    Abstract Background: In Xuan Wei, China, the lung cancer mortality rate is rising significantly more than that of the nation overall. However, it remains unclear 1) if improved diagnosis can just partially explain this observation and how other local risk factors may be correlated with the lung cancer mortality rate and 2) how the lung cancer mortality rates differ within Xuan Wei and how these spatiotemporal patterns are linked with local risk factors. To increase etiological knowledge, this study evaluated the spatial and temporal distributions of the health effects (the lung cancer mortality rates) from 2011 to 2015.
    Methods: Four steps of spatial analysis were applied, as follows: 1) hotspot analysis to determine the geographical patterns of lung cancer mortality, 2) spatially-weighted sum to identify areas with higher health risks, 3) bivariate statistical analysis to assess the overall correlation between coal mines and lung cancer mortality, and 4) geographically-weighted regression to test for correlations among different towns within Xuan Wei.
    Results: Women had higher lung cancer mortality rates than those in men, with an increasing trend in both sexes over time. The incidence rates in Laibin Town were the highest in Xuan Wei every year. Over the 5-year study period, the lung cancer mortality was increasingly concentrated in Laibin, Shuanglong, and Longchang, where the smoky coal mines are most concentrated. The population-level health risks from the coal mine in Xuan Wei were mapped and divided into five types of risk areas (Type I - Type IV). Correlation analysis revealed that there was no significant correlation between lung cancer mortality as a whole and coal mine distribution during the 5-year study period. However, the geographically-weighted regression revealed a stronger correlation in medium (Type III) and second-lowest (Type IV) health risks.
    Conclusions: Xuan Wei lung cancer mortality has increased continuously since the third national retrospective surveys on the causes of death by the Ministry of Health of the People's Republic of China (2004-2005), especially for local women and residents over 35 years of age. Geographically, lung cancer in Xuan Wei showed unique spatiotemporal clustering. The local lung cancer mortality was significantly correlated with the smoky coal mine geographically. Some specific towns (Laibin, Shuanglong, and Longchang) within Xuan Wei manifested high correlations between lung cancer mortality and coal mines. The effects of coal mines on lung cancer mortality rates also spread geographically outward from these areas. Public health concern regarding lung cancer in Xuan Wei should prioritize higher-risk towns surrounded by smoking coal mines. Intervention strategies for particular toxic coal types require further studies on their chemical characteristics and mechanisms of carcinogenesis. Additional studies are also warranted to systematically examine the local environmental health risks related to coal industries and combustion air pollution and eventually to conduct early screening of lung cancer for local people who are more exposed to smoky coal in high-risk areas.

    Abstract Ethnopharmacological relevance: Aqueous extract of Qu Feng Xuan Bi Formula (QFXBF, a Chinese herb formula) which composed of Radix Glycyrrhizae, Radix Glycyrrhizae Preparata, Paeonia sterniana Fletcher in Journ, Pheretima, Allium macrostemon Bunge, Astragalus membranaceus (Fisch) Bunge and Divaricate Saposhnikovia Root has been used in treatment of allergic rhinitis and asthma (ARA) as an approved hospital prescription for many years in Jiangsu Province Hospital of Traditional Chinese Medicine, China. The present study was designed to investigate the effect of the aqueous extract of QFXBF in the gene expression of Toll-like receptor 9 (TLR9) and the manners of immune modulation of T cell-associated interleukin (IL-4 and IL-13) in rat ARA models.
    Materials and methods: Fifty SD male rats were divided into five groups: not treated group, OVA only group (treated only with OVA), dexamethasone (DXM) group, low dose QFXBF group and high dose QFXBF group randomly (n=10 per group). Rat allergic rhinitis and asthma model was developed by ovalbumin (OVA) sensitization and nose infusion. Pathological changes of nasal tissue and lungs were examined by H&E staining. Gene expressions of TLR9, Stat 3, Jak-1 and C-Jun in nasal tissue were assayed by real-time polymerase chain reaction (RT-PCR). The serum and broncho-alveolar lavage fluid (BALF) levels of T cell-associated interleukin (IL-4 and IL-13) were determined by enzyme-linked immunosorbent assay (ELISA).
    Result: The ARA model was successfully established. Marked EOS count was observed in BALF from ARA models. The aqueous extract of QFXBF could reduce EOS levels and increase TLR9 expression, but did not affect the gene expression of Stat-3 and Jak-1 and C-Jun. The reduction of IL-13 concentration in serum from high dose QFXBF group was observed in BALF, albeit not significantly. Despite the not treated group, serum levels of IL-4 had significantly increased in other four groups (P<0.001, n=4-6) but made higher in low dose QFXBF group and DXM group (P<0.05, n=4-6).
    Conclusion: This study originally provides the evidence that the aqueous extract of Qu Feng Xuan Bi Formula alone is effective in the treatment of anaphylactic rhinitis-asthma symptoms. The extract of Qu Feng Xuan Bi Formula was effective to reduce the eosophil recruitment to the lung. In addition it increased the IL-4 concentration in the BALF and expression of TLR9 in the nasal tissue. No alteration was observed in the IL-13 concentration in the BALF and expression of STAT-3, JAK-1 and C-Jun in nasal tissue. The results thereby scientifically provided mechanism of these aqueous extract of QFXBF in improvement of ARA and supported its clinical use.

    Abstract Parts of Xuan Wei County, Yunnan Province, China have the highest known lung cancer mortality in nonsmoking women. This high mortality displays a clear spatial relationship to the mines producing coal from the uppermost Permian. Geochemical, petrographic, and grainsize analyses were undertaken on a set of coal samples from Xuan Wei. Results demonstrate that the single geochemical property that makes this coal unusual is its high concentration of quartz (13.5 wt %) of which 35-55% occurs as <10 microm grains. We propose the potential for silica-volatile interaction (PSVI) as a new method for assessing the combined influence of silica and volatile organic matter and use this as a basis for re-evaluating existing ecological data. Published lung cancer mortality values are more strongly correlated with PSVI values for Xuan Wei coal than with volatiles or silica alone and the PSVI values measured are distinct from those of other coals. Finally we propose that the localization of this epidemic to Xuan Wei results from enhanced weathering of the local Emeishan basalts as a consequence of geochemical perturbations at the Permo-Triassic Boundary.
