    Abstract Background: Continuous enteral nutrition (CEN) remains standard practice in intensive care units (ICUs) worldwide. Intermittent enteral nutrition (IEN) may be a suitable alternative method. This meta-analysis aims to investigate the safety, tolerance, and effectiveness of IEN compared with that of CEN in critically ill adults in the ICU.
    Methods: MEDLINE, CINAHL, Scopus, Embase, and Google Scholar were searched for studies published up until April 30, 2020, along with manual searches in bibliographies. Eligible studies were trials that compared CEN with IEN feeding in the ICU and reported on at least one of the relevant outcomes (nutrition intake, gastric residual volume [GRV], aspiration pneumonia, vomiting, diarrhea, abdominal distension, and glycemic variability). Secondary outcomes were ICU length of stay (LOS) and mortality. The Cochrane risk-of-bias tool for randomized trials was used to assess methodological quality.
    Results: Ten studies with 664 participants were included in the meta-analysis. Most studies had an overall "high" risk of bias. Incidence of vomiting was significantly higher in CEN than in IEN groups (risk ratio, 2.76; 95% CI, 1.23-6.23). There were no significant differences between CEN and IEN groups for nutrition intake, GRV, incidence of aspiration pneumonia, diarrhea, abdominal distension, ICU LOS, and mortality. Definition and reporting of outcome measures were not standardized across studies; hence, this heterogeneity limits the generalizability of results.
    Conclusion: Overall, the safety, tolerance, and effectiveness of CEN and IEN were found to be comparable. Future studies should explore monitoring larger sample sizes to determine best feeding practices in the ICU.

    Abstract Research toward food waste valorisation has been an attractive subject and aligned with the UN’s Sustainable Development Goal (SDG) 12.3 aiming at drastic reduction of the ecological footprint via minimizing food losses by 2030. In this context, we draw our research focused on upcycling the rice wine residue (RWR) into a value-added ingredient. This study aimed to identify the colloidal properties of RWR and explore its potential application as a stabilising agent for foams or bubbles. Ultrasound treatment was employed at 20 kHz for 30 minutes to breakdown the rice grain and refine the particulates into smaller sizes. The particle sizes of RWR were characterized via Mastersizer and resulted in bimodal distribution with sizes of 0.12 and 27 μm. The surface net charge was found to be around -7.8 ±0.5 mV as measured via Zetasizer. The residue was dried and incorporated into a model foam system using 0.5 wt.% sodium caseinate to initiate the bubble formation. RWR at 0.5 wt.% has shown its ability to retain a stable foam over time for more than 6 hours. To conclude, RWR may potentially be deemed as a foam stabiliser via Pickering stabilization at the bubble interface.

    Abstract Purpose: To develop a current profile of prosthodontics program directors (PPDs) in United States dental schools and describe the current challenges and responsibilities of those serving in the position and assess the professional needs of the program directors (PDs) to better support them in their roles.
    Materials and methods: A survey was sent to 51 participants who were identified as PPDs in North America via Qualtrics in 2019 to assess the professional needs of PDs to better support their roles. The survey consisted of 29 items organized into six sections: (1) demographic information; (2) hours and time spent in various areas of the job; (3) greatest challenges; (4) perceived importance of specific competencies; (5) opinions on the greatest rewards of the position; (6) their needs for support from the American College of Prosthodontists and advice for the future PD.
    Results: The response rate for this survey was 88.3%. Of the 45 respondents, the majority were white/non-Hispanic, followed by Asian/Pacific Islanders. The two largest and equal numbers of participants who worked for more than 10 years and between 1 and 3 years were observed with approximately 29% each, and PDs who worked less than a year occupied the least amount. Overall, 34% of PDs are on the tenure track and 80% of those are tenured. Teaching and clinical services took away the greatest time with approximately 42% and 22.1%, respectively. Thirty-five (78%) PDs listed the greatest challenge they encountered was the workload of the role. Skills listed in leadership and personnel competencies were all rated more important than most of the skills listed in management.
    Conclusions: There is a wide gender gap and a lack of racial diversity among the PPDs. The biggest challenge faced by them is the amount of workload followed by budget and fiscal constraints. And for their improved efficiency, they gave emphasis on learning time management and continuous clinical training of newer advances.

    Abstract Transplantation of solid organs from donors with active SARS-CoV-2 infection has been advised against due to the possibility of disease transmission to the recipient. However, with the exception of lungs, conclusive data for productive infection of transplantable organs do not exist. While such data are awaited, the organ shortage continues to claim thousands of lives each year. In this setting, we put forth a strategy to transplant otherwise healthy extrapulmonary organs from SARS-CoV-2-infected donors. We transplanted 10 kidneys from five deceased donors with new detection of SARS-CoV-2 RNA during donor evaluation in early 2021. Kidney donor profile index ranged from 3% to 56%. All organs had been turned down by multiple other centers. Without clear signs or symptoms, the veracity of timing of SARS-CoV-2 infection could not be confirmed. With 8-16 weeks of follow-up, outcomes for all 10 patients and allografts have been excellent. All have been free of signs or symptoms of donor-derived SARS-CoV-2 infection. Our findings raise important questions about the nature of SARS-CoV-2 RNA detection in potential organ donors and suggest underutilization of exceptionally good extrapulmonary organs with low risk for disease transmission.

    Abstract Australia has the highest incidence of melanoma in the world. General practitioners encounter melanoma in 9.9 per 10,000 clinical encounters and play a key role in diagnosis. A systematic review was conducted to study the efficacy of training methods to improve general practitioners' diagnostic skills in melanoma. Article abstracts (1307) were screened, from a Medline search. Four trials met our criteria and were highly variable in their intervention methods and outcome measures. The Cochrane risk of bias tool was used to assess study quality with only one good, one poor, and two of questionable quality. Our results showed limited evidence via one study that training of general practitioners in surface microscopy improved melanoma diagnosis, from a clinical (naked eye) pre-intervention score of 54.6 % to a post-intervention surface microscopy score of 75.9 % in 74 general practitioners. Future work should explore the barriers to implementing this strategy in clinical practice.

    Abstract This study aimed to investigate and compare the color stability and stainability of computer-aided design/computer-aided manufacturing (CAD/CAM) restorative materials in their glazed (G) and polished (P) state when exposed to cigarette smoke, as well as after brushing. Three CAD/CAM restorative materials were investigated: lithium disilicate CAD (LD), zirconia (Zr), and Telio PMMA CAD (PMMA), according to their surface finishing and assignment to cigarette smoking exposure or soaking in the saliva (control) group. The color change (∆E) was calculated before and after the intervention performed for all specimens, using the L*a*b values to quantitatively assess the shade differences. Statistical analysis was performed using one-way repeated measures ANOVA and Bonferroni multiple comparison analysis (α = 0.05). The surface finishing did not influence the materials’ stainability. Color change was noted after smoking, LD and Zr-G and Zr-P had a comparable color change (p > 0.05), while PMMA presented lower ∆E values (p < 0.05). After brushing, all specimens had a significant color change that was high for LD-G and LD-P, and Zr-G, compared with Zr-P and PMMA (p < 0.05). In conclusion, the exposure to cigarette smoke showed that LD, Zr, and PMMA are all susceptible to staining, but brushing decreases surface staining.

    Abstract Objectives: We examined the demographics and the clinical profile of teleconsultations conducted between an acute geriatric medicine department and 8 nursing homes over a period of 6.5 years.
    Design: This is a prospective, descriptive study.
    Setting and participants: This study was conducted in 8 nursing homes of Singapore. All nursing home patients referred to the program for teleconsultation between December 2010 and May 2017 were enrolled in the study.
    Measures: The unit observed and studied was the individual telemedicine consultation. Each unique patient contributed 1 or more observation points. The data collected included consultation dates, patient demographics, background medical comorbidities, reasons for referral, physical examination findings, primary diagnoses, and general management plans recommended by the doctors.
    Results: There were 1673 teleconsultations conducted with 850 unique patients. Most of these patients were categorized as having moderate to severe disability. Ninety-five percent of the consultations were scheduled. The main reasons for referral were for medication review (47.6%) and behavioral problems (22.1%). The 4 commonest comorbidities were hypertension (57.0%), dementia (40.0%), diabetes (39.0%), and hyperlipidemia (38%). The most common primary diagnosis was dementia (21.6%), of which 227 of those diagnosed (62.8%) were referred for behavioral and psychological symptoms of dementia.
    Conclusions and implications: A telemedicine service for older patients is feasible and could be of a high quality, especially if made available in places where a high density of the latter reside, such as in the care homes and other nursing and rehabilitation facilities.

    Abstract Objective: Despite being the third commonest cancer in Singaporean men, there is a dearth of basic data on the detection rate of prostate cancer and post-procedure complication rates locally using systematic 12-core biopsy. Our objective is to evaluate prostate cancer detection rates using 12-core prostate biopsy based on serum prostate specific antigen (PSA) levels and digital rectal examination (DRE) findings in Singaporean men presenting to a single tertiary centre. The secondary objective is to evaluate the complication rates of transrectal prostate biopsies.
    Methods: We retrospectively examined 804 men who underwent first transrectal-ultrasound (TRUS) guided 12-core prostate biopsies from January 2012 to April 2014. Prostate biopsies were performed on men presenting to a tertiary institution when their PSA levels were ≥4.0 ng/mL and/or when they had suspicious DRE findings.
    Results: Overall prostate cancer detection rate was 35.1%. Regardless of DRE findings, patients were divided into four subgroups based on their serum PSA levels: 0-3.99 ng/mL, 4.00-9.99 ng/mL, 10.00-19.99 ng/mL and ≥20.00 ng/mL and their detection rates were 9.5%, 20.9%, 38.4% and 72.3%, respectively. The detection rate of cancer based on suspicious DRE findings alone was 59.2% compared to 36.5% based on serum PSA cut-off of 4.0 ng/mL alone. The post-biopsy admission rate for sepsis was 1.5%.
    Conclusion: In conclusion, using contemporary 12-core biopsy methods, the local prostate cancer detection rate based on serum PSA and DRE findings has increased over the past decade presumably due to multiple genetic and environmental factors. Post-biopsy sepsis remains an important complication worldwide.
