    Abstract Lumbar spinal stenosis refers to the narrowing of the spinal canal in the lumbar region. There is an increasing need to determine the treatment modality for lumbar spinal stenosis by comparing the outcomes of X-stop interspinous distractors and laminectomy. The objective of this study is to determine the effectiveness of the X-stop interspinous distractor compared to laminectomy. This systematic review fundamentally abides by the procedures delineated in the Cochrane methodology while the reporting is done according to the Preferred Reporting Items for Systematic Review and Meta-Analyses guidelines. Three databases searched generated a total of 943 studies, with PubMed being the source for the bulk of the articles. Six studies were selected for inclusion in this study. The effectiveness of the interspinous distractor devices and laminectomy can be determined through their impact on the quality of life, rates of complications, and the amount of money utilized. This meta-analysis fundamentally emphasizes that laminectomy is a more effective intervention for the treatment of lumbar spinal stenosis as it is more cost-effective and results in fewer complications in the long term.

    Abstract Background: First characterized in the 19
    Case description: Herein, the authors present two case examples describing the clinical presentation of SSEH, while also reviewing the literature to provide a comprehensive overview of its presentation, pathology, and treatment. The first case is a patient with nontraumatic sudden onset neck pain with rapidly progressing weakness. The second case is a patient with painless weakness that developed while taking 325 mg of aspirin daily.
    Conclusion: Clinicians should keep SSEH in their differential diagnosis when seeing patients with nontraumatic sources of weakness in their extremities. The appropriate steps should be followed to diagnose and treat this condition with magnetic resonance imaging and surgical decompression if there are progressive neurological deficits. There is a continued need for more extensive database-driven studies to understand better SSEHs clinical presentation, etiology, and ultimate treatment.

    Abstract Cardiovascular disease has affected a large percentage of the world, and as a result, we have had major advancements in pharmacological and procedural intervention of this disease. With the increased burden of rising healthcare costs, alternative treatment with exercise has shown to be much more cost effective and just as beneficial to patients compared to pharmacological and procedural treatment. We highlight some of the major mechanisms behind the beneficial effect of exercise on atherosclerosis and hope to encourage patients and providers to attempt to adopt this form of treatment that has not only shown to be beneficial to heart disease, but diseases such as diabetes and obesity as well.

    Abstract Background: Pneumorrhachis (PNR) is the presence of air within the spinal canal and may be either intramedullary or extramedullary in location. The etiology is most commonly iatrogenic or traumatic in nature. Treatment is dependent on underlying cause and physical exam.
    Case description: Herein, we describe the second case in the literature of spontaneous holocord PNR in a young patient without risk factors. A 22-year-old male with no past medical history presented to the hospital for 2 days of vomiting and cramping in his hands and feet secondary to severe dehydration. He recently started a new job as a manual laborer and had to leave work early 2 days prior due to overexertion working outside in heat ranging from 100 to 120 degrees Fahrenheit. CT abdomen and pelvis demonstrated spontaneous pneumomediastinum and extramedullary PNR extending upward from L3 throughout the thoracic spine to the upper limit of the scan. Subsequent CT cervical and thoracic spine showed the full length of the extradural air from C2-T12 and again at L3.
    Conclusion: Spontaneous PNR is an uncommon, typically self-limited condition in which air is introduced into the spinal axis. Anatomic predisposition makes the extradural, dorsal cord in the cervicothoracic region the most common location. Patients are rarely symptomatic, and treatment is supportive in nature once secondary causes with high rates of morbidity and mortality are ruled out.

    Abstract Coronal imbalance is a type of spinal deformity with deviation from midline in the coronal plane. It is challenging to correct even in the hands of experienced spine surgeons. Many conventional techniques lead to unsuccessful results or complications. However, the incorporation of "kickstand rod" (KR) instrumentation is now understood to provide a more supported coronal correction and improve spinal deformities. Sometimes it can be used to provide additional spinal support in instances where spinal fusion has already occurred. The KR is placed from a posterior approach along the lateral spine from lumbar spine to ilium and exerts distraction forces that counteract misaligned spinal segments. Our objective is to present a clinical case example with a brief review of literature. Herein, we present a case of a 62-year-old male with the development of significant coronal imbalance following his posterior lumbosacral instrumentation and fusion 11 years prior to presentation. KR supplementation to his hardware improved his functional outcome significantly. Further, we provide a literature review of the surgical characteristics, indications, and functional outcomes of KR instrumentation. A term search of "kickstand rod" was performed in PubMed, and relevant English language publications were included. The literature search yielded only six publications. A total of 45 patients across three studies were assessed. A mean postoperative coronal balance magnitude of 26.83 mm was calculated compared to the preoperative coronal magnitude of 64.16 mm. Results also showed only four cases of intraoperative or postoperative complications. Moreover, the presented case reported successful KR implementation without any intraoperative complications. KR instrumentation is a safe and effective technique for coronal imbalance correction. The results show favorable outcomes in terms of coronal adjustment and low complication rates. Nevertheless, we caution the fact that further studies are warranted with long-term follow-ups.
