This paper summarized China’s approaches and aimed to provide illumination for health services decision-makers.Methods: We systematically reviewed the construction of precise community-based public health management from three aspects: organizational structure, service content, and flexible adjustment, and summed up four essential elements for success. Then, we selected 9 typical countries to compare their non-pharmaceutical strategies and daily new cases.Results: China’s community-based public health management has a clear four-level organizational structure. It has worked on infection control, surveillance for new cases, management of contacts, health education, medicare service, outdoor environment disinfection, and living material supply in different stages, and the daily new confirmed cases in Wuhan sustainedly declined to zero. The outbreak was relatively well contained in China, Italy, and Germany as of June 2020, as they adopted stricter movement restrictions, social distance, and patient tracking.Conclusion: China’s experience has proved the feasibility of non-pharmaceutical strategies responding to COVID-19. The precise community-based public health management strategy can be considered, as it’s pretty much the same as public health and social measures (PHSMs) advocated by WHO.Keywords: COVID-19, public health management, community, ...

    Abstract Community is directly responsible for implementing the policy measures against public emergencies such as extreme weather, earthquake, flash flood and pandemic diseases. Summarizing the experience at the urban community level can help promote the global emergency management. This paper, from the perspective of COVID-19 prevention and control, selects three communities with different features to explore an effective response system for public emergency management at the urban community level. It demonstrates that based on a centralized leadership, the multi-level governance (MLG) network combined with information technology has mobilized their participation in combating COVID-19 and ensured the effective and timely response to the public emergencies, which blocked the spread of epidemic to the greatest extent, proving worthy of being promoted as one of the Chinese approaches and Chinese wisdom.

    Abstract In response to COVID-19 that has constituted a global pandemic, countries around the world have successively adopted a myriad of prevention and control measures. As the first country with the COVID-19 outbreak, the Chinese government has adopted a series of timely and strict prevention and control measures against the spread of the SARS-CoV-2, which has effectively slowed down the spread of the SARS-CoV-2 and created a valuable window for the international community to overcome the epidemic. China's experience in combating the COVID-19 has shown that building a community prevention and control system is essential to control the spread of coronavirus. As the backbone of the epidemic prevention and control system, the community prevention and control system plays an important role in improving the pattern of disorderly medical treatment, screening suspected patients, preventing the input of pathogens, ensuring residents' medical needs, stabilizing public sentiment, reducing disease fear, and maintaining residents' national security. At the same time, it also exposed the problems of the community prevention and control epidemic system in terms of infrastructure, human resources, and internal systems. Based on this, this article suggests that we should improve the hardware facilities of community, improve the internal mechanism of the community, strengthen the stability of the community talent team, improve the level of linkage between the community and other departments to prevent and control the spread of SARS-CoV-2, effectively use information technology and actively mobilize social forces to help community prevention and control COVID-19.

    Abstract Introduction: Coronavirus disease (COVID-19), a respiratory illness, first discovered in China in December 2019 has now spread to 213 countries or territories affecting millions across the globe. We received a request from National Health Systems Resource Centre, a public agency in India, for a Rapid Evidence Synthesis (RES) on community health workers (CHWs) for COVID-19 prevention and control. Methods: We searched PubMed, websites of ministries, public agencies, multilateral institutions, COVID-19 resource aggregators and pre-prints (without language restrictions) for articles on the role, challenges and enablers for CHWs in pandemics. Two reviewers screened the records independently with a third reviewer resolving disagreements. One reviewer extracted data in a consensus data extraction form with another reviewer cross-checking it. A framework on CHW performance in primary healthcare not specific to pandemic was used to guide data extraction and narrative analysis. Results: We retrieved 211 records and finally included 36 articles on the role, challenges and enablers for CHWs in pandemics. We found that CHWs play an important role in building awareness, countering stigma and maintaining essential primary healthcare service delivery. It is essential that CHWs are provided adequate Personal Protective Equipment (PPE) and appropriately trained in its usage in the early stages of the pandemic. Wide range of policies and guidance is required to ensure health systems functioning. A clear guidance for prioritizing essential activities, postponing non-essential ones and additional pandemic related activities is crucial. CHWs experience stigmatization, isolation and social exclusion. Psychosocial support, non-performance-based incentives, additional transport allowance, accommodation, child-support, awards and recognition programs have been used in previous pandemics. We also created inventories of resources with guiding notes for quick utility by decision makers on guidelines for health workers (n=24), self-isolation in the community (n=10) and information, education and counselling materials on COVID-19 (n=16). Conclusions: CHWs play a critical role in pandemics like COVID-19. It is important to ensure role clarity, training, supportive supervision, as well as their work satisfaction, health and well-being. There is a need for more implementation research on CHWs in pandemics like COVID-19.
