    Abstract Due to the flooding of the communal territory Juan de Grijalva in the Municipality of Ostuacan, state of Chi-apas, and the risk in the zone, this town and others were relocated to the Rural Sustainable City (CRS by its acronym in Spanish), which is part of a public policy of the federal and state governments during the presidency of Felipe Calderon and the governorship of Juan Sabines (2006-2012). The CRS was posed as a regional devel-opment strategy to address population dispersion and to eradicate poverty in an effective way. In this context, the objective of this work is to analyze what has happened with the residents relocated during the period of 2011-2016, evidencing their processes of territorialisation. This research was developed by using qualitative research methods, particularly the ethnographic method, supported by participant observations, fields notes and interviews. A comparative method was also employed, which allowed for the observation of the processes of territorialisation of the residents of the CRS over a period of 5 years. The results point out that the federal and state governments lack knowledge about their territory and population. They also highlight the absence of monitoring of public policies that are executed and the null participation of the population in them, a situation that perpetuates poverty instead of eradicating it.

    Abstract Background: Active case finding (ACF) in household contacts of tuberculosis (TB) patients is now recommended for National TB Programs (NTP) in low- and middle-income countries. However, evidence supporting these recommendations remains limited. This study evaluates the effectiveness and cost-effectiveness of ACF for household contacts of TB cases in a large TB endemic district of Lima, Peru.
    Methods: A pragmatic stepped-wedge cluster randomized controlled trial was conducted in 34 health centers of San Juan de Lurigancho district. Centers were stratified by TB rate and randomly allocated to initiate ACF in groups of eight or nine centers at four-month intervals. In the intervention arm, NTP providers visited households of index patients to screen contacts for active TB. The control arm was routine passive case finding (PCF) of symptomatic TB cases. The primary outcomes were the crude and adjusted active TB case rates among household contacts. Program costs were directly measured, and the cost-effectiveness of the ACF intervention was determined.
    Findings: 3222 index TB cases and 12,566 household contacts were included in the study. ACF identified more household contact TB cases than PCF, 199.29/10,000 contacts/year vs. 132.13 (incidence rate ratio of 1.51 (95% CI 1.21-1.88)). ACF was associated with an incremental cost-effectiveness ratio of US $16,400 per disability-adjusted life year averted and not cost-effective assuming a willingness-to-pay threshold for Peru of US $6360.
    Conclusion: ACF of TB case household contacts detected significantly more secondary TB cases than PCF alone, but was not cost-effective in this setting. In threshold analyses, ACF becomes cost-effective if associated with case detection rates 2.5 times higher than existing PCF programs.

    Abstract We determine the prevalence and trends of open neural tube defects (ONTDs) during 1991 to 2019 at the "Dr. Juan I. Menchaca" Civil Hospital of Guadalajara (Mexico). Also, details of potential risks were obtained in 662 newborns, including those 143 patients with anencephaly and open spina bifida (OSB) classified as isolated (cases) and 519 controls. Data were analyzed using multivariable logistic regression. Among 267 201 live births during the study period, 336 were born with ONTDs, yielding an overall prevalence of 12.6 per 10 000. After folic acid (FA)-related programs began in Mexico (2003-2019), only OSB showed a decline of 20.6%.     Abstract We test the hypothesis that climate and environmental conditions are becoming favorable for dengue transmission in San Juan, Puerto Rico. Sea Level Pressure (SLP), Mean Sea Level (MSL), Wind, Sea Surface Temperature (SST), Air Surface Temperature (AST), Rainfall, and confirmed dengue cases were analyzed. We evaluated the dengue incidence and environmental data with Principal Component Analysis, Pearson correlation coefficient, Mann-Kendall trend test and logistic regressions. Results indicated that dry days are increasing and wet days are decreasing. MSL is increasing, posing higher risk of dengue as the perimeter of the San Juan Bay estuary expands and shorelines move inland. Warming is evident with both SST and AST. Maximum and minimum air surface temperature extremes have increased. Between 1992 and 2011, dengue transmission increased by a factor of 3.4 (95% CI: 1.9-6.1) for each 1 °C increase in SST. For the period 2007-2011 alone, dengue incidence reached a factor of 5.2 (95% CI: 1.9-13.9) for each 1 °C increase in SST. Teenagers are consistently the age group that suffers the most infections in San Juan. Results help understand possible impacts of different climate change scenarios in planning for social adaptation and public health interventions.

    Abstract Se realizó un estudio observacional, descriptivo, de corte transversal, con una población de estudio compuesta por 634 pacientes pediátricos con cuadro de diarrea aguda secretoria, que fueron ingresados en el Hospital General Docente “Juan B. Viñas González” de Palma Soriano, Cuba, durante el período enero-diciembre, 2014. Los resultados arrojaron un predominio del rango de edades de 1 a 4 años, para un 30.4%; la mayoría de ellos, residentes en zonas rurales (64.5%), donde la calidad del agua de consumo no era la adecuada (46.8%) y la disposición de las excretas tenían deficiencias en un 45.1% de los casos. Los síntomas predominantes fueron fiebre y vómitos para un 47.4% y 45 % respectivamente y los signos fueron: los ojos hundidos en un 57.4% y sequedad de las mucosas (45.1%), como complicaciones se reportó: la deshidratación con 55.2%, la hipoglucemia (50.9%) y la acidosis metabólica (34.9%), presentando en ocasiones, un niño más de una complicación, lo que motivó un 35% de ingresos en los servicios de cuidados progresivos; sin embargo, no hubo fallecidos por esta causa en el tiempo estudiado.

    Abstract The general behavior of the tourism sector in Puerto Rico, with its marked seasonality, hints at a close relationship between tourism activities and climate conditions. Even if weather condition is only one of many variables considered by travelling tourists, climate conditions weigh heavily in the majority of the decisions. The effect of climate variability on the environment could be manifested in warmer temperature, heat waves, and changes in the frequency of extreme weather events, such as severe storms and hurricanes, floods, and sea level rise. These conditions affect different sectors of society, among them public health and the economy. Therefore, our research has two main objectives: to establish a tourism climate index (TCI) for Puerto Rico and to analyze if occupancy rates in hotels correspond to local weather conditions. Even though there are many other variables that could have positive or negative effects on tourism activities, results showed a significant association between occupancy rate in Puerto Rico and climate indexes. According to both TCI and the mean historical climate for tourism indexes, the most favorable months for tourism in Puerto Rico were February and March (winter), whereas the worst season was the end of August and the beginning of September (summer-fall). Although winter represents dry conditions and lower temperatures in San Juan, it also represents the highest occupancy rate during the years examined. In summer and fall, data showed high occupancy rates, yet climate conditions were not suitable; these months also correspond to the hurricane season. During this season, high relative occupancy rates responded to internal and local tourism patterns. It can therefore be assumed that until the climate-tourism relationship is well characterized, there is little hope of fully understanding the potential economic effects, detrimental or beneficial, of global climate change, not only on tourism in Puerto Rico, but on other economic sectors as well.
