    Abstract The trajectory of Colonel (R) Juan Benigno Castañer Moreno, a tireless fighter who has dedicated his life to medicine and the Revolution, is outlined. His investigative work was explored through a documentary review, in his employment record, a search for information available on the internet, his works and an interview carried out by the authors, with which it was possible to specify important passages of such long life. With this work, the authors intend to disseminate the main achievements of Professor Castañer's work, scientific and teaching career as well-deserved recognition of his life's work.

    Abstract Se caracterizó la composición y estructura arbórea del sitio Solteros de Juan Rosas, Papantla, Ver. Para ello se establecieron siete transectos de 50m x 20m, distribuidos al azar, utilizando la metodología de Gentry (1982) contando individuos con un DAP mayor o igual a dos punto cinco centímetros, se registraron un total de 87 individuos, con una riqueza de 11 especies, distribuidas en siete familias y 11 géneros. La diversidad alfa (α) estimada mediante el índice de Shannon Wiener dio un valor de H= 1.872, indicando la baja diversidad presente en este ecosistema. La diversidad verdadera para el transecto seis fue mayor comparada con los otros transectos, con un valor de 3.6840, siendo el más diverso.

    Abstract El municipio de San Juan de Rioseco, perteneciente a la provincia de Magdalena Centro, basa su economía en actividades agrícolas de pequeña escala, renglón que representa la segunda industria aportante al PIB del municipio. La gestión de actividades agrícolas tiene su centro en procesos asociativos que buscan mejorar las condiciones de los pequeños productores asociados; sin embargo, el municipio presenta indicadores de competitividad bajos en relación con otros municipios del departamento. Con este marco de referencia se realiza el presente estudio de investigación, que tiene como objetivo analizar las dinámicas de las organizaciones solidarias presentes en el municipio, teniendo como base los elementos de caracterización general de las organizaciones, estructura organizacional yalianzas estratégicas. La metodología de investigación establecida fue de diagnóstico rural participativo, por medio de métodos interpretativos y herramientas de tipo mixto en tres fases establecidas, tomando como muestra cuatro de las trece asociaciones que se encuentran constituidas en la actualidad en el territorio. Como resultados principales del proceso investigativo, se generó la cartografía social de las organizaciones solidarias, en donde se identifican diecisiete organizaciones establecidas en el municipio. El segundo resultado fue la identificación de las características organizacionales y administrativas y su influencia sobre la gestión y cumplimiento de las metas organizacionales y productivas de las organizaciones. El tercer resultado presenta el análisis de las redes y articulación interorganizacional en San Juan y su influencia en los procesos de competitividad y gestión de las asociaciones.

    Abstract Abstract The clinical and biochemical findings in a cohort of 51 patients with urea cycle disorders followed at the Hospital Garrahan, Buenos Aires, Argentina were analyzed at the time of diagnosis (3 female patients were excluded). Of this cohort, 13/48 patients had early-onset (EO), 23/48 had late-onset (LO), and 12/48 had a different presentation because they had a family risk background (FRB) and had been diagnosed since they were born. The most frequent deficiency disorder was OTCD (65%). The initial ammonium value was evaluated, being higher in the EO group, with a statistically significant difference when compared with LO and FRB. 15/48 patients fell into a coma at the time of diagnosis, mean ammonia was 829.54 μmol / L, and 33/48 did not fell into a coma, the mean ammonium was of 159.3 μmol / L (p = 0.001). 15 patients died: 62% EO, 22% LO (p=0.0216), 17% FRB. A molecular study was performed on 35 patients. Patients with EO presentation suffer the most severe forms and still have high morbimortality. On the other hand, LO forms are forms of less severity that are finally diagnosed as a result of one or more acute episodes.

    Abstract The Juan Fernandez Ridge, a vulnerable marine ecosystem located far off the coast of central Chile and formed by several seamounts, guyots and three islands (Robinson Crusoe, Santa Clara and Alejandro Selkirk), has recently been declared a Coastal Marine Protected Area of Multiple Uses with several National Parks embedded in it. Recent studies have highlighted the influence of remote and local oceanographic structures on the hydrographic dynamics of this ridge. However, there is still a gap in understanding how they affect the structure and dynamics of the surrounding insular planktonic communities. A hydroacoustic and oceanographic survey was conducted during the austral spring (October 2016), including hydrographic and zooplankton sampling around Robinson Crusoe Island. Oceanographic features were identified and tracked using satellite data (chlorophyll‐a (Chl‐a) and sea surface temperature) and modelling results. Two events of Chl‐a increase relative to a threshold (>0.45 mg m⁻³) were forced by different physical processes, both affecting the western side of Robinson Crusoe Island. In event A during the cruise period, Chl‐a subsurface maxima were associated with the arrival of a coastal meander originating on the continental shelf off Chile (remote process); the zooplankton was dominated by copepods and salps, with an evident coastal–oceanic gradient. In event B, Chl‐a maxima were linked to a local upwelling forced by the intensification of a localized SSW wind. No influence of remote eddies or local Von Kármán vortices on Chl‐a distribution was observed. These findings highlight the influence of remote and local physical processes on the structure of planktonic communities around Robinson Crusoe Island. Understanding the variability of these mechanisms and their effects at the base of the pelagic food web is critical in adopting an ecosystem‐level approach.

    Abstract Global breast cancer incidence varies considerably, particularly in comparisons of low- and high-income countries; rates may vary even within regions. Breast cancer rates for Caribbean countries are generally lower than for North America and Europe. Rates in Puerto Rico are in the middle of the range between the highest and the lowest Caribbean countries. Populations in transition, with greater variability in risk factor exposures, provide an important opportunity to better understand breast cancer etiology and as potential sources of variation in rates. Understanding of exposures across the life span can potentially contribute to understanding regional differences in rates. We describe here the design and implementation of a population-based, case-control study in the San Juan Metropolitan Area (SJMA) of Puerto Rico, the Atabey Epidemiology of Breast Cancer Study. We describe steps taken to ensure that the study was culturally appropriate, leveraging the Atabey researchers' understanding of the culture, local health system, and other required resources to effectively recruit participants. A standardized, in-person interview was developed, with attention to life course events customized to the study population. In order to understand variation in global breast cancer rates, studies customized to the populations outside of North America and Europe are required.

    Abstract Background: A survey was conducted to evaluate the effect that the current medical liability environment has had on physicians in San Juan, Puerto Rico.
    Methods: In August 2008 a questionnaire consisting of 15 items which evaluated medical liability was mailed to all physicians in the San Juan District. The questionnaire requested information on basic demographics, medical specialty, insurance coverage, changes made in medical practice to decrease medical liability risk, malpractice claims received during the last ten years, and whether awards exceeding insurance coverage were paid. Physicians were requested to return the completed questionnaires by FAX or mail.
    Results: There were 951 completed questionnaires returned. The survey indicated that 70% of physicians have made changes in their medical practices to decrease medical liability risk. Currently, 48% of physicians do not accept emergency cases and 50% do not accept high risk patients. Malpractice claims were received at least once in the previous ten years by 40% of the physicians. The awards requested by these claims exceeded the insurance coverage 75% of the time. After the final court decision was reached, 20% of the payments exceeded physicians' insurance coverage.
    Conclusions: The adverse medical liability environment has affected the majority of physicians in San Juan, leading 70% of them to make changes in their practices to decrease the liability risk. As a result of these changes, the number of physicians willing to care for high risk and emergency patients has decreased significantly.

    Abstract With a few months in between we have come to know two panels depicting Saint James at the battle of Clavijo that may relate to the Reinaissance production from Toro, whose iconography is practically unprecedented in this pictorial context. Here we study the panel at the Museum of Pilgrimages in Santiago, examining the formal and stylistic features that lead us to propose to attribute it to the painter Juan de Borgoña de Toro, a disciple and collaborator of Lorenzo de Ávila, as we provide a short commentary on the profiles of these two artists.
