Title translation Lobomycosis in a Post-Covid 19 Patient: A Case Report and Review of Literature.
Abstract Aim: To document a case of lobomycosis and to discuss its epidemiology & diagnosis.
Case report: A 53-year-old male presented with a history of nasal congestion, nasal discharge, and epistaxis following Covid 19 infection. On physical examination, there was necrotic slough in the nasal vestibule near the inferior turbinate. Scrapings and punch biopsy were taken from the lesion. Hematoxylin-eosin-stained sections showed necrotic and mucoid areas with mixed inflammatory cell infiltration and numerous budding yeasts 3- 7μm diameter in singles, and small clusters with single narrow based budding as well as multiple budding including sequential budding forming "chains of yeasts". A diagnosis of Lobomycosis was made. Yeasts of lobomycosis are often confused with other yeasts such as P. brasiliensis, Candida spp., B. dermatitidis, and Cryptococci, but characteristic 'sequential budding' with a 'chain of yeasts" aid in the final diagnosis. Demonstration of yeasts with characteristic chains either in tissue sections or in potassium hydroxide (KOH) preparation of scraped material, exudate, or exfoliative cytology is the mainstay in the diagnosis as the organisms are uncultivable in vitro in culture medium.

Abstract Introduction: Many hepatic lesions which are detected clinically or by imaging technique are easily accessible to Fine Needle Aspiration Cytology (FNAC). Swift diagnosis by FNAC helps to save precious time and allow clinicians to duly plan treatment. Ultrasound guided FNAC of the liver is a safe, cheap, relatively non-invasive procedure with minimum complication and can be routinely done for evaluation of liver diseases. Aim: To evaluate hepatic lesions by ultrasound guided FNAC and classifying into inflammatory or non-inflammatory, benign or malignant, primary or secondary tumour and also to find novel cytological features. Materials and Methods: The present two-year descriptive crosssectional prospective study was carried out in the Department of Pathology, Karnataka Institute of Medical Sciences, Hubli. A total of 90 patients who satisfied the inclusion criteria during the two years study (August 2012 to July 2014) were assessed for clinical history and subjected to FNAC under ultrasound guidance. Descriptive statistical analysis was performed to calculate the percentage and frequency for the categorical variable included in the study. Results: Out of 90 cases, majority 73 cases (81.1%) were neoplastic and 11 cases (12.2%) were non-neoplastic and the remaining 6 cases (6.6 %) were inconclusive. Out of 73 cases, 4 cases were benign tumours and 69 cases were malignant tumours. Out of 69 cases, 32 cases were Hepatocellular Carcinoma (HCC), 32 cases were metastatic deposits, 3 cases were cholangiocarcinoma, one case was suggestive of malignancy. Among 33 cases of metastatic deposits, adenocarcinoma was most common (32 cases) and squamous cell carcinoma the least (1 case). The most consistent cytological findings in HCC were macronucleoli (32 cases) and trabecular pattern of arrangement (32 cases). The most consistent finding in metastatic adenocarcinoma was coarse clumping of nuclear chromatin (32 cases). Conclusion: The study concludes that the ultrasoundguided FNAC of the liver is a safe, simple, ...

Abstract Chondroblastoma of squamous part of temporal bone is a very rare bone tumor. Although most of them are benign, the prognosis is not predictable. A 14-year-old girl presented with recurrent slowly growing mass over the right side of the temporo-parietal region of skull vault. Initial curettage material showed extensive chondroid areas and diagnosed histologically as "enchondroma." Histology of completely excised recurrent mass showed identifiable chondroblastic foci. She was followed up for 3 years and was free from recurrence. Chondroblastomas at very rare sites such as squamous part of temporal bone have variable histology, confusing with other giant cell lesions, variable prognosis, and require prompt diagnosis and complete excision.

Abstract Background: Diagnosis of chromoblastomycosis is frequently missed for many reasons: (1) rarity of the lesion (2) requirement of careful search for diagnostic 'sclerotic' bodies which are often sparse in clinical material such as tissue and exudates (3) often they elicit tissue reactions such as verrucous lesion and micro abscesses, misleading the diagnosis (4) lack of 'clinical suspicion'.
Aims: (1) To compare the feasibility of 'unstained', 'stained' and 'de stained' sections in identification of the diagnostic ' sclerotic' bodies (2) To study histopathological features of chromoblastomycosis, and (3) To highlight the importance of clinical suspicion in the diagnosis of chromoblastomycosis.
Design: This is a retrospective study conducted on six clinically unsuspected, histopathologically diagnosed cases of chromoblastomycosis.
Results: Most common clinical presentation was verrucous lesion over extremities affecting adult men engaged in outdoor works. Sclerotic bodies and their characteristic features were easily identified in both 'unstained' and 'de stained' sections. Special stains such as Fontana Masson and Gomori's methenamine silver nitrate are useful for demonstrating hyphal forms in keratinized layers and for illustration purposes.
Conclusion: Both 'unstained' and 'de stained' sections can be used for rapid screening of sclerotic bodies. 'De stained' sections can be used as a suitable substitute for special stains for confirmation and for retrospective review of all verrucous lesions to diagnose the missed lesion. Clinical suspicion is very important in the diagnosis of chromoblastomycosis as it helps pathologist to screen for diagnostic sclerotic bodies.
