacy of corticosteroids and budesonide has been demonstrated in the vast majority of cases studied.

    Abstract Mycophenolate mofetil is a medication used for the prevention of allograft rejection in organ transplant patients. Eosinophilic colitis due to mycophenolate is a recognized condition in patients with a history of renal transplantation and persistent diarrhea, requiring a biopsy to achieve an appropriate diagnosis and treatment. The case of an 11-year-old boy with diarrhea is presented; colon biopsy showed edema and eosinophilic inflammatory in-filtrate in the lamina propria with microabscess formation, with epithelial and crypt damage. Because of the absence of another cause of the clinical case, mycophenolate was withdrawn from the treatment scheme, with re-solution of the symptoms. The Naranjo adverse drug reaction probability scale was definitive with a score of 10. The report contributes to the literature with the characterization of the histopathological findings in mycophenolate-induced colonic injury.

    Abstract Introduction: eosinophilic colitis is a gastrointestinal disorder characterized by an inflammatory infiltration in the colon and rectum, with predominance of eosinophils, its clinical manifestation depends on the affected intestinal epithelium layer, the exclusive affectation of the colon is exceptional, intestinal obstruction is the most evident manifestation and it is a disease of complex diagnosis, where the histological study is conclusive. It is present from 1 to 20 inhabitants per 100.000, its etiology is imprecise, it has been determined that there is an important genetic and allergic component. Corticosteroids are the treatment of first choice. Case report: a 12-year-old patient, presented abdominal pain, distension and was admitted, evolved with clinical and radiological signs of a mechanical ileus with progressive worsening, it was decided an exploratory laparotomy and colon biopsy. The patient was diagnosed with eosinophilic colitis and treated with corticosteroids, showing a satisfactory progression at 72 hours, with regression of the clinical chart. Conclusion: it is rare and little study pathology, not frequently found, it affects the muscular layer as a type of intestinal obstruction, this should be taken into account as a differential diagnosis of acute abdomen in pediatric patients.
