    Abstract Background: Excruciating generalized abdominal pain with features suggestive of shock, at the end of the first or early second week after laparoscopic cholecystectomy (LC), is a frightening and formidable diagnostic predicament. This is because the early known complications like biliary leak or vascular injuries are unlikely diagnoses. Hemoperitoneum, is not usually considered, but instead more common occurrences like acute pancreatitis, choledocholithiasis, and sepsis are suspected. A delay in diagnosis and subsequent management of hemoperitoneum could have disastrous consequences.
    Case studies: Two patients presented with hemoperitoneum, in the second week after laparoscopic cholecystectomy. The first was because of a leak from a pseudoaneurysm of the right hepatic artery and the other was a bleed from a subcapsular liver hemangioma as a part of Osler Weber Rendu syndrome. Initially, a clinical assessment in both the patients was diagnostically inconclusive. Ultimately the diagnosis could be made, based on computed tomography angiography and visceral angiography. In the second patient, a positive family history and genetic testing were helpful. The first patient was successfully managed by intravascular embolization, while the second patient was successfully managed conservatively with intraperitoneal drains and conservative management of comorbidities.
    Conclusions: The presentation is to generate awareness that hemorrhage could be a presentation, in the early second week, after LC. A common cause to be considered is a pseudo aneurysmal bleed. Secondary hemorrhage and other rare coincidental unassociated conditions could also be responsible for the hemorrhage. A high index of suspicion, and early and timely management are keys to a successful outcome.

    Abstract The deep learning method (DLM) is one way to fix issues in optical nonorthogonal multiple access (O-NOMA) systems that are caused by signals that overlap and interfere with each other. NOMA increases the optical framework's spectrum efficiency, allowing several users to share the same time-frequency resources. However, NOMA-DLM-based detection's complicated interference patterns and variable channel conditions are challenging for conventional detection methods to manage. By utilizing deep neural networks' advantages, these methods are able to overcome these challenges and improve detection performance. An overview of the main features and advantages of DLM detection in massive multiple input and output (M-MIMO) O-NOMA systems is given in this article. It describes the essential elements, such as the training procedure and the network design. In order to process the sent symbols or decode data streams, DLM networks are built to process the incoming signal, power allocation coefficients, and extra information. Gradient descent optimization is used to update the network parameters iteratively while training the network, and a diverse and representative dataset is created. Additionally, the challenges of detecting deep learning in O-NOMA systems are examined. It recognizes that in order to get the best results, significant computational resources, a large amount of training data, and careful model design are required. It looks at and compares the 16 × 16, 32 × 32, and 64 × 64 M-MIMO-NOMA models in terms of bit error rate (BER), complexity, and power spectral density (PSD). The suggested DLM algorithms have been demonstrated to perform better than traditional methods by achieving an excellent BER of 10-3 at 4.1 dB and PSD (-2500) performance with low complexity.

    Abstract Imaging a case of autoimmune encephalitis (AIE) can be challenging as the underlying tumor may be occult. The aim of this retrospective case-based study is to evaluate role of whole-body MRI/Positron emission tomography (PET) in workup of AIE. Standardizing the whole-body MRI/PET protocol, Cross modality yield with serology and magnetic resonance/PET (MR/PET) and finally highlight the advantage of hybrid MR/PET. We present the retrospective review data from January 2016 to December 2019 referred for whole body MR/PET with suspected AIE/Paraneoplastic syndrome, per consensus criteria, treated at a single tertiary center. Analysis is done group wise based on referral being for oncological, immunological or neuropsychiatric condition. Detailed results with sensitivity and specificity are presented in tabular format with case-based review in our series for protocols and advantages of MR/PET. Among total of 600 MR/PET cases, 227 were suspected of AIE/paraneoplastic syndrome and were referred for whole body imaging. Distribution of Group 1 Known oncology group (n = 10), Group 2 Non oncological systemic illness group (n = 174) and group 3 the primary neuropsychiatric illness (n = 43) with Group 2 being largest. The gender distribution was similar and mean age was 42 years. Seronegative cases (n = 130) were greater than seropositive cases (n = 97). Seropositivity was in the following order Autoimmune > Paraneoplastic > Myositis panel. Whole body MRPET yielded occult malignancy in 9% and imaging abnormality in 88% of cases. Whole body MR/PET has an important role in workup of AIE. Selection of the appropriate protocol is important, especially when history and physical examination are nonspecific.
