    Abstract Purpose: To report predisposing factors, clinical presentation, antibiotic sensitivity, and management of Citrobacter-caused infectious keratitis.
    Methods: We retrospectively reviewed the medical records of culture-proven cases of Citrobacter keratitis in a tertiary referral center for 8 years (from January 2012 to September 2020). Demographic data of the patients, predisposing factors, and presenting signs were extracted.
    Results: Eighteen cases of microbial keratitis due to Citrobacter spp. were identified. The median age of the patients was 66 years (range: 10-89, interquartile range : 59-81). Thirteen patients were male and 5 were female. Multiple predisposing factors were identified in all eyes, including ocular surface disease (n = 8), previous corneal surgery (n = 6), and history of ocular trauma (n = 6). Five patients were diabetic. Corrected distance visual acuity (CDVA) of patients was light perception (LP) in 8 patients, hand motion (HM) in 7, counting fingers (CF) at 1 m in 1, and CF at 2 m in 2 patients. Thirteen eyes exhibited hypopyon. An area of corneal thinning was observed in 7 eyes (38.9%). Endophthalmitis due to infectious keratitis developed in one patient. In vitro susceptibility testing confirmed high sensitivity to ceftazidime and aminoglycosides. Medical management consisted primarily of topical amikacin (20 mg/ml) combined with topical cefazoline (50 mg/ml) (72.2%). Surgical tectonic procedures were carried out in 7 eyes (38.9%).
    Conclusion: Citrobacter spp. is a rare cause of bacterial keratitis.Previous keratoplasty and ocular surface problems are important risk factors. The prognosis is not good and surgical tectonic intervention is required in many cases to resolve the corneal infection.

    Abstract Background: Cystic fibrosis (CF) is a hereditary disease with a high mortality rate. The present study aimed to determine the effect of a physical activity program (PAP) on the life quality of children with CF at school age.
    Materials and methods: This study conducted on 70 children with CF, the samples were randomly divided into intervention and control groups. A demographic information questionnaire and the Pediatric Quality of Life Inventory were used for data collection. This study was conducted in three stages and 2 months after the last intervention. Finally, the data were analyzed in SPSS software version 22 using the Chi-square test, independent t-test, and paired t-test at a p-value of less than 0.05.
    Results: The results indicated that the two groups had no significant difference in terms of demographic variables before the intervention. Moreover, it was observed that the mean scores of life quality in the physical, emotional, social, and academic performance of children in both groups before the intervention in child and parent evaluations did not show significant differences. After the intervention in the experimental group, the mean scores of life quality in all aspects significantly increased, compared to those of the control group (P<0.001). Moreover, there were no significant differences between the mean scores of life quality in the control group before and after the intervention.
    Conclusion: Given the effectiveness of the intervention to enhance life quality, PAP is feasible and possible in the field of CF. It has to be noted that this method is an effective way to improve life quality.

    Abstract Due to the cultural and geographical characteristics of the actors, the Caspian Basin is prone to affect many actors. The special importance of this region in the international system has attracted the attention of many trans-regional powers, each of which somehow wants to participate. Caspian littoral actors have also redefined identity and national interests within their territorial boundaries in the years since the collapse of the Soviet Union. Each of the coastal actors in the Caspian geopolitical structure can improve their position and influence others. In addition, the genomes of each are negative in two ways: basic nature - basic human and positive - some are active and some are passive. Therefore, the main question of the research is what effect does the structure and geopolitical genomes of the Caspian littoral states have on the national security of the Islamic Republic of Iran? The present study has studied the subject with a practical purpose and in taking a descriptive-analytical approach and collecting information in a library. The results show that each of the Caspian coastal actors has their own socio-political characteristics, through which they shape the mindset of the agents and make decisions in this geopolitical structure. In addition, the geopolitical genomes of each of them are different from each other, which can differentiate the type of geopolitical approach of each of them based on the type of power distribution and the pattern of friendship and enmity towards the Islamic Republic of Iran. Although Iran's military and security challenges in the Caspian region have diminished in recent decades, the cultural, social, geographical, and economic potential of coastal actors suggests that they have become a real competitor to Iran.     Abstract Introduction: Healthcare workers, including dental students are at risk of transmission of blood-borne infections like Hepatitis and AIDS because of their continuous contact with blood and other body fluids. Thus it is necessary for them to achieve enough knowledge, proper attitude and right behavior regarding to these diseases through efficient training so they can treat infected patients, which is their ethical and legal duty. The present study was designed to evaluate knowledge, attitude and practice of dental students in School of Dentistry, Shahid Sadoughi University of Medical Sciences about HIV and Hepatitis in 2019-2020. Methods: This observational-descriptive cross-sectional study was conducted on 188 4th, 5th and 6th year dental students of Yazd Faculty of Dentistry, 170 of whom completed the questionnaire. Data were collected using a researcher-made questionnaire including two parts of demographic information and knowledge, attitude and performance questions; data were analyzed using T-Test, ANOVA and chi-score tests and SPSS version 16. Results: The mean score of knowledge, attitude and practice regarding hepatitis and AIDS was 20/77 (out of 30), 32/04 (out of 45) and 46/17 (out of 52), respectively. The students had low knowledge level about prophylaxis treatments after needlestick injuries and screening tests. They also had poor attitude towards treating the infected patients. There was a significant difference in practice between males and females. (P-value<0.05). Conclusion: Dental students performed well in relation to hepatitis and AIDS diseases despite their average knowledge and attitude. Thus there should be an emphasis on knowledge improvement and attitude alternation in Yazd Faculty of Dentistry.     Abstract Purpose: To evaluate the rate of infectious keratitis after photorefractive keratectomy (PRK) before and during the coronavirus disease 2019 (COVID-19) pandemic.
    Methods: A retrospective chart review was performed on patients who were diagnosed as having infectious keratitis after PRK between January 2015 and January 2021.
    Results: The study period was divided into the pre-COVID-19 era (between January 2015 and February 2020) and the COVID-19 era (between February 2020 and January 2021). A total of 47 patients were diagnosed as having infectious keratitis after PRK: 22 were diagnosed in the pre-COVID-19 era and 25 were diagnosed in the COVID-19 era. The rate ratio for infectious keratitis after PRK was 5.68 during the COVID-19 pandemic (CI: 3.20 to 10.07,
    Conclusions: The rate of infectious keratitis after PRK increased during the COVID-19 pandemic. This may be due to the increased use of face masks in this era.

    Abstract We report the case of a 37-year-old man who was admitted to Baqiyatallah hospital in Tehran (Iran) for retrosternal pain, fever, fatigue, dyspnoea and severe non-productive cough. The patient was subsequently confirmed as positive for COVID-19 at real-time polymerase chain reaction (RT-PCR) test. Chest computed tomography (CT) revealed also the presence of pneumomediastinum. This case highlights the importance of chest CT imaging for COVID-19 pneumonia to detect co-existing conditions as pneumomediastinum.

    Abstract BACKGROUND COVID-19 infection has led to a worldwide pandemic, and new cases are on the rise. Intravenous drug users (IVDU) are presumably at a higher risk of being infected since they have poor personal hygiene, live in groups, and have risky behaviors. The current study aimed to evaluate the seroprevalence of COVID-19 in IVDU in comparison with non-drug users (N-DU). METHODS This cross-sectional study was conducted on 167 IVDU and 134 N-DU. A questionnaire gathering data on demographics, comorbidities, and use of personal protective equipment was administered to all participants. In addition, 5 cc of blood was taken from each individual to test for SARS-CoV-2 specific antibodies (Pishtaz Teb SARS-Cov-2 ELISA kits). RESULTS The mean age of N-DU and IVDU were 38.9 ± 12.9 and 40.38 ± 10.24 years, respectively. COVID-19 seroprevalence in IVDU was 9.7%, and 4.8% in N-DU, but this finding was not statistically significant (

    Abstract Background: A critical necessity before surgical resection in mesial temporal lobe epilepsy (mTLE) is lateralizing the seizure focus in the temporal lobe. This study aimed to investigate the differences in perfusion pattern changes in right and left mTLE.
    Methods: 42 mTLE patients (22 left and 20 right mTLE) and 14 controls were surveyed with pulsed arterial spin labeling at 3.0 T. The mean cerebral blood flow (CBF) and asymmetry index (AI) were calculated in the bilateral temporal lobe, amygdala, hippocampus, parahippocampus, and nine bilateral vascular territories ROIs. The alterations in whole-brain CBF were identified using statistical parametric mapping (SPM).
    Results: CBF decreased in ipsilateral sides in both epilepsy subcohorts, with right mTLE showing a significant difference in most ROIs while left mTLE exhibiting no significant change. CBF comparison of left mTLE and controls showed a significant drop in ROI analysis in left middle temporal and left intermediate posterior cerebral artery and in AI analysis in parahippocampus, distal anterior cerebral artery, distal middle cerebral artery, and intermediate anterior cerebral artery. CBF hypoperfusion was seen in ROI analysis in the left intermediate anterior cerebral artery, left middle temporal, right middle temporal, left superior temporal in the right mTLE compared to controls. Left mTLE CBF differed significantly from right mTLE CBF in right distal middle cerebral artery ROI and AI of proximal middle cerebral artery.
    Conclusion: Our result revealed that mTLE affects extratemporal regions and both mTLE subcohorts with different perfusion patterns, which may enhance the performance of preoperative MRI assessment in lateralization procedures.

    Abstract Background: Previous studies have reported that melatonin intake is inversely associated with reduced markers of atherosclerosis development such as carotid intima-media thickness (CIMT).
    Objective: This study was designed to assess the effects of melatonin administration on CIMT and pulse wave velocity (PWV) in patients with diabetic nephropathy (DN).
    Materials and methods: This randomized, double-blind, placebo-controlled trial was conducted on 32 DN patients. Subjects were assigned to receive melatonin or placebo (starch) for 24 weeks. Individuals in the melatonin group (n = 19) received 10 mg/day. CIMT and PWV levels were taken at the study baseline and after 24 weeks of intervention.
    Results: After 24 weeks of intervention, melatonin intake did not affect mean levels of left (P = 0.51) and right (P = 0.16) CIMT and maximum levels of left (P = 0.76) and right (P = 0.15) CIMT, and PWV (P = 0.55) compared with the placebo. In addition, within-group difference demonstrated a significant reduction in mean levels of right CIMT (P = 0.01) in the melatonin group. We did not observe any significant change in C-reactive protein (CRP) concentrations after melatonin intake (P = 0.81).
    Conclusions: Melatonin intake did not affect mean levels of left and right CIMT and maximum levels of left and right CIMT, PWV, CRP levels compared with the placebo. In addition, within-group difference demonstrated a significant reduction in mean levels of right CIMT in the melatonin group.     Abstract A perfect anesthesia technique is one in which anesthesia can be done in the shortest possible time with minimal hemodynamic changes and side effects This study is aimed to investigate the effects of concomitant administration of dexamethasone with bupivacaine on the duration of anesthesia and the quality of anesthesia-induced in patients undergoing cesarean section. This is a quasi-experimental study done on patients undergoing cesarean section by spinal anesthesia. All patients underwent standard monitoring after admission, then Ringer serum (5 cc/kg) was administered to the patients. The patients was then seated and subjected to intrathecal anesthesia with a midline approach in L3-L4 space. Patients were matched for baseline conditions. After recording background information, type and dosage of drugs used, blood pressure, heart rate, number and frequency of nausea and vomiting were listed in the checklist, and the data were analyzed using SPSS software. In this study, 70 pregnant women who were candidates for cesarean section were included. All our studied parameters, including the mean time to the start of analgesia, mean time of analgesia duration, analgesia duration, mean time to first request for analgesics were significantly improved among the intervention group. The use of dexamethasone and bupivacaine to induce spinal anesthesia during cesarean section can significantly shorten the onset time of analgesia and reduce postoperative nausea and vomiting. Dexamethasone can be used as an effective drug in cesarean section.
