    Title translation Molecular and cytogenetic prognostic factors in acute myeloid leukemia (AML).
    Abstract Acute myeloid leukemia (AML) constitutes a group of diseases that are very heterogeneous with regard to clinical course, response to therapy as well as cytogenetic aberrations and gene mutations. Such lesions are of prognostic value. Patients with t(8;21), inv(16)/t(16;16) or t(15;17) have a favorable prognosis. Patients with normal karyotype and aberrations +6, +8 –Y, t(9;11) and del(12p) are classified in the group of intermediate prognosis. In the case of patients with complex karyotype or with the aberrations inv(3)/t(3;3), t(6;9), –5, –7, del(5q), del(7q) and 11q23 rearrangements, the prognosis is poor. Unfavorable molecular factors include C-MYC amplification, MLL amplification and rearrangement, FLT3-ITD, WT1 mutation and overexpression of BAALC, ERG or MN1. The changes in miRNA expression may also be important for AML prognosis. That is why the cases with normal karyotype (CN-AML) and cases with complex aberrations remain to be better characterized at the genetic level. Array technology enables the analysis of genomic DNA and gene expression. This approach may be used in the search for new prognostic and predictive markers in AML.

    Abstract Ostre białaczki szpikowe (OBS) stanowią niejednorodną grupę chorób, zróżnicowaną pod względem przebiegu, odpowiedzi na terapię oraz zmian cytogenetycznych i molekularnych. Zmiany te mają znaczenie prognostyczne. Korzystnie rokują chorzy z t(8;21), inv(16)/t(16;16) i t(15;17). Chorzy z prawidłowym kariotypem i aberracjami: 6, 8, –Y, t(9;11) i del(12p) należą do grupy rokowania pośredniego. W przypadku chorych ze złożonym kariotypem lub zmianami typu: inv(3)/t(3;3), t(6;9), –5, –7, del(5q), del(7q), rearanżacje 11q23 – rokowanie jest niekorzystne. Do niekorzystnych molekularnych czynników prognostycznych zalicza się: amplifikację [i]C-MYC[/i], amplifikacje i rearanżacje [i]MLL[/i], [i]FLT3-ITD[/i], mutację [i]WT1[/i], nadekspresję BAALC, [i]ERG[/i] i [i]MN1[/i]. Mutacje genów CEBPA i NPM1 uznaje się za korzystne czynniki prognostyczne. Istotne rokowniczo w OBS są także zmiany ekspresji niektórych miRNA.Heterogenność OBS uzasadnia potrzebę prowadzenia badań cytogenetycznych i molekularnych w celu określenia zmian m.in. u chorych z prawidłowym kariotypem w klasycznym badaniu cytogenetycznym. Techniki macierzowe umożliwiają analizę genomowego DNA oraz profilowanie ekspresji genów w poszukiwaniu sygnatur prognostycznych i predykcyjnych.

    Title translation Some genomic aberrations in B-cell chronic lymphocytic leukemia and their clinical relevance. Part II. Trisomy 12 in B-cell chronic lymphocytic leukemia detected by fluorescence in situ hybridization.
    Abstract Among 75 untreated patients with typical (CD19+, CD5/CD19+, CD23/CD19+) B-cell chronic lymphocytic leukemia (B-CLL) cytogenetic aberrations of peripheral blood cells were evaluated, using fluorescence in situ hybridization techniques. Two cytogenetic aberrations were evaluated: trisomy 12 and TP53 deletion. The clonality was determined when > or = 10% of the cells had of trisomy 12 or deletion TP53 gene. Trisomy 12 in 7 patients was detected, while trisomy 12 and TP53 deletion simultaneously in 6 patients were present. If the first group will be linked to the second one then 13 patients among 75 (17%) will have trisomy 12. In group of patients with trisomy 12 and TP53 deletion percentage of cells with trisomy 12 was almost two time more compare to patients with trisomy 12 as a single aberration. It is possible, that TP53 deletion ("the guardian of the genome") facilitates proliferation clones with others genomic aberrations. In two patients with trisomy 12 control cytogenetic study was performed. Increase of percentage cells with trisomy 12 for 8% and 30% respectively was detected. However, proliferation of cells with TP53 deletion was observed too. Clinical course of B-CLL in group of patient with trisomy 12, trisomy 12 and TP53 deletion simultaneously is more aggressive compared to the course of disease of patients with no cytogenetic aberrations (patients of Group I from Part I of paper). Frequency of IGHV gain mutation occurrence was not analyzed in both groups of patients. But trisomy 12 together with unmutated IGHV gene is found by some authors. The absence IGHV gene mutation is independent unfavourable prognostic factor.

    Title translation In vitro studies directed at inducing differentiation of leukemic B-lymphocytes.
    Abstract Lipopolysaccharide (LPS) and phorbol esters (TPA) stimulate lymphocytes proliferation in two different ways. While LPS primary function is specific receptor binding, TPA directly activate cellular protein kinase C. The stimulation of human leukaemic lymphocytes (from chronic lymphocytic leukaemia patients) with LPS and TPA results in two different types of response: to both stimulators, and to LPS only. Therefore the supposed defect of cellular receptors can not explain all the observed effects. The existence of TPA independent second messengers and changes in signal transduction pathways downstream of PKC can be considered.

    Abstract In a group of 75 untreated patients with a typical B cell chronic lymphocytic leukaemia (B-CLL) (CD19+, CD5/CD19+, CD23/CD19+), the frequency and clinical significance of TP53 gene deletion and chromosome 12 trisomy were assessed. The studies of peripheral blood lymphocytes were conducted using interphase in situ hybridization technique. Clonality was identified when TP53 deletion or chromosome 12 trisomy was found in at least 10% of cells. From all 75 examined patients 32 individuals without any of the genetic aberrations were analyzed (Group I) and 30 subjects with TP53 deletion (Group II) were chosen. In the other 13 patients, discussed in the next paper, either chromosome 12 trisomy (Group III--seven subjects) or both chromosome 12 trisomy and TP53 deletion (Group IV--six subjects) were found. In the Group I, there has been no further contact with three patients, while in the Group II--with two individuals. In the Group I, one patient of 29 in the study (3%) died after 84 months (seven years) from the diagnosis, whereas in the Group II--nine subjects of 28 in the study (32%) died within 1-36 months from the diagnosis. In three of those patients in the terminal condition, cytogenetic studies were repeated revealing an increase of approximately 5% in the percentage of peripheral blood cells with TP53 deletion. The frequent presence of TP53 deletion detected in 48% of patients is surprising. It is generally thought that the aberration is found in 10-15% of clinical cases. The studies should be confirmed on a larger group of patients.

    Abstract Harmful Internet use (HIU) describes unintended use of the Internet. It could be both self-harm and harming others. Our research goal is to develop a more accurate method for measuring HIU by this novel peer assessment. As such, it may become, with our call for more research, a paradigm shift supplementing every rating scale or other type of Internet use assessment. In addition to classic statistical analysis, structural equations have been employed. Results indicate that the true positive rate (TPR) is substantially higher than assessed in other studies.•Peer assessment improvement.•AUC for ROC was computed to establish cut-off points for the used scale.•Results obtained by the Structural Equation model indicate that parental care has a moderate influence on subjects’ attempts to fight HIU.

    Abstract Background: Cancer is one of the leading causes of death among Polish women in general, and first in women aged 25−64. Contributing to this cancer burden are modifiable behavioral risk factors, including low utilization of cancer screenings. Poland has an urgent need for new systemic solutions that will decrease cancer burden in the female Polish population. This study examined the United States’ implementation of preventive wellness visits as a viable solution for implementation in Poland. Methods: Health insurance claims data for nearly three million women in five states of the U.S. were examined to identify use of mammograms, colorectal cancer screening, and lung cancer screening. Three subgroups of the cohort were assessed for the probability of receipt of screening associated with type of healthcare visit history (women with wellness visits—W; with wellness visits and related preventive services and screenings—W+P; and control group—C). All multiple comparisons were significant (alpha = 0.05) at p < 0.0001, except comparison between subgroups (W vs. P+W) for lung cancer screening. Results: Breast and colorectal cancer screenings had substantially higher participation after W and W+P in comparison with C; moreover, a slight increase after W or P+W was seen for lung cancer as well. Conclusions: Results indicate that wellness visits are an effective tool for increasing cancer screening among women in the U.S. Introduction of a similar solution in Poland could potentially help produce higher screening rates, address cancer prevention needs (not only for secondary cancer prevention), and lower cancer burden.
