    Abstract This review article considers the possibilities of combined antiviral therapy in the treatment of patients with COVID-19, based on the analysis of the mechanism of action of known antiviral drugs in the framework of the medical hypothesis. The potential effectiveness of the joint use of viral RNA polymerase inhibitors and a fusion inhibitor in this pathology is discussed. The review discusses the main representatives of these groups of drugs - ribavirin, riamilovir, umifenovir, favipiravir. The efficacy and safety profile of these drugs was analyzed, including the experience of their use in clinical trials conducted during the COVID-19 pandemic, as well as earlier work performed during the SARS and MERS epidemics.

    Title translation Comparative analysis of RDW variability depending on belonging to a defined nosological group of diseases according to ICD-10 classification.
    Abstract The aim of this study was to investigate RDW variability and conduct a comparative analysis of the RDW level in patients depending on their belonging to a defined nosological group of diseases according to the ICD-10 classification. All patients who complete blood count tests in our hospital from January to December 2016 were included in the study. The identification of the patient's belonging to a specific nosological group according to the ICD-10 classification was carried out on the basis of the disease analysis code indicated in the direction of the blood test. 8056 patient records were included in the final analysis. Deviations beyond the upper reference range for the RDW indicator (> 14.5%) in this study were identified for patients of the following nosological groups according to ICD-10: C - neoplasms; D - blood diseases; S - injuries; T - poisoning. Significant intergroup differences were obtained according to the Kruskal-Wallis rank analysis of variance (Kruskal-Wallis test: H (19, n= 7622) = 214.9672 p = 0.0000). According to the results of this study, we can conclude that RDW has specificity for certain diseases (neoplasms; blood diseases; injuries; poisoning). In case of cardiovascular and other diseases, deviations of this indicator beyond the limits of the upper reference values were not found in this study.

    Abstract Aim of the study: to assess the relationship between Lp(a) serum levels grading and carotid artery stenosis (CAS). The Duplex Registry database was used for this study. CAS was verified by duplex scanning in the presence of an atherosclerotic plaque (AP), stenosing the lumen of the carotid artery (CA) by 20 % or more. Patients who underwent a blood test for Lp(a) and the results were entered into the registry database were selected for this study. The immunoturbidimetric method was used to determine the serum level of Lp(a) (mg/dl). Data from 51 patients (66.6 % men) were included in the final analysis: median age 49.0 [46.0; 59], total cholesterol (TC) 5.93 [5.13; 6.56], Lp(a) 26.5 [14.2; 76.0]. Spearman rank correlation analysis showed the presence of significant relationships (p < 0.05) between Lp(a) and age (r = 0.3), gender (r = 0.3), the presence of AP in the right ICA (r = 0.5), HDL (r = 0.3). OR and 95 % CI were calculated to determine the effect of Lp(a) grades on the probability of CAS detection: Lp(a) < 30 mg/dl OR 0.36 [0.11; 1.14] p = 0.04; Lp(a) > 30 mg/dl OR 1.42 [0.44; 4.58] p = 0.27. The prevalence of CAS in the group with Lp(a) level < 30 mg/dl was 33.3 %, 30–50 mg/dl – 50 %, 50–100 mg/dl – 40 %, > 100 mg/dl – 37.5 %. The model of multiple regression analysis for Lp(a) with TC in relation to the right ICA stenosis predicting showed R = 0.51, F = 8.4, p = 0.0007. The statistics of 3M model of the logistic regression function for CAS predicting based on the Lp(a) and TC data showed: -2 • log(likelihood) = 57.16, Chi-square = 8.17 (cc = 2), p = 0.016. The present study confirmed the relationship between the Lp(a) level and the CAS detection and the presence of an additive effect of total cholesterol on this. The reference role of Lp(a) gradation at the level of 30 mg/dl was determined as significant in relation to predicting CAS detection.

    Abstract This review analyzes existing approaches to the treatment of COVID-19 patients with antimalarial drugs from the standpoint of rational pharmacotherapy. Particular attention is paid to drug interactions and side effects of co-administration of chloroquine/hydroxychloroquine and azithromycin, considering the problem of cardiotoxicity and the high likelihood of cardiovascular disease in middle-aged and elderly people with COVID-19. The article discusses the prospects of using another representative from the group of antimalarial drugs - mefloquine in the treatment of COVID-19. Special attention is paid to the “proarrhythmogenic” side effects of azithromycin therapy, as well as to the problem of resistance of microorganisms to macrolides during antibiotic therapy of pneumonia, in particular to azithromycin. In conclusion, the article discusses the possibilities of alternative methods in the treatment of this disease.

    Abstract Introduction. Chronic heart failure (CHF) is recognized as one of the most severe cardiovascular diseases with a high mortality rate. Early identification of patients at risk of poor outcomes is critical to optimize treatment.Aim: to conduct comparative analysis of laboratory markers, including the erythrocyte heterogeneity index (RDW) in CHF patients of different functional classes (f.cl.) according to ICD-10 codes.Materials and methods. A retrospective cross-sectional study with an analysis of the electronic laboratory register (JanuaryDecember 2016). 8056 patients who underwent a clinical blood test and added CRP parameters were included. The determination of the patient’s belonging to CHF wascarried out according to the ICD-10 code indicated in the direction for analysis. Identification of CHF patients by f.cl. (NYHA) was carried out according to the ICD-10 codes: code I25.2 (Postponed myocardial infarction) – 1 f. cl.; code I11.0 (Hypertensive disease with predominant heart damage with heart failure) – 2–3 f.cl .; code I50.0 (Congestive heart failure) – 4 f.cl.Results. The proportion of patients with diseases of the circulatory system (code I) was 33.4% (n = 2686), of which 403 CHF patients were selected for the study. Distribution by f.cl. according to the code MKB-10: 1 ph.cl. – 0.83% (n = 67); 2–3 f.c. – 3.84% (n = 309); 4 ph.cl. – 0.33% (n = 27). Significant intergroup differences were obtained between ph.cl. (according to Kruskal–Wallis) for indicators: hemoglobin (H = 9.741111, p = 0.0077), erythrocytes (H = 7.176770, p = 0.0276), RDW indicator (H = 34.78287, p = 0.0000). There were no statistically significant differences in leukocytes, platelets, ESR, CRP.Conclusions. The presence of anemia in groups of patients with CHF of different f.cl., which often accompanies the disease, has not been confirmed. But statistically significant differences were revealed depending on the severity of f.cl. CHF according to the level of hemoglobin, the number of erythrocytes, and highly significant - according to the RDW indicator. The latter allows us to discuss the role of the RDW index as a possible new laboratory biomarker of CHF severity available for routine clinical practice.
