    Abstract Objective To investigate the relationship between different serum uric acid (SUA) levels after admission and the incidence of cardiac major adverse cardiac events (MACE) in the vulnerable stage of heart failure after discharge in patients with acute heart failure (AHF). Methods Clinical data of AHF patients admitted to our department from October 2019 to July 2021 were collected, and all of them were followed up for 3 months. According to whether MACE events occurred during the vulnerable period after discharge, the patients were divided into event group and control group. The differences of the baseline data were compared between the 2 groups, and the factors that may affect the MACE events in the vulnerable period were analyzed with multivariate logistic regression analysis. The correlation between high SUA level and prognosis in the vulnerable period was analyzed. Results A total of 539 hospitalized AHF patients were included in this study, including 194 (35.9%) in the event group and 345 (64.1%) in the control group. In the event group, 175 cases (90.3%) had high SUA level and 19 cases (9.7%) had normal SUA level (P < 0.05). The event group had higher levels of SUA, D-dimer, blood urea nitrogen, brain natriuretic peptide (BNP) and NYHA cardiac function class, left atrial anteroposterior diameter, and left ventricular transverse diameter (P < 0.05), while lower body mass index (BMI), mean arterial pressure, serum sodium, albumin, and total cholesterol (TC) (P < 0.05). Logistic regression analysis showed that high SUA (OR=3.419, 95%CI: 1.805~6.477, P < 0.05) was an independent risk factor for MACE events in the vulnerable period of AHF patients. Conclusion The AHF patients with high SUA level are prone to the occurrence of MACE events in the vulnerable period after discharge. SUA is a helpful indicator for evaluating the prognosis of AHF patients in the vulnerable period.

    Abstract Background and aim: Patients with end-stage liver disease (ESLD) are susceptible to invasive pulmonary aspergillosis (IPA). This study aimed to investigate the risk factors affecting the occurrence and short-term prognosis of ESLD complicated by IPA.
    Methods: This retrospective case-control study included 110 patients with ESLD. Of them, 27 ESLD-IPA received antifungal therapy with amphotericin B (AmB); 27 AmB-free-treated ESLD-IPA patients were enrolled through 1:1 propensity score matching. Fifty-six ESLD patients with other comorbid pulmonary infections were enrolled as controls. The basic features of groups were compared, while the possible risk factors affecting the occurrence and short-term outcomes of IPA were analyzed.
    Results: Data analysis revealed invasive procedures, glucocorticoid exposure, and broad-spectrum antibiotic use were independent risk factors for IPA. The 54 patients with ESLD-IPA exhibited an overall treatment effectiveness and 28-d mortality rate of 50.00% and 20.37%, respectively, in whom patients treated with AmB-containing showed higher treatment efficacy than patients treated with AmB-free antifungal regimens (66.7% vs. 33.3%, respectively, χ
    Conclusions: Glucocorticoid exposure, invasive procedures, and broad-spectrum antibiotic exposure increased the risk of IPA in ESLD patients. AmB alone or combined with other antifungals may serve as an economical, safe, and effective treatment option for ESLD-IPA.

    Abstract Objective To observe and analyze the efficacy, the effect on cardiac structure and function and the dosage of sacubitril/valsartan (ARNI) in real clinical practice in patients with heart failure with reduced ejection fraction (HFrEF). Methods A total of 118 HFrEF patients treated with ARNI in our hospital from September 2017 to February 2020 were recruited in this study. Their clinical data before and after treatment were collected. After all of them were followed up for a median time of 8 (6 to 12) months, the changes of New York Heart Association (NYHA) classification of cardiac function and myocardial structure, and the readmission due to heart failure were analyzed retrospectively. The side effects and current situation of drug use were also observed among these patients. Results After ARNI treatment, the improvement of NYHA grade ≥1 accounted for 83.1% of the HFrEF patients, and the total effective rate of treatment was 83.1%. They obtained significantly increased left ventricular ejection fraction (LVEF) (P < 0.001), and obviously decreased left atrial anterior and posterior diameter (LAD), left ventricular end-diastolic diameter (LVEDD), right atrial left-right diameter (RA) and right ventricular left-right dimension (RV) (P < 0.001). During the follow-up period, the target dose (200 mg, bid) rate of ARNI was 3.4% and the readmission rate of heart failure was 24.6%. Adverse reactions of drugs included symptomatic hypotension (14 cases, 11.9%) and skin pruritus (2 cases, 1.7%). Conclusion Although ARNI has the problem of being maintained at low dose in clinical application, it can still significantly improve the cardiac structure and function of HFrEF patients. Symptomatic hypotension is the common adverse reaction in the use of ARNI and the main reason for the low rate of ARNI target maintenance dose.

    Abstract Objective To evaluate the impact of baseline pulmonary artery systolic pressure (PASP) on cardiac function, left ventricle reverse remodeling and clinical outcomes in patients receiving cardiac resynchronization therapy (CRT). Methods A total of 73 patients undergoing CRT in our hospital between June, 2014 and December, 2017 were enrolled in this retrospective analysis. According to baseline PASP estimated by echocardiography, the patients were divided into pulmonary artery hypertension (PAH) group (PASP≥ 50 mmHg; n=27) and non-pulmonary artery hypertension (NPAH) group (PASP < 50 mmHg; n=46) for comparison of cardiac function, electrocardiogram and echocardiogram at 6 months after CRT. A receiver operator characteristic (ROC) curve was used to evaluate the value of baseline PASP in predicting super-response to CRT. The combined endpoints including all-cause mortality and rehospitalization for heart failure were compared between the 2 groups using Kaplan-Meier analysis. Cox regression models were constructed to analyze the risk factors for long-term prognosis of the patients. Results The 73 patients enrolled included 52 male and 21 female patients at a mean age of 60.32±9.78 years. At 6 months after the operation, the patients in PAH group had significantly poorer improvements in cardiac functions, left ventricular ejection fraction (LVEF) and left ventricular end-diastolic diameter (LVEDD) than those in NPAH group (P < 0.05), and showed no obvious improvement in LVEDD (P > 0.05). ROC curve analysis showed that PASP ≤35 mmHg was capable of predicting super-response to CRT with an area under the ROC curve of 0.709 (95%CI: 0.575-0.843, P < 0.05). The patients in PAH group had a higher incidence of combined endpoints than those in NPAH group. PAH (HR=3.64, 95%CI: 1.25-10.61, P < 0.05) and left atrial diameter (HR=1.10, 95%CI: 1.01-1.20, P < 0.05) were independent predictors for the clinical outcomes of the patients.     Abstract The research on safe and efficient hydrogen storage methods is the key scientific and technological issue which restricts the implementation of hydrogen economy. Lithium amidoborane (LiNH2BH3) has received extensive attention for its excellent hydrogen storage properties. It can desorb 10.9 wt% of hydrogen below 100 °C without deleterious by-product such as borazine. What is more, it is chemically stable at room-temperature conditions. The preparation methods and crystal structure of LiNH2BH3 were reviewed at length in this paper, and its dehydrogenation performance and mechanisms were analyzed combined with the theoretical calculation results. Moreover, the barriers and improvement measures in hydrogen release thermodynamics/kinetics and regeneration of the spent fuel were summarized. It is proposed that reducing the synthesis cost, decreasing dehydrogenation temperature with increased rates, and exploring high-efficiency, low-energy consumption, and low-cost regeneration techniques are the critical striving directions of lithium amidoborane. To the best of our knowledge, this work is the first review focusing on LiNH2BH3, one of the most promising materials for onboard hydrogen storage application, which is expected to give clues for the next advance of LiNH2BH3.
