    Abstract Background: More children presenting to Emergency Departments (EDs) with acute infections are now directly referred for outpatient parenteral antibiotic therapy (OPAT). Sparse data exist on what clinical features in these children are associated with OPAT failure. We hypothesised that children who were younger or presented with systemic features of infection would be more likely to need admission.
    Methods: We conducted a service evaluation over a 5-year period (12 September 2015-12 September 2020) at a single UK tertiary centre paediatric ED formally known as the Royal Hospital for Sick Children Edinburgh. All children referred from the ED for OPAT with ceftriaxone were included. OPAT failure was defined as a decision by a senior clinician of need for admission. Univariate statistical testing and multivariate logistic regression modelling were performed.
    Results: 754 children received OPAT in the ED over a 5-year period. 95 children (13%) required admission for inpatient management. Need for admission was independently associated with having a positive blood culture (adjusted OR (aOR) 8.9; 95% CI 1.49 to 47; p=0.01) or an ultrasound performed (aOR 6.8; 95% CI 3.74 to 12.7; p<0.001). We observed no significant association between age and systemic features (fever, white cell count or C reactive protein) with need for admission in our multivariate analysis.
    Conclusion: In children presenting with acute infections to our paediatric ED who were deemed suitable by senior clinicians to be managed using OPAT with ceftriaxone, younger age (above 3 months) and the presence of systemic features were not independently associated with need for admission. Overall, our service was safe and no child came to harm from early ambulation during this evaluation.

    Abstract Objective: Conflicting data exist on the diagnostic sensitivity of core-needle biopsy (CNB) compared to fine-needle aspiration (FNA) in the evaluation of axillary lymph node metastasis from breast cancer. Our purpose was to evaluate the sensitivity of CNB and FNA using subsequent axillary surgery as the gold standard and to compare the patients' subjective pain levels for each biopsy method.
    Methods: This IRB-approved prospective study enrolled 140 patients from February 2014 to May 2019 with known or suspected breast cancer. Patients underwent both US-guided FNA and 14-gauge CNB of the same node with clip placement and rated their pain level using a verbal numerical rating scale of 0 to 10. The diagnostic sensitivities were determined by pathology of the surgically excised lymph node using the McNemar test of correlated proportions. Changes in pain scores for CNB and FNA were determined using the Wilcoxon rank sum test.
    Results: A total of 94 patients had confirmatory excision of the biopsied node with nodal metastasis detected in 71.3% (67/94). The sensitivity of CNB for detection of nodal metastasis was 95.5% (64/67), while the sensitivity of FNA was 67.2% (45/67) (P < 0.05). Overall pain score ratings for CNB increased by 0.6 from baseline on an 11-point numerical rating scale, while overall pain score rating for FNA decreased by 0.2 from baseline (P < 0.05).
    Conclusion: Our study demonstrates that 14-gauge CNB has superior sensitivity for detection of axillary nodal metastases and mildly increased pain compared with 25-gauge FNA in patients with breast cancer.

    Abstract Many species across taxa select habitat based on conspecific presence, known as conspecific attraction. Studies that document conspecific attraction typically provide social information (i.e., cues that indicate the presence of a given species) and then determine if a given species is more likely to settle at locations where the social information is provided compared to those locations that do not. Although the number of studies examining conspecific attraction has grown in recent years, a comprehensive review has not yet been undertaken. Here, we conducted a review of the literature and found 151 studies investigating conspecific attraction across eight taxa. We found that conspecific attraction is widespread with between 80% and 100% of studies, depending on taxa, documenting positive associations between habitat selection and the presence of conspecific cues. Conspecific attraction has been documented more frequently in bird and fish species with less attention given to invertebrate and mammal species. We use the patterns we found to (a) provide an overview of the current state of research on conspecific attraction and (b) discuss how important factors, such as cue characteristics and life history traits, may play a role in shaping conspecific attraction patterns within and across taxa.

    Abstract As women's rugby league grows, the need for understanding the movement patterns of the sport is essential for coaches and sports scientists. The aims of the present study were to quantify the position-specific demographics, technical match statistics, and movement patterns of the National Rugby League Women's (NRLW) Premiership and to identify whether there was a change in the intensity of play as a function of game time played. A retrospective observational study was conducted utilizing global positioning system, demographic, and match statistics collected from 117 players from all NRLW clubs across the full 2018 and 2019 seasons and were compared between the ten positions using generalized linear mixed models. Strong, Fast, Fit, Lean, and Safe: A positional comparison of physical and physiological qualities within the 2020 Australian Women's Rugby League team. J Strength Cond Res 35(12S): S11-S19, 2021-The purpose of the present study was to report the physical and physiological characteristics of elite women Rugby League (RL) players. Thirty-nine women (25.6 ± 4.3 years, 171.3 ± 7.7 cm, 83.5 ± 13.9 kg) from the 2020 Australian women's RL squad were recruited for this study. Players were categorized as adjustables (n = 7), backs (n = 15), or forwards (n = 17) for analysis. Each player was assessed for anthropometry, body composition (dual-energy X-ray absorptiometry), speed (5, 10 and 20 m sprint times), lower-body power (countermovement jump), upper-body power (medicine ball throw and explosive push up force), estimated one repetition maximum (e1RM) bench press, squat and bench pull, isometric mid-thigh pull strength, eccentric knee flexor strength, isometric hip abduction and adduction, and intermittent endurance performance (30-15 intermittent fitness test; 30-15 IFT). Linear mixed models were performed to compare positional groups. Forwards were significantly heavier and had greater fat mass, fat-free mass, and body fat percentage compared with backs and adjustables (P < 0.01). Backs were faster over 20 m compared with forwards (P = 0.025), whereas forwards had a lower 30-15 IFT peak velocity and estimated V̇o2peak compared with backs and adjustables. Nonetheless, when including body mass in the model, there were no differences between groups in 30-15 IFT peak velocity. There were no significant differences in other variables. These results provide contemporary benchmark physical, physiological, and anthropometric data for elite women RL players, which can inform recruitment, selection, training, and testing.

    Abstract Introduction: Lives lost in North America due to the unregulated drug poisoning emergency are preventable and those who survive an opioid overdose may suffer long-term disability. Rates of opioid overdose more than doubled following the onset of the COVID-19 pandemic in British Columbia, Canada.
    Materials and methods: Our analytical sample was comprised of 1447 participants from the 2018, 2019, and 2021 Harm Reduction Client Survey who responded yes or no to having experienced an opioid overdose in the past 6 months. Participants were recruited from harm reduction sites from across British Columbia. We used logistic regression to explore associations of experiencing an opioid overdose.
    Results: Overall, 21.8% of participants reported experiencing an opioid overdose in the last six months (18.2% in 2019 and 26.6% in 2021). The following factors were positively associated with increased adjusted odds of experiencing a non-fatal opioid overdose: cis men relative to cis women (AOR 1.49, 95% CI 1.10-2.02), unstably housed compared to people with stable housing (AOR 1.87, 95% CI 1.40-2.50), and participants from 2021 compared to those from 2019 (AOR 3.06, 95% CI 1.57-5.97). The effects of both previous experience of a stimulant overdose and having witnessed an opioid overdose depended on the year of study, with both effects decreasing over subsequent years.
    Conclusions: Overdoses have increased over time; in 2021 more than one in four participants experienced an overdose. There is an urgent need for policy and program development to meaningfully address the unregulated drug poisoning emergency through acceptable life-saving interventions and services to prevent overdoses and support overdose survivors.

    Abstract Purpose: To determine the micro-longitudinal effects of duration and timing of screen-based activities on sleep within and between adolescents.
    Methods: Daily survey and actigraphy data from the age 15 wave of the Future of Families and Child Wellbeing Study were analyzed using multilevel modeling. Four hundred seventy five adolescents provided three or more days of valid daily survey and nighttime sleep data.
    Results: Within-person results showed that on days when adolescents played video games more than their daytime average ± SE (1.3 ± 1.2 hours), sleep onset (6 ± 2 minutes, p < .01) and midpoint (4 ± 2 minutes, p < .02) were delayed for each additional hour of gaming. Between-person results showed that for each hour adolescents used screens to communicate with friends across the day, sleep onset was later (11 ± 3 minutes, p < .01), sleep midpoint was later (8 ± 3 minutes, p < .01), and sleep duration was shorter (-5 ± 2 minutes, p < .03). Adolescents who used screens to communicate with friends or play video games in the hour before bed had later sleep onset (30 ± 14 minutes, p < .03) and midpoint (25 ± 13 minutes, p < .05).
    Discussion: Among adolescents, passive screen usage such as browsing the Internet or watching videos may not affect sleep timing or duration, but limiting interactive screen-based activities could protect adolescent sleep health and well-being.

    Abstract Background: In light of the ongoing opioid overdose crisis, there is an urgent need for research on the impacts of mental health among people presenting with concurrent pain and substance use. This study examined the effect of depressive symptoms on pain severity and functional interference among people who use drugs (PWUD) during a community-wide overdose crisis.
    Methods: From December 1st 2016 to December 31st 2018, 288 participants in two cohort studies of PWUD in Vancouver, Canada completed interviewer-administered questionnaires that included the Brief Pain Inventory and PROMIS Emotional Distress-Depression instruments. Generalized linear regression modelling (GLM) was used to examine the cross-sectional effect of depressive symptoms and other confounding factors on pain severity and interference.
    Results: Moderate to severe depressive symptoms were significantly associated with greater pain-related functional interference (adjusted β = 1.24, 95% confidence interval [CI] = 0.33-2.15), but not significantly associated with greater average pain severity (adjusted β = 0.22, 95% CI = - 0.3 - 0.82), when controlling for confounding variables. Reported daily heroin use (adjusted β = 1.26, 95% CI = 0.47-2.05) and non-fatal overdose (adjusted β = 1.02, 95% CI = 0.08-1.96) were also significantly associated with greater pain-related functional interference.
    Conclusions: In a substance-using population, greater pain-related functional interference was positively associated with depressive symptoms as well as overdose and daily heroin use. These findings emphasize the need to address the functional impact of pain, mental health comorbidity, and high-risk substance use that may contribute to overdose and other harms.
