Abstract Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in women during the fertile period. Women with PCOS have an increased risk of developing major cardiovascular risk factors during the fertile period: obesity, impaired glucose tolerance, diabetes mellitus, dyslipidemia, and metabolic syndrome. The possible effect of PCOS on cardiovascular disease (CVD) has been reported in different studies, but the results are not clear for several reasons. Indeed, most of the studies analyzed a cohort of fertile women who, given their relatively young age, have a low frequency of cardiovascular diseases. In addition, longitudinal studies have a short follow-up period, insufficient to draw firm conclusions on this topic. Finally, pharmacological treatment is limited by the lack of specific drugs available to specifically treat PCOS. In this review, we report on studies that analyzed the possible effect of PCOS on the most common CVD (hypertension, arterial stiffness, atherosclerosis, and cardiovascular event) and available drugs used to reduce CVD in PCOS women.

Abstract Polyp detection through colonoscopy is a widely used method to prevent colorectal cancer. The automation of this process aided by artificial intelligence allows faster and improved detection of polyps that can be missed during a standard colonoscopy. In this work, we propose to implement various object detection algorithms for polyp detection. To improve the mean average precision (mAP) of the detection, we combine the baseline models through a stacking approach. The experiments demonstrate the potential of this new methodology, which can reduce the workload for oncologists and increase the precision of the localization of polyps. Our proposal achieves a mAP of 0.86, translated into an improvement of 34.9% compared to the best baseline model and 28.8% with respect to the weighted boxes fusion ensemble technique.

Abstract Prone positioning with continuous positive airway pressure (CPAP) is widely used for respiratory support in awake patients with COVID-19-associated acute respiratory failure. We aimed to assess the respiratory mechanics and distribution of ventilation in COVID-19-associated ARDS treated by CPAP in awake prone position. We studied 16 awake COVID-19 patients with moderate-to-severe ARDS. The study protocol consisted of a randomized sequence of supine and prone position with imposed positive end-expiratory pressure (PEEP) of 5 and 10 cmH

Abstract Senegalese agriculture is threatened by climate change effects, affecting rainfall variability both at interannual and interdecadal timescales. Using FAO’s AquaCrop crop-growth model, we tested the efficiency of an in situ water harvesting technique - tied ridges - for maize cropping in the Fatick region in Senegal, in response to changes in temperature and precipitation with different fertility levels and different soils. Results showed that, considering the current climate and soil fertility, tied ridges did not significantly impact maize yields. The rainfall amount was enough for maize production and to avoid water stress during the cropping season. Under perturbed climates and, especially, in years with low average rainfall amounts, high losses in yield were registered under optimal fertility conditions (up to 80%). The strongest effect was obtained when tied ridges were simulated on clay soil, enhancing yields by 5.6% and 13% at actual and optimal fertility conditions, respectively. Our results highlighted how the current maize production in the Fatick region in Senegal is sustainable in the current climate scenario, while it could be potentially impacted by climate change in the near future. In a pessimistic climate change scenario, in situ water harvesting has the potential to avoid excessive crop losses.

Abstract Patent classification is one of the areas in Intellectual Property Analytics (IPA), and a growing use case since the number of patent applications has been increasing worldwide. We propose using machine learning algorithms to classify Portuguese patents and evaluate the performance of transfer learning methodologies to solve this task.

Abstract Checkpoint inhibitors are monoclonal antibodies that elicit an anti-tumor response by stimulating immune system. Their use has improved the treatment of different types of cancer such as melanoma, breast carcinoma, lung, stomach, colon, liver, renal cell carcinoma, and Hodgkin's lymphoma, but several adverse events have been reported. Although the etiology of these effects is not completely understood, an uncontrolled activation of the immune system has been postulated. Indeed, some studies showed a cross reactivity of T cells, which acted against tumor antigens as well as antigens in the tissues of patients who developed immune-related adverse events. Despite the known possibility of developing immune-related adverse events, early diagnosis, monitoring during therapy, and treatment are fundamental for the best supportive care and administration of immune checkpoint inhibitors. The aim of this review is to guide the clinician in early diagnosis, management, and treatment of the endocrinological adverse effects in the major endocrine glands (thyroid, pituitary, adrenal, endocrine pancreas, and parathyroid).

Abstract Background: Myeloproliferative neoplasms (MPNs) are often associated with splanchnic vein thrombosis (SVT). Not all the factors involved in the thrombotic tendency are currently known.
Objectives: This study aims to evaluate a possible association between ADAMTS13, von Willebrand factor (VWF), platelet microvesicles (MV), and factor VIII activity (FVIII:C) with thrombotic events in MPN patients.
Materials and methods: In total, 36 consecutive MPN patients with SVT were enrolled. The MPNs were diagnosed based on clinical characteristics and one or more gene mutations among JAK-2, CALR, and MPL. As controls, 50 randomly selected patients with MPN without thrombosis, 50 patients with deep vein thrombosis without MPNs, and 50 healthy blood donors were evaluated. Complete blood count, ADAMTS13, VWF, MV, and FVIII:C in plasma were measured in all the subjects.
Results: The JAK-2 mutation was found in 94% of the patients with SVT, but none were triple-negative for genetic mutations (JAK2 V617F, CALR, MPL, and exon 12). Compared to the normal subjects, in all the MPN patients (with or without SVT), the levels of ADAMTS13 were found to be significantly lower (
Conclusions: The significant increase in circulating MV, VWF, and FVIII:C in the MPN patients and in the patients with thrombosis supports the role of endothelium damage in promoting thrombotic events. In particular, a significant increase in VWF and FVIII:C levels was found in the MPN patients with SVT.
