Abstract [Purpose] The aim of this study was to develop a novel wearable surface electromyograph called NOK, and compare its reliability and validity to an existing electromyograph. [Participants and Methods] The study participants were 23 healthy university students (Seven males and 16 females; age 20.3 ± 1.1 years [mean ± standard deviation]; height 162.0 ± 6.7 cm; weight 58.4 ± 10.1 kg) who all gave informed written consent. The newly developed electromyograph (NOK) features a rubberized skin contact surface that requires no electrodes and allows the acquisition of up to 10 channels of muscle waveforms on a portable personal computer. After measuring maximal isometric elbow extension and flexion, we examined muscle waveforms during isometric contractions of elbow joint flexion and extension at approximately 50% of maximal voluntary contraction using both NOK and Delsys electromyographs and compared the results of the two devices. [Results] We found a significant moderate correlation between the measurements by the two devices for biceps and triceps. The measurements by the two devices also showed strong measure-retest reliability. Systematic errors were observed for elbow flexion and extension in the two measurements, indicating limited agreement between the two measurement methods. [Conclusion] Although the new device also has high repeatability and reliability, it is unsuitable for analyzing detailed muscle activity. However, since it can measure up to 10 channels of muscle activity, it is expected to be used in the rehabilitation and sports field in the future.

Abstract Background/aim: Although certain treatment options exist for intestinal incontinence, none are curative. Adipose-derived stem cells (ADSCs) have emerged as promising therapeutic agents, but most preclinical studies of their effectiveness for anal function have used autologous or allogeneic ADSCs. In this study, the effectiveness, timing of administration, and required dosage of human ADSCs were investigated for clinical application.
Materials and methods: A 10-mm balloon catheter was used to induce anal sphincter injury in immunodeficient mice in the following experimental groups (n=4 per group): ADSC (injected ADSCs after injury), PBS (injected phosphate-buffered saline after injury), and control (uninjured). The effects of different timing (immediately after injection and 30 days following injury) and number of human ADSCs administered was compared among groups based on defecation status and pathological evaluation.
Results: In terms of defecation status, groups receiving ≥1×10
Conclusion: Human ADSC administration in a mouse model of anal sphincter injury was effective. Injection of ≥1×10

Abstract Background: There has been a significant increase in scientific investigations of the hearing-dementia association among the research on potentially modifiable risk factors for cognitive impairment. We tested two clinical questions. Analysis 1: does persistent hearing aid (HA) use decrease the decline in cognitive function caused by ageing? Analysis 2: does cognitive function at the time of HA fitting predict future persistent HA use?
Methods: This case-control study performed at two referral centres reported data obtained over a 4.5-year period. We recruited a group of patients with cognitive decline, aged 65 or older with or without hearing loss. The intervention consisted of the use of HAs. The primary outcome measures were adherence to continuous HA use and cognitive function measured using the Japanese version of the Mini-Mental State Examination Test and the Reading Cognitive Test Kyoto.
Results: Eighteen HA users and 18 controls were included in the first analysis. HA use was associated with a deceleration of cognitive decline 12 months later. In the second analysis, 11 participants with good adherence to HA use were compared with 12 participants who showed poor adherence to HA use. Among the variables employed in this study, cognitive function measured using the Reading Cognitive Test Kyoto was significantly lower in participants with poor adherence to HA.
Conclusions: HA use in cognitively impaired individuals with hearing loss can slow age-related cognitive decline. Cognitively impaired people with hearing loss who fail to commit to HA use tend to have lower cognitive measurement scores before HA fitting. HA use is generally more challenging as people age and their cognitive abilities decline. Therefore, it is desirable that HAs be used when hearing loss and dementia are in their early stages.

Abstract Background: Perineural catheters placed parallel to the nerve course are reported to have lower migration rates than those placed perpendicular to it. However, catheter migration rates for a continuous adductor canal block (ACB) remain unknown. This study compared postoperative migration rates of proximal ACB catheters placed parallel and perpendicular to the saphenous nerve.
Methods: Seventy participants scheduled for unilateral primary total knee arthroplasty were randomly assigned for parallel or perpendicular placement of the ACB catheter. The primary outcome was the migration rate of the ACB catheter on postoperative day (POD) 2. Catheter migration was defined as being unable to confirm saline administration via the catheter around the saphenous nerve at the mid-thigh level under ultrasound guidance. Secondary outcomes included active and passive range of motion (ROM) of the knee on postoperative rehabilitation.
Results: Sixty-seven participants were included in the final analyses. The catheter migrated significantly less often in the parallel group (5 of 34 (14.7%)) than in the perpendicular group (24 of 33 (72.7%)) (p<0.001). The mean (SD) active and passive knee flexion ROM (degrees) improved significantly in the parallel than in the perpendicular group (POD 1: active, 88.4 (13.2) vs 80.0 (12.4), p=0.011; passive, 95.6 (12.8) vs 85.7 (13.6), p=0.004; POD 2: active, 88.7 (13.4) vs 82.2 (11.5), p=0.036; passive, 97.2 (12.8) vs 91.0 (12.0), p=0.045).
Conclusion: Parallel placement of the ACB catheter provided a lower postoperative catheter migration rate than perpendicular placement of the ACB catheter along with corresponding improvements in ROM and secondary analgesic outcomes.
