    Abstract Since 2003, the Research Institute of Organic Agriculture (FiBL) is realizing a herd health management programme (“pro-Q” project) focussing on udder health and the use of homeopathic based treatment and prevention. Aims of the project are: (1) to reduce antibiotic mastitis treatments, (2) to optimize udder health and (3) to improve longevity, measured as averaged herd lactation number. The farms get expert advice on prevention and treatment at herd- and animal-level. After 2 years, treatment recordings of the 78 investigated farms showed that antibiotic mastitis therapies were reduced from 37 to 26 treatments per 100 cows and year (equals a reduction of 28%). The mean herd lactation number increased significantly by 0.2 lactations from 3.3 to 3.5 lactations per cow. Udder health remained constant over all farms during 2 years: theoretical bulk milk cell counts averaged constantly at approximately 180'000 cells/ml.

    Abstract Zusammenfassung Ziel des pro-Q-Projektes ist die flächendeckende Tiergesundheits- und Milchqualitätssicherung in der Biomilcherzeugung unter Bioverordnungs-konformen Bedingungen. Die Milch soll mit einem minimalen Einsatz chemischer Arzneimittel produziert werden. Dabei soll eine gute Eutergesundheit gewährleistet werden. Um diese Ziele zu erreichen, werden wichtige Negativfaktoren speziell für die Eutergesundheit im Betrieb definiert. Sanierungskonzepte werden mit der Landwirtin / dem Landwirt und dem Tierarzt / der Tierärztin erarbeitet. Zur Darstellung der aktuellen Eutergesundheit werden die Kühe und ihre Euter klinisch, die Milch zytobakteriologisch untersucht. Gemeinsam mit den Bestandesveterinären wird ein Therapiekonzept für erkrankte Kühe erstellt, welches auf vorwiegend homöopathischer Behandlung in Verbindung mit zusätzlichen Massnahmen (z.B. häufigerem Ausmelken) beruht. Wesentliches Ziel ist dabei, das Therapiesystem derart umzustellen, dass der Einsatz von Antibiotika weitgehend vermieden wird. Ein wesentlicher Teil des Projektes ist die stetige Analyse aller verfügbaren Daten mit darauf basierenden umfangreichen Informationen für Landwirt und Tierarzt. Die Ergebnisse der 78 mind. einjährig und 16 über zwei Jahre betreuten Betriebe sind sehr unterschiedlich und müssen im Kontext der betriebsindividuellen primären Sanierungsstrategien gesehen werden. Während die Eutergesundheit sowie die Milchleistung im ersten Jahr kaum Unterschiede zeigen, werden die Herden im Durchschnitt etwas älter, insbesondere diejenigen mit primär besserer Eutergesundheit. Nach zwei Jahren zeigen sich in Betrieben mit schlechterer Eutergesundheit deutliche Verbesserungen der Zellzahl, während die eutergesünderen Betriebe sich verschlechtern. Dies ist in erster Linie auf die Umstellung auf antibiotikafreie Therapie zurückzuführen, wobei der Trend zum 3. Jahr hin wieder rückläufig ist. Es konnten bereits im ersten Jahr 15 von 19 Betrieben mit S. aureus-Problematik teilsaniert werden, 5 Herden davon sind vollständig saniert. ...

    Abstract Data of mastitis causing factors will be collected on 100 new farms per year in Switzerland: housing, feeding, human-animal-interaction, milking technology, milking hygiene. These data will be connected to the mastitis status of the herd based on quarter milk samples and milk recording data. During a period of at least 2 years these farms will be intensively advised by the project team and their practical veterinarians. Therapies will primarily be based on homeopathic remedies. The development of mastitis causing factors and the mastitis status of the farms is followed up at regular intervals to show possible correlations between (changing) factors and mastitis status. In addition, a network of health data should be implemented for providing information for farmers and veterinarians in preventive herd health management. The aims of the project are reduction in the use of antibiotics in udder treatment, improvement of the udder health state of the herds and, as a consequence, improvement of milk quality.

Im kommenden Jahr soll das Programm um die Bereiche Fruchtbarkeit und Jungtiergesundheit erweitert werden. Neue Milchviehbetriebe, die bei pro-Q mitmachen wollen, sind willkommen.

    Abstract The pro-q-project is an udder health programme especially for organic farmers under Swiss conditions. The aim is to minimize the use of s antibiotics and to assure a good udder health state on the farms. At the end of the third project year of 100 project farms 16 participate for more than two years. The udder health situation of these 16 farms improved moderately in the second project year (37% of the cows with a so-matic cell count > 100’000/ml) compared to the year before project start (40% of the cows with a somatic cell count > 100’000/ml). Furthermore, a nearly linear increase of the arithmetic mean of herd lactation number (3.27 year before project start, 3.33 first project year, 3.44 second project year, resp.) could be found. The number of antibiotic treatments per cow and year decreased from 0.37 in the year before project start to 0.24 in the second project year.

    Abstract The objective of this study was to assess effects on intramammary infections and somatic cell score (SCS) upon reduction antibiotic udder treatments. The study was part of an udder health programme (pro-Q) implementing preventive measures and complementary treatment methods. The treatment proportion decreased from annually 39 to 20 per 100 cows. A total of 10'000 of quarter milk samples out of 4 yearly investigations of 33 Swiss dairy herds were analysed .There was no general change in somatic cell score (SCS) considering all taken samples. A significant increase of Staphylococcus ssp. findings (12%) could be observed beside a decrease of the SCS in samples of this pathogens group. Using GLM statistical analysis, significant effects by lactation number, days in milk, pathogens and farm on SCS could be shown.

    Abstract Acute respiratory distress syndrome (ARDS) is a common condition in intensive care medicine. Various intra- and extrapulmonal causes may trigger an epithelial and endothelial permeability increase, which leads to impaired gas exchange due to fluid overload of the alveoli and transmigration of leukocytes. This results in hypoxemia and hypercapnia, as well as deleterious consequences for the macro- and microcirculation with the risk of multi-organ failure and high mortality. This review summarizes ARDS pathophysiology and clinical consequences.

    Abstract The use of temporary mechanical circulatory support (tMCS) devices and in particular the increasing use of the Impella device family has gained significant interest over the last two decades. Nowadays, its use plays a well-established key role in both the treatment of cardiogenic shock, and as a preventive and protective therapeutic option during high-risk procedures in both cardiac surgery and cardiology, such as complex percutaneous interventions (protected PCI). Thus, it is not surprising that the Impella device is more and more present in the perioperative setting and especially in patients on intensive care units. Despite the numerous advantages such as cardiac resting and hemodynamic stabilization, potential adverse events exist, which may lead to severe, but preventable complications, so that adequate education, early recognition of such events and a subsequent adequate management are crucial in patients with tMCS. This article provides an overview especially for anesthesiologists and intensivists focusing on technical basics, indications and contraindications for its use with special focus on the intra- and postoperative management. Furthermore, troubleshooting for most common complications for patients on Impella support is provided.

    Abstract In organic livestock production a high status of animal health should be reached and maintained mainly with preventive measures. The Swiss pro-q project provides and example of how to change the focus from treating diseases to keeping animals healthy on organic farms. Data of the mastitis status and the use of antibiotics of about 100 Swiss organic dairy herds have been collected since 2003. Quarter milk samples and milk recording data are used as criteria to determine the mastitis status. During a period of at least 2 years the project farms will be intensively advised by the project team in relation to observed mastitis causing factors in housing, feeding, human-animal-interaction, milking technology and milking hygiene. Therapies are primarily based on homeopathic remedies. The development of the mas-titis status of the farms is followed up at regular in-tervals to show possible correlations between exten-sion inputs and mastitis status. In addition, a data-base has been set up to provide information for farm-ers and veterinarians in preventive herd health man-agement. The aims of the project are reduction of the use of antibiotics in udder treatment, improvement of the udder health status of the herds and, as a conse-quence, improvement of milk quality.
