Abstract The social perception of the nursing profession in Poland is profoundly affected by social stereotypes that may discourage young people from entering the profession and lead to prejudice towards nurses. During the COVID-19 pandemic, nurses gained visibility, which caused the social image of nurses to likewise grow. In this study, we explore nurses' experiences with respect to how the COVID-19 pandemic influenced the social image of nursing. Semi-structured interviews were conducted with fifteen hospital nurses. Three major themes emerged: (1) social attitudes towards nurses during the pandemic, (2) nurses' perception of the impact of the pandemic on the social image of the nursing profession and (3) the impact of the pandemic on nurses' mental health. Although the pandemic promoted the image of nursing to the general public, nurses were disappointed that in the face of the healthcare crisis and the constant threat, they had to struggle with difficult working conditions and professional, social and economic recognition. This study therefore stresses the duty of policy makers to undertake a systemic approach to improving the organisation of health care and increase nurses' safety by providing them with a safe working environment and prepare them better for the next health crisis.

Abstract Background: Due to additional responsibilities and uncertainties during the COVID-19 pandemic, primary healthcare (PHC) workers are at increased risk of burnout.
Aim: To determine and compare the burnout levels and related factors in PHC nurses and family physicians (FPs) during the COVID-19 pandemic.
Methods: An online survey was delivered to PHC workers. Non-random sampling method was used. To evaluate burnout, the Maslach Burnout Inventory was used, which investigates burnout in three categories: emotional exhaustion (EE), depersonalization (DP) and reduced personal accomplishment (PA). Multivariate linear regression was used to analyze factors associated with burnout for FPs and nurses separately.
Findings: Among the participants, 55.7% were nurses, the mean age was 42.34. FPs and nurses experienced similar levels of burnout in terms of EE. Family physicians had higher levels of low PA and DP. Based on the results of the multivariate analysis, while higher EE levels were significantly associated with unequal distribution of workload and communication problems within the Family Health Center for physicians, the unequal distribution of PPE, lack of appreciation by patients or colleagues and restrictions on work-related rights were relevant factors for nurses. Lack of appreciation and restrictions of the rights were associated with increased DP scores in both groups. Unequal distribution of workload was also associated with reduced PA among FPs.
Conclusion: PHC physicians and nurses are affected by burnout in different ways under the conditions of the COVID-19 pandemic based on gender, socioeconomic status and working conditions. To protect the mental health of PHC workers in the next public health emergency, clarification in the organization of services, empowering PHC workers in emergency risk communication and provision of timely, adequate and free PPE is essential. It is also crucial to ensure the rights of health workers through macro policy changes especially during emergencies.

A CHALLENGE OF PROTECTING THE GENERAL POPULATION AND HEALTH-CARE WORKERS

    2020  Volume 1, Issue : KMUJ 2020

Abstract: ... from patients in Wuhan,2 and within next month WHO declared it as “Public Health Emergency of International ... prevention and control of infection and risk communication. COVID-19 is caused by a novel coronavirus ... for critically ill COVID-19 is a global issue and shortage of ventilators and other equipment has been reported ...

Abstract Year 2020 proved to be a nightmare for global health and economy by widespread outbreak of coronavirus across the globe. First reported at Wuhan, the capital city of Hubei province of China as pneumonia of unknown cause on December 31, 2019.1 Within a week, a novel coronavirus was isolated from patients in Wuhan,2 and within next month WHO declared it as “Public Health Emergency of International Concern”. WHO named this disease as new coronavirus disease 2019 (COVID-19) on February 11, 2020.1 On March 11, 2020, WHO declared COVID-19 as pandemic.3 As per WHO COVID-19 situation Report–70, total confirmed cases of COVID-19 until March 30, 2020 were 693224 and deaths due to COVID-19 were 33106 in the world.4 COVID-19 has affected more than 200 countries and regions in a very short period. In Pakistan, since the confirmation of first case on February 26, 2020; the total confirmed cases of COVID-19 are 1,865 and 25 deaths till March 31, 2020.5The unprecedented pandemic of COVID-19 led to a panic situation around the world. The major issue is to prevent the spread of corona virus to other regions. Recent reports have confirmed the person-to-person transmission of COVID-19, leading to affect the immediate contacts of patients including health-care workers. People traveling from the affected areas are the potential source of disease transmission.6,7 This led to international and domestic travel and/or trade restriction in affected areas and quarantine for returning travelers by various countries. To ensure social distancing among common people, lock-down of provinces, regions and cities was executed by various countries. WHO recommends public health education regarding promotion of personal hygiene like washing hands, use of sanitizers, ensuring cough etiquette and avoiding crowded places (religious places, markets, etc) as preventive strategy. However, decision regarding lockdown and travel restriction is very difficult due to financial reasons and to maintain the supply-chain of essential goods and food items. Delay in decision-making regarding such actions by some countries resulted in rapid spread of COVID-19 with increased morbidity and mortality. Italy (n=10781) and Spain (n=6528) are such countries with highest number of COVID-19 related deaths so far.4 In resource-poor countries like Pakistan, the decision regarding complete lock-down and social distancing could be very challenging due to socioeconomic and other reasons. Preparedness and response of the country to such pandemics is critical for disease surveillance, diagnostics workup, clinical management, prevention and control of infection and risk communication. COVID-19 is caused by a novel coronavirus (severe acute respiratory syndrome coronavirus-2) and until now, there is no vaccine or cure available for it.8, 9 Majority of patients are with mild to moderate severity and have good survival. However, critically ill patients require intensive care and mechanical ventilation for hypoxemic respiratory failure. Mortality in these patients is very high even in best centres of the world.9 Availability of necessary equipment for critically ill COVID-19 is a global issue and shortage of ventilators and other equipment has been reported from USA as well.10 In developing countries like Pakistan, the shortage of high dependency units, fully equipped intensive care units and ventilators is a main challenge for governments. Availability of intensivist and trained staff for operating ventilators is another major issue in managing seriously ill COVID-19 patients in developing countries. Another major challenge faced during coping with COVID-19 pandemic was ensuring the safety of frontline health-care workers. So far, more than 3300 health-care providers were infected with COVID-19 at China11 and more than 60 doctors died in Italy.12 In Pakistan, Dr. Osama Riaz, a young doctor who was infected with COVID-19 during handling patients died at Gilgit.13 These frontline health-care workers are fighting “unarmed" against COVID-19. Health-care workers should be fully equipped with adequate number of personal protection equipment including N95 respirator and surgical masks, gloves, face shields, goggles, gowns and hand sanitizers. Proper triage system should be established at every health-care facility for screening of COVID-19 patients. Number of trained frontline health-care workers should be increased to reduce the overburden and exhaustion of the health workforce. This is a critical time for all stakeholders of the government to establish a national command and control system to combat COVID-19 and similar outbreaks. A substantial increase in current health budget is crucial to meet the requirements of national health related issues. Government must prioritize the health and rationalize the number of beds per 1000 people in hospitals and intensive care units as per international requirements. Specialised training in field of intensive care and other relevant specialties must be ensured to doctors, nurses and paramedics to provide and maintain high-quality critical care to patients. Community education regarding various Public health issues and personal hygiene through electronic and social media as well as at school level can prove vital in prevention of such outbreaks.

Abstract Objectives: To better understand the knowledge, practice, importance, awareness, usefulness, and confidence of non-Hispanic Black and English- and Spanish-speaking Hispanic/Latino adults with diabetes.
Design: A descriptive cross-sectional survey study design was used and descriptive statistics was conducted.
Sample: Non-Hispanic Black and Hispanic/Latino adults with diabetes were recruited from three New York City public hospitals.
Measurements: A one-time survey was delivered via email, text message or over the phone.
Results: Of the 96 participants, 47.9% were Hispanic/Latino and 52.1% were non-Hispanic Black individuals; 43.8% of the surveys were completed in Spanish and 56.3% in English; 41.7% were female and 58.3% male; 77.1% preferred to complete the survey via the telephone, 14.6% through email, and 8.3% via text message. Chi-square findings showed, 90.6% knew mask wearing prevented COVID-19; 96.9% knew that covering the nose and mouth during mask wearing is needed, 93.8% wore a mask, and 92.8% felt it important or very important to wear a mask to prevent the spread of COVID-19. For social distancing, 88.5% knew it prevented the spread of COVID-19, 93.8% practiced it, and 95.8% felt it important or very important.
Conclusion: In having a better understanding of the knowledge and practices of COVID-19 among non-Hispanic Black and Hispanic populations with diabetes, the development of culturally and linguistically tailored community-based mitigation strategies can be developed that are aimed at improving the preparedness of these groups for the next emerging infectious disease, such as COVID-19.

Hypothesis of the Lab Origin Versus a Zoonotic Event which can also be of a Lab Origin

Abstract Abstract: To treat the cause of a disease and not only its effects is of the utmost importance; hence, we need to know the origin of this pandemic of COVID-19, in order to be able, if possible, to prevent an event of such a nature and magnitude in the future, and to be able to avoid every sort of abuses to humanity, as it is happening right now. Bullet points here addressed are: 1) To have, inside the backbone of a virus from a bat (mostly ~97.55% of the viral RNA (by deducting the HIV inserts found by Perez, Montagnier and others), & as per the findings of Petrovsky, see below, and also to contrast the differences), the insertion similar to that of a pangolin virus for the Receptor Binding Domain (RBD, which basically consists of six separated key amino acids, or the 0.06% of its genome for these particular 18 nucleotides), being their receptor the ACE2 of the human lung, appearing at a time (as earlier as since September of 2019), were there were already mature all of the molecular methodologies necessary to modify individual nucleotides (Crispr-Cas9, “Seamless”, etc.) that then modify at will the resulting amino acids, with the possibility to give an extra passage to the virus through ferrets (or other lab animals) that have an ACE2 very similar to the humans, to give it then a more “natural” appearance (by random trivial changes); because, had it been natural, this could had required an animal host infected with these two viruses simultaneously, and that with an unexplainable marksmanship, to specifically modify the key six codons (and a second independent of such impossible recombinants, to give raise to the differences exclusively present at the end of the long Orf1ab, into the Nsf15 and Nsf16); 2) To have an even more important and unique peculiar site, PRRAR (encompassing the needed 12 bases to complete that sequence, being this the 0.04% of the full genome), for protease cleavage (new to Plasmin and Furin, plus Trypsin, TMRPSS2, etc.) inside the protein called Spike (S), to obtain the fragments S1 and S2 in order to allow the viral RNA to penetrate into the cell (expanding the range, not only to lung cells as the previous modification, but also to white and to neural cells), whose nucleotides producing it are highly strange to the rest of the viral sequence, because they contain more than an 83% of richness in its nucleotides GC, being these 12 nucleotides alien to the rest of the virus: CCUCGGCGGGCA (similar to bacterial and to methodological sequences patented by Moderna, Inc., cleavable by restriction enzymes BsaJI, AciI, Cac8I, MnlI…), that are engrained to the three remaining bases: CGU present in the frame of the bat virus to complete the necessary sequence. This will require, either a third virus completely unknown until now, either in the same utopian animal described before, or through a second passage of the first chimera into another animal, and then that such viral beast, could also be able to target exclusively this region, and no other site whatsoever; then, it is explored, 3) The biggest shot in variation, when it is compared to the first sequence obtained of the virus of COVID-19, with its immediate ancestor, that according to Shi Zheng-Li is the RaTG13 (submitted a posteriori of the COVID-19 first sequence, and which researchers demonstrate that this is a partially made-up sequence (see below), having her deliberately ignored even to cite her previous identical reference called BtCoV/4991 (2016), or even her most recent reference of the same that she put under the name of SARSr-CoV Ra4991 (2019), being very dishonest for her to change in at least three identified times the names of her same sequence, actions that render her highly suspicious, because she hid the rest of the sequence at least during the last four year (having been obtained from excrement in a cave, she says, after a call due to a serious case of miners infected at Yunnan, and nobody knows still what was inside those at least six miners), but her publishing it until now, after the emergence of a similar virus, makes her highly suspicious, rather than making her look innocent; and, who can say that she did not manipulate as well artificially such sequence, or that the CCP Chinese military did not do the same to the other two previous sequences that are also somehow similar to Sars-CoV-2?, and how many more hundreds of sequences will they be hiding?, because nobody independently has been able to verify the accuracy of their claims, being everything based only in what they say), given that the nucleotides of six proteins exhibit a 99% of similitude between both sequences, while twelve of them go down to a 96% or even are below of this number, being the most extreme changes, the ones that are inside the sequence for the protein Spike, which while exhibiting a global similitude of 93%, is the one having the highest discrepancy between the two sequences, and within this same one there are extreme shorter variations, with a low similitude of 44% on that specific of the RBD mentioned before, which goes down to some 17% for the region of those 18 key bases, and of only the 20% percent for that sequence of 12 bases for the resulting protease cleavage site; other changes include the optimal nucleotides of an even shorter region of 16 segments similar to immunodeficiency genes (plus two more distant ones), and even a couple of concatenated Plasmodium yoelii found by Perez and Montagnier at the S2 place, all that could be better explained with artificial processes already in place to do this and more within the frame of the awful Gain-of-Function sinister and dual-purpose (or double-talk) research. So, basically is their word against the world, and that is why since at least 2010 I have been proposing an independent verification by at least three other labs of results reported, specially by CCP Chinese researchers, as they did cost me already my first job in the US by their lying during at least ten years about a methodological artifact that I called “Palindromati”, and that they kept on reporting as “natural”, while receiving grants to explore a chimera, and how more is it costing their apparent lying about the artificial origin of COVID-19 at this time?) So, all of these points and so much more, because Jesse Morrell, for example, is reaching a set of almost 40 (and counting) evidences of a lab origin versus cero otherwise, things and persons that are leading us to conclude that it is evident to see that there was human intervention in the emergence of this Sars-CoV-2 virus, because in 2015-2018 there was not in existence any zoonotic history of any class in Wuhan, so, having been originated this virus already mature and fully capable to attack the human population, implies an artificial “injecting” source. “…have no fellowship with the unfruitful works of DARKNESS, but rather expose them. For it is shameful even to speak of those things which are done by THEM in secret. But all things that are exposed are made manifest by the LIGHT.” An inspired Paul in Eph. 5:11-13. Dedicated to Francis S. Collins, so as for the many to be able to see…

Data were extracted and ...

Abstract Aims and objectives: This study aimed (a) to identify the communication issues and problems faced by individuals with hearing impairment (HI)/deafness during the COVID-19 pandemic and (b) to describe strategies to overcome the issues/problems and/or prevent their negative impact.
Background: Individuals with mild or severe HI face everyday communication problems, which have been worsened during the COVID-19 pandemic. However, no studies have summarised the available evidence to better understand the communication challenges faced by them and strategies allowing better interactions. The long duration of the outbreak-more than 2 years, with policies that have just been lifted in some countries-and the possible return of restrictions in the next Winter suggest the need to summarise evidence in the field.
Design and methods: A rapid review is reported here in accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analysis guidelines. Medline, CINAHL and Scopus databases were searched, including (a) primary or secondary studies published from January 2020 to 12 January 2022, (b) involving individuals with HI/deafness, (c) during the COVID-19 pandemic and (d) written in English. Data were extracted and summarised by using a content analysis approach.
Patient or public contribution: No Patient or Public Contribution.
Results: Fourteen studies were included as follows: three non-systematic reviews, seven cross-sectional, three quasi- experimental and one qualitative study, performed mainly in the US and the UK. Face mask covering use; physical and social distancing; and information, education, rehabilitation, and healthcare accessibility have emerged as the main challenges triggering consequences such as social isolation, loneliness, poor knowledge regarding the prevention and mental health issues. Strategies mitigating these challenges are as follows: (a) adopting transparent face masks, (b) using basic skills while interacting (e.g. maintaining eye contact), (c) improving the availability of sign language interpreters, (d) allowing the presence of family members and (e) teaching basics of sign language to healthcare professionals.
Conclusions and relevance to clinical practice: Individuals with HI/deafness live with several challenges, suggesting that their vulnerability has increased tremendously during the COVID-19 pandemic. The effectiveness of strategies to overcome these difficulties should be scrutinised by conducting more research. Moreover, there should be increased awareness among all citizens by equipping them with simple strategies to communicate effectively with individuals with HI, an approach that may increase inclusiveness and prevent further negative consequences and burden.

Abstract The 2003 SARS pandemic heralded the return of quarantine as a vital part of twenty-first century public health practice. Over the last two decades, MERS, Ebola, and other emerging infectious diseases each posed unique challenges for applying quarantine ethics lessons learned from the 2003 SARS-CoV-1 outbreak. In an increasingly interdependent and connected global world, the use of quarantine to contain the spread of SARS-CoV-2, or COVID-19, similarly poses new and unexpected ethical challenges. In this essay, we look beyond standard debates about the ethics of quarantine and state power to explore a key quarantine principle, Reciprocity, and how it is being negotiated by healthcare workers, volunteers, and citizens in the context of the Wuhan, China, quarantine. We analyze Reciprocity through the lens of two Wuhan case studies: (1) healthcare workers, particularly nurses, who are simultaneously essential workers and quarantined citizens, asked by their hospital administration to shave their heads because adequate PPE was not available, and (2) citizen-to-citizen mutual aid societies attempting to fill gaps in essential supplies left unfilled by the state. We analyze social media and video-blogs from Wuhan, on the platforms of Douyin and Sina Weibo, to understand how people define and respond to ethical and legal obligations in the wake of COVID-19. It is no surprise that quarantine principles from the 2003 SARS outbreak are inadequate for COVID-19 and that both infectious disease outbreak responses and ethics must adapt to the virtual age. We offer ideas to strengthen and clarify Reciprocal obligations for the state, hospital administrators, and citizens as the globe prepares for the next wave of COVID-19 circulating now.
