Abstract William Louis (Bill) Stern passed away on 1 November 2021 at the age of 95. Bill had a long and illustrious career as a botanist. He was a life member of IAWA. From 1951 to around 1982 his focus was on wood anatomy and then his attentions moved on to orchids, culminating in volume 10 of the Anatomy of the Monocotyledons: Orchidaceae in 2014. Bill’s last major wood publication was the incredibly useful Index Xylariorum III (IX3) in 1988, of which he had published a first version in 1957. There’s a photo in IAWA Bulletin 9(4): 394 of him receiving the first printed copy of IX3 from Pieter Baas in Madison in October 1988. Peter first met Bill at Kew and inherited the task of compiling the next version IX4, quickly realising just how big a task this was, especially since Bill hadn’t had the benefits of email! The latest iteration of IX4 can be found on both the IAWA and GTTN websites. Thirty-three years since IX3 younger wood anatomists may not realise just how influential Bill has been in the wood science world. After spending 1944–1946 in the US Naval Reserve he went to Rutgers University where he gained his BS in 1950 and then the University of Illinois for his MS in 1951 and PhD in botany 1954. From 1953 to 1960 he was instructor and curator and then professor of wood anatomy at Yale University School of Forestry and then moved to the Division of Woods in the Department of Botany at the Smithsonian Institution until 1967 when he became Professor of Botany at the University of Maryland until 1979. The Yale wood collection is now at the USDA Forest Service Forest Products Lab in Madison under the acronym SJRw, in honour of its founder Samuel J. Record.

Abstract William Hunter along with his brother, John, and their colleagues William Hewson, William Cruikshank and John Sheldon made a large contribution to understanding of lymphatic vessels. Hewson, Cruikshank and Sheldon all carried out mercury injections and made much progress in mapping the distribution of lymphatics in the human body. William Hunter appreciated that lymphatics absorbed fluid from the tissues of the body and that lacteals of the intestine and lymphatics are similar structures. John Hunter carried out an elegant series of experiments that proved that lacteals absorb products of digestion. The Hunters, however, were wrong in dismissing absorption by blood vessels and missed the importance of blood capillaries. William Hewson showed that lymphatics were not confined to mammals but that they are present in reptiles, birds and fish. Hewson also demonstrated that tracheobronchial glands are lymph nodes and not mucus-secreting glands as previously thought. Although William Hunter appreciated that tuberculosis and venereal diseases might involve the regional lymph nodes, he does not seem to have fully grasped that malignant disease might involve the local nodes or the concept that knowledge of lymph drainage could be used to define the likely site of a primary malignancy.

Abstract Purpose: We examined the yield from EMFIT bed alarms and staff response time to generalised seizure in a medium term residential assessment unit for epilepsy.
Methods: The Scottish Epilpesy Centre (SEC) has a Video Observation System (VOS) that provides continuous recording of all patient spaces (external and internal) and allows retention of clinically relevant events. A retrospective audit of daily EMFIT test records, nursing seizure record sheets (seizure type and EMFIT alert status), clinical incident reporting systems and the VOS database of retained clinical events was conducted for an 9 month period from April 1st 2016 till December 31st 2016. All generalized tonic clonic seizures (GTCS) were noted by patient, time and location and staff response time to GTCS was calculated.
Results: There were 85 people admitted during the audit period who had 61 GTCS. 50 events were in bed and EMFIT alert status was recorded. On 8 occasions the EMFIT did not alert: 5 events were not of sufficient duration or frequency, in 2 the patient fell from the bed early and 1 event the alarm did not trigger. The average response time to GTCS was 23s. The longest response time was 69s (range, 0-69s, sd 15.76.).
Conclusions: The EMFIT bed alarm appears to be a valuable adjunct to safety systems. Within the novel environment of the SEC it is possible to maintain a response time to GTCS that is comparable to hospital based UK video telemetry units.

Abstract Background: Osler taught that splenic infarction presents with left upper abdominal quadrant pain, tenderness and swelling accompanied by a peritoneal friction rub. Splenic infarction is classically associated with bacterial endocarditis and sickle cell disease.
Objectives: To describe the contemporary experience of splenic infarction.
Methods: We conducted a chart review of inpatients diagnosed with splenic infarction in a Jerusalem hospital between 1990 and 2003.
Results: We identified 26 cases with a mean age of 52 years. Common causes were hematologic malignancy (six cases) and intracardiac thrombus (five cases). Only three cases were associated with bacterial endocarditis. In 21 cases the splenic infarction brought a previously undiagnosed underlying disease to attention. Only half the subjects complained of localized left-sided abdominal pain, 36% had left-sided abdominal tenderness; 31% had no signs or symptoms localized to the splenic area, 36% had fever, 56% had leukocytosis and 71% had elevated lactate dehydrogenase levels. One splenectomy was performed and all patients survived to discharge. A post hoc analysis demonstrated that single infarcts were more likely to be associated with fever (20% vs. 63%, p < 0.05) and leukocytosis (75% vs. 33%, P = 0.06)
Conclusions: The clinical presentation of splenic infarction in the modern era differs greatly from the classical teaching, regarding etiology, signs and symptoms. In patients with unexplained splenic infarction, investigation frequently uncovers a new underlying diagnosis.
