    Abstract Gabbroic xenoliths and diverse megacrysts (e.g., clinopyroxenes, amphiboles and plagioclases), which correspond to the lithology ranging from gabbro-norite to gabbro, occur in the Pleisto-Holocene alkali basalts from Jeju Island, South Korea. The gabbroic xenoliths consist primarily of moderate-K2O plagioclase, Ti-Al-rich clinopyroxene and CaO-rich orthopyroxene; additionally, TiO2-rich amphibole (kaersutite) and Ti-Fe oxides might or might not be present. The plagioclase is the most dominant phase (approx. 60-70 vol.%). The xenoliths and megacrysts provide evidence for the modal metasomatism of the lower continental crust by the mafic magmas during the Pleistocene. The coarse grain size (up to 5 mm), moderate Mg# [=100xMg/(Mg+Fe(total)) atomic ratio] of pyroxenes (70-77) and textural features (e.g., poikilitic) indicate that the gabbroic xenoliths are consistent with a cumulus origin. The clinopyroxenes from these xenoliths are enriched in REE with smooth convex-upward MREE patterns, which are expected for cumulus minerals formed from a melt enriched in incompatible trace elements. The strikingly similar major and trace element variations and the patterns of constituent minerals clearly indicate a genetic link between the gabbroic xenoliths (plus megacrysts) and the host basalt, indicating that the xenoliths belong to the Jeju Pleisto-Holocene magma system. On the basis of the textural features, the mineral equilibria and the major and trace element variations, the xenoliths appear to have crystallized from basaltic melts at the reservoir-roof environment within the lower crust (4-7 kbars) above the present Moho estimates beneath Jeju Island, where the xenoliths represent wall rocks. Following the consolidation of the xenolith lithologies, volatile- and incompatible element-enriched melt/fluid, as metasomatic agents, infiltrated through the grain boundaries and/or cracks and reacted with the preexisting anhydrous phases, which produced the metasomatic amphiboles. This volatile-enriched melt/fluid could have evolved from the initially anhydrous compositions to the volatile-saturated compositions by the active fractional crystallization in the Jeju Pleisto-Holocene magma system. This process was significant in that it was a relatively young event and played an important role in the formation of the hydrous minerals and the metasomatization of the lower continental crust, which is a plume-impacted area along the Asian continental margin.
    The major and trace element analyses of the mineral phases from the xenoliths were performed to define the principal geochemical characteristics of the crustal lithosphere segment represented by the studied xenoliths.

    Abstract Objective: To study the clinical and radiologic factors related with overall patient satisfaction of joint scarifying reconstruction on severe rheumatoid forefoot deformity (RFD).
    Methods: Forty cases of RFD were retrospectively enrolled. A questionnaire on the factors for patient's expectations and satisfactions of the greater and lesser toes was administered, including repression of relapse in deformity (D), pain reduction (P), improvement in shoe wearing (S), barefoot activity (B), and appearance (A). Overall satisfaction were assessed using the 5-digit-scale. Hallux valgus angle, 1, 2 intermetatarsal angle, and other radiologic parameters were measured. Pearson's correlation and multiple linear regression analyses were used to evaluate the relationships between these factors and overall satisfaction.
    Results: Overall satisfaction was 4.0±0.82. Postoperative radiologic parameters were corrected in adequate range. Visual analog scale (VAS) was reduced from 7.2±2.1 to 2.2±1.8. For the greater toe, patient's expectations (D, P, S, B, and A) were 4.2, 4.1, 3.0, 2.5, 2.7 and satisfactions were 4.2, 4.0, 3.4, 3.5, 3.3, respectively. For the lesser toes, patient's expectations (D, P, S, B, and A) were 3.9, 4.1, 3.4, 3.0, 2.8, and satisfactions were 3.4, 4.0, 3.4, 3.6, 2.9, respectively. Satisfactions with P and B, and reduction amounts of VAS were significantly correlated with overall satisfaction.
    Conclusion: Although forefoot reconstruction with a joint sacrificing procedure is non-physiological, it could be a good surgical option for severe RFD. Each patient's expectations and satisfactions with this procedure could vary. Thus, it seems important to inform patients preoperatively that expectation could be fulfilled well or less.

    Abstract Background: Macrolide antibiotics are widely used to treat various infections such as pneumonia and sinusitis, and previous exposure to macrolides is presumed to be a risk factor for standard triple therapy failure in Helicobacter pylori (H. pylori) eradication. We aimed to determine whether previous use of macrolide antibiotics could affect clarithromycin resistance of H. pylori.
    Materials and methods: From the Korea National Health Insurance Service (NHIS2021-1-775) database, a total of 46,160 patients who were tested for clarithromycin resistance of H. pylori from 2016 to 2019 in Korea were identified. Their history of antibiotics in the past 10 years and history of respiratory comorbidity in the past 1 year were investigated.
    Results: Clarithromycin resistance rate of H. pylori in Korea was 16.2%. A multivariate analysis revealed that female sex (OR: 1.472, p < .001), age > 50 years (OR: 1.340, p < .001), previous use of macrolide antibiotics (clarithromycin, OR: 2.902, p < .001; azithromycin, OR: 1.930, p < .001; erythromycin, OR: 2.060, p = .001; roxithromycin, OR: 2.022, p < .001), and history of respiratory comorbidity (sinusitis, OR: 1.271, p < .001; laryngopharyngitis, OR: 1.135, p = .032; bronchitis, OR: 1.245, p = .001; pneumonia, OR: 1.335, p = .026) were independent risk factors of clarithromycin resistance in H. pylori.
    Conclusions: The use of macrolide antibiotics and a recent diagnosis of respiratory disease might increase clarithromycin resistance of H. pylori.

    Abstract Background: Oral potentially malignant disorders (OPMDs) are associated with an increased risk of cancer of the oral cavity including the tongue. The early detection of oral cavity cancers and OPMDs is critical for reducing cancer-specific morbidity and mortality. Recently, there have been studies to apply the rapidly advancing technology of deep learning for diagnosing oral cavity cancer and OPMDs. However, several challenging issues such as class imbalance must be resolved to effectively train a deep learning model for medical imaging classification tasks. The aim of this study is to evaluate a new technique of artificial intelligence to improve the classification performance in an imbalanced tongue lesion dataset.
    Methods: A total of 1,810 tongue images were used for the classification. The class-imbalanced dataset consisted of 372 instances of cancer, 141 instances of OPMDs, and 1,297 instances of noncancerous lesions. The EfficientNet model was used as the feature extraction model for classification. Mosaic data augmentation, soft labeling, and curriculum learning (CL) were employed to improve the classification performance of the convolutional neural network.
    Results: Utilizing a mosaic-augmented dataset in conjunction with CL, the final model achieved an accuracy rate of 0.9444, surpassing conventional oversampling and weight balancing methods. The relative precision improvement rate for the minority class OPMD was 21.2%, while the relative [Formula: see text] score improvement rate of OPMD was 4.9%.
    Conclusions: The present study demonstrates that the integration of mosaic-based soft labeling and curriculum learning improves the classification performance of tongue lesions compared to previous methods, establishing a foundation for future research on effectively learning from imbalanced data.

    Abstract Background: A force-based tension protocol that uses a certain amount of tension at graft fixation could still give rise to variations in initial constraint levels of the knee joint in terms of side-to-side difference (SSD) in anterior translation.
    Purpose: To investigate the factors influencing the initial constraint level in anterior cruciate ligament (ACL)-reconstructed knees and compare outcomes according to the level of constraint in terms of anterior translation SSD.
    Study design: Cohort study; Level of evidence, 3.
    Methods: Included were 113 patients who underwent ipsilateral ACL reconstruction using an autologous hamstring graft and had minimum 2-year follow-up outcomes. All grafts were tensioned and fixed at 80 N using a tensioner at the time of graft fixation. The patients were classified into the following 2 groups according to the initial anterior translation SSD, measured using the KT-2000 arthrometer: a physiologic constraint group with restored anterior laxity ≤2 mm (group P; n = 66) and a high-constraint group with restored anterior laxity >2 mm (group H; n = 47). Clinical outcomes were compared between the groups, and preoperative and intraoperative variables were evaluated to identify factors affecting the initial constraint level.

    Conclusion: Greater anterior translation measured in the contralateral knee was an independent predictor of a more constrained knee after ACL reconstruction.     Abstract This study aimed to examine change in the graft bending angle (GBA) according to various knee flexion angles in creating femoral tunnel by the transportal technique in ACL reconstruction, and to reveal knee flexion angle minimizing GBA while maintaining stable femoral tunnel characteristics. Patients who underwent ACL reconstruction using the transportal technique between January 2017 and December 2018 were retrospectively reviewed. Patients were classified into three groups according to knee flexion angle when creating femoral tunnel (group 1: < 120° (n = 19); group 2: 120-129° (n = 32); group 3: ≥ 130° (n = 33). GBA was measured on three-dimensional knee model reconstructed from postoperative computed tomography images. The length of the femoral tunnel and posterior wall blow-out were also checked. There was significant difference of GBA between the groups (group 1 = 112.1°; group 2 = 106.4°; group 3 = 101.4°, p < 0.001). The knee flexion angle in creating femoral tunnel was negatively correlated with GBA (r = - 0.733, p < 0.001). Five patients in group 1 had short femoral tunnel. GBA was influenced by knee flexion angle in creating femoral tunnel and got more acute as the knee flexion angle increased. Considering length and risk of wall blow-out in femoral tunnel, and GBA, knee flexion angle between 120 and 130° could be recommended as appropriate angle to create optimal femoral tunnel in ACL reconstruction using the transportal technique.

    Abstract A three-dimensional frequency-domain numerical wave tank (FR-NWT) based on the Rankine panel method was developed. An optimal artificial damping zone (ADZ) scheme was first applied to the FR-NWT to prevent reflection waves from the end walls. Parametric studies of ramp function shape with artificial damping coefficients and damping zone length were conducted to find a proper damping scheme for the frequency domain program. Applying both the Sommerfeld radiation condition and the ADZ scheme to the frequency domain program can reduce the length of the ADZ to less than one wavelength. The FR-NWT developed by the authors was used to calculate the hydrodynamic response of a hemispherical-heaving buoy wave energy converter (WEC) integrated with a seawall-type breakwater of infinite length. A linear power take-off system was used to calculate power generation of the WEC. The global motion of the WEC combined with the breakwater was up to 1.85 times greater than that of the WEC without the breakwater. Moreover, the capture width ratio of the WEC increased approximately 3.67 times more than that of the WEC without the breakwater.
