    Abstract Background: Embryonal carcinoma is a rare tissue type in germ cell tumors. According to our literature review, metastatic embryonal carcinoma misdiagnosed as lymphoma because of its high similarity to lymphoma is extremely rare and has not been reported yet.
    Case presentation: A 46-year-old middle adulthood male presented with unexplained fever, night sweats, abdominal distension for 3 months, and weight loss of around 7kg during almost 6 months, which is extremely similar to lymphoma from the clinical features and imaging examinations. After a clear diagnosis, the case not only obtained the opportunity of surgery but was also exempted from radiotherapy. The treatment effect was good. We report a case of rare metastatic embryonal carcinoma, which can provide insight into the diagnosis and treatment of embryonal carcinoma.
    Conclusion: Metastatic embryonal carcinoma of abdominal lymph nodes can be highly similar to lymphoma; the diagnosis can only be based on clinical manifestations and imaging examination but also combined with patient history, tumor markers and biochemical examination. However, the final diagnosis depends on pathological biopsy.

    Abstract Introduction: China initialised the expanded hepatitis A vaccination programme (EHAP) in 2008. However, the effectiveness of the programme remains unclear. We aimed to comprehensively evaluate the effectiveness of EHAP in the country.
    Methods: Based on the provincial data on the incidence of hepatitis A (HepA), the population and meteorological variables in China, we developed interrupted time series (ITS) models to estimate the effectiveness of EHAP with the autocorrelation, seasonality and the meteorological confounders being controlled. Results were also stratified by economic zones, age groups and provinces.
    Results: We found a 0.9% reduction (RR=0.991, 95% CI: 0.990 to 0.991) in monthly HepA incidence after EHAP, which was 0.3% greater than the reduction rate before EHAP in China. Across the three economic regions, we found a 1.1% reduction in HepA incidence in both central and western regions after EHAP, which were 0.3% and 1.2% greater than the reduction rates before EHAP, respectively. We found a decreased reduction rate for the eastern region. In addition, we found generally increased reduction rate after EHAP for age groups of 0-4, 5-14 and 15-24 years. However, we found decreased reduction rate among the 25-64 and ≥65 years groups. We found a slight increased rate after EHAP in Shanxi Province but not elsewhere.
    Conclusion: Our finding provides comprehensive evidence on the effectiveness of EHAP in China, particularly in the central and western regions, and among the population aged 0-24 years old. This study has important implications for the adjustment of vaccination strategies for other regions and populations.

    Abstract Skin flap transplantation is one of the most common tissue transplantation methods for wound repair and organ reconstruction in plastic surgery. During the transplantation process, the inflammatory response of transplanted flap and angiogenesis are critical to the successful rate of skin flap transplantation. In recent years, to improve the biocompatibility and cell affinity of biomedical materials, the modified biomaterials have gradually become a popular subject in scientific researches. In our study, the IL-4 modified expanded polytetrafluoroethylene (e-PTFE) surgical patch IL4-e-PTFE was prepared, and the rat skin flap transplantation model was constructed. The results of cell experiment prove that IL-4 has potentiation in the angiogenesis of human umbilical vein endothelial cell (HUVEC) induced by monocyte, and IL-4 can also promote angiogenesis by inducing the M2 macrophages. According to the results of in vivo experiment, the apoptosis level of transplanted flap cells of rats in the IL4-e-PTFE group was lower than that in the e-PTFE group, and in the IL4-e-PTFE group, the expression levels of pro-inflammatory cytokines IL-1β, IL-6 and TNF-α showed significantly decline compared to the e-PTFE group, while the expression levels of anti-inflammatory cytokines IL-1Ra, IL-10 and TGF-β presented significant increase compared to the e-PTFE group; the immunofluorescence staining results show that the number of M2 macrophages in transplanted flap area of rats in the IL4-e-PTFE group was significantly higher than that in the e-PTFE group, and the angiogenesis level was remarkably improved. In this study, by preparing IL4-e-PTFE and carrying out the cell and in vivo experiments, a reference method is proposed, which can reduce the inflammatory response during skin transplantation process using e-PTFE and optimize the long-term effects of flap blood vessels, hoping to provide a broader space for the applications of e-PTFE in medicine.

    Abstract Background and objective: Liver cancer is the second highest cause of cancer-related deaths worldwide. It is commonly treated with liver transplantation, where tacrolimus is typically used as an antirejection immunosuppressant. The purpose of this study was to evaluate the effect of tacrolimus time in therapeutic range (TTR) on liver cancer recurrence in liver transplant recipients and to compare the performance of TTRs calculated according to the target ranges recommended in published guidelines.
    Methods: A total of 84 patients who underwent liver transplantation for liver cancer were retrospectively included. Tacrolimus TTR was calculated using linear interpolation from the date of transplantation until recurrence or the last follow-up according to target ranges recommended in the Chinese guideline and international expert consensus.
    Result: Twenty-four recipients developed liver cancer recurrence after liver transplantation. The CTTR (TTR calculated according to the Chinese guideline) for the recurrence group was significantly lower than that of the nonrecurrence group (26.39% vs. 50.27%, P < 0.001), whereas the ITTR (TTR calculated according to the international consensus) was not significantly different between the two groups (47.81% vs. 56.37%, P = 0.165). Multivariate survival analysis revealed that age, microvascular invasion, hepatocellular carcinoma, CTTR, and mean tacrolimus trough concentration were independent predictors of liver cancer recurrence after liver transplantation.
    Conclusions: TTR predicts liver cancer recurrence in liver transplant recipients. The range of tacrolimus concentrations recommended in the Chinese guideline was more beneficial than that recommended in the international consensus for Chinese patients undergoing liver transplantation for liver cancer.

    Abstract Drought events have considerable direct and indirect economic, environmental, and social impacts, but few studies have analyzed and assessed future changes in drought disasters from a risk perspective to guide responses and adaptations thoroughly. Studying the potential climate-related impacts on future crop yield is therefore urgently needed. Intercomparison of the three Shared Socio-economic Pathway (SSP) scenarios based drought risks and yield loss of China was carried out using the climate models from the Coupled Model Intercomparison Project Phase 6 (CMIP6), and the hotspots of high drought risk regions were identified. This study found that the areas affected by severe maize drought (loss ratio larger than 0.2) accounted for 16.13 %, 20.79 %, and 18.87 % of the total national corn areas under three low, medium-to-high and high emission scenarios (SSP1-2.6, SSP3-7.0, SSP5-8.5) respectively. The northwest China maize region, the ecotone between agriculture and animal husbandry, and the western central northern China maize region have relatively high loss risk. Compared with SSP1-2.6, the yield loss rates increased with 70.73 % and 61.52 % of national corn areas for SSP3-7.0 and SSP5-8.5, respectively. There is a decrease in the areas with low-risk and a significant increase in the areas with high-risk for SSP3-7.0 and SSP5-8.5 compared to the SSP1-2.6. These results may provide theoretical support for agricultural drought risk reduction and adaptation planning to ensure food security under climate change.

    Abstract Previous studies on solid organ transplantation have reported that a low time in therapeutic range (TTR) of tacrolimus increases the risk of poor outcomes. However, the reproducibility of the findings in liver transplantation has not yet been confirmed. The TTR, coefficient of variation (CV) and standard deviation (SD) were calculated for 207 adult liver transplant patients from the date of transplantation until the first episode of acute rejection (AR), graft loss, acute kidney injury (AKI), biliary complications, infection or the last follow-up. Kaplan-Meier curves, log-rank tests and Cox regression analyses were performed. Sixty-one (29.5%) patients reached the composite endpoint of AR, biliary complications and graft loss. The log-rank test indicated that the low TTR group had an increased risk of the composite endpoint (P < 0.001), AKI (P < 0.001) and infection (P < 0.001). Multivariate Cox regression analyses revealed that a 10% decrease in TTR was associated with an increased hazard for composite endpoint (hazard ratio [HR]: 1.185, P = 0.010), AKI (HR: 1.355, P < 0.001) and infection (HR: 1.357, P < 0.001). Unexpectedly, SD and CV demonstrated no association with the above-mentioned inferior outcomes. Compared with SD and CV, the TTR of tacrolimus was more correlated with inferior outcomes and may be a novel indicator for predicting the prognosis of liver transplantation.

    Abstract Objective: To explore the anti-hypoxic effects of oat oligopeptides (OOPs) in mice.
    Methods: We randomly divided mice into six groups, including a vehicle control group, a whey protein group (0.50 g/kg), and four OOPs-treated groups (0.25, 0.50, 1.00, and 2.00 g/kg). The test substances were administered by gavage once a day for 30 days. The normobaric hypoxia, sodium nitrite toxicosis, and acute cerebral ischemia survival times were recorded. Also, the MDA content, the lactate levels, the LDH activity, and the mRNA levels of HIF-1α and VEGF in the brains were measured. We performed a whole blood cell analysis using a blood analyzer.
    Results: The OOPs significantly extended the survival times of normobaric hypoxia, sodium nitrite toxicosis, and acute cerebral ischemia. Notably, the OOPs enhanced the RBC, Hb, and Hct levels, decreased the malonaldehyde (MDA) and lactate content in the brain, enhanced the brain lactate dehydrogenase (LDH) activity, and increased the hypoxia-inducible factor 1alpha (HIF1α) mRNA and the vascular endothelial growth factor (VEGF) mRNA expression levels.
    Conclusion: OOPs have anti-hypoxic effects, and the mechanism may involve improving the blood's oxygen carrying capacity and oxygen utilization rate minimizing the lipid peroxidation lesions, increasing the brain's ability to buffer against lactic acidosis in mice, and promoting angiogenesis and regulating the hypoxic response.
