    Abstract Objective: To evaluate the accuracy of transvaginal ultrasound in the diagnosis of adenomyosis using MRI as the gold standard, and to characterise the most commonly seen and accurate ultrasonographic features and their combination.
    Study design: Cross-sectional, descriptive study. Place and Duration of the Study: Department of Radiology, The Aga Khan University Hospital, Karachi, from January 2018 to July 2021.
    Methodology: Transvaginal ultrasound examination was performed on patients (n = 208) who presented with symptoms related to menstrual cycles and pelvic abnormalities. Additionally, patients who sought infertility evaluation were also included in the study. The findings from the ultrasound examinations were assessed and tabulated alongside the results of the MRI scans. All examinations were conducted by senior radiologists / sonographers. The sensitivity, specificity, positive and negative predictive values (PPV and NPV, respectively) of ultrasound features were calculated individually and in combination, taking MRI as the gold standard. To enhance the accuracy of ultrasound findings, various variables were combined, and their sensitivities and specificities were calculated.
    Results: Overall, transvaginal ultrasound had a high specificity of 96.15% (95% CI: 85.67 - 99.33), a relatively low sensitivity of 74.36% (95% CI: 66.63 - 80.85), PPV of 98.31 (95% CI: 93.40 - 99.70) and NPV of 55.56 (95% CI: 44.73 - 65.90). The most sensitive dual variable used was a bulky uterus combined with altered myometrial echotexture, with a sensitivity of 72.97% (95% CI: 64.95 - 79.78) and specificity of 95.83% (95% CI: 84.57-99.27). The best combined triple variable was a bulky uterus with altered echotexture and streaky myometrium, with a sensitivity of 71.85% (95% CI: 63.35 - 79.10) and a specificity of 95.46% (95% CI: 83.30 - 99.21).
    Conclusion: Transvaginal ultrasound features can identify adenomyosis characteristics in most of the patients. This could reduce the number of pelvic MRIs performed for the detection of adenomyosis.

    Abstract Objective: To compare the results of local administration of platelet-rich plasma (PRP) with methylprednisolone in the treatment of tennis elbow.
    Methods: This retrospective cohort was conducted at Jinnah Postgraduate Medical Center (JPMC) during January 2017 to April 2018. Patients conservatively managed for lateral epicondylitis with local methylprednisolone injection or PRP injection were approached for possible inclusion in the study at 12 months of treatment. The primary outcome of the study was to determine the Numerical Pain Rating Score (NPRS) on resisted wrist extension. Whereas, the secondary outcomes were quick disability arm, shoulder, and hand score (qDASH), the grip strength and VAS for satisfaction. The baseline, six weeks and three month data on Grip strength, NPRS, and qDASH were extracted from the patients' medical records maintained at the hospital. The data were analyzed by using SPSS software.
    Results: A total of 91 patients were approached, of them 81 (89.01%) agreed to participate. There were 46 (56.79%) who received local methylprednisolone injection and 35 (43.20%) received PRP. At 12 months follow up, there was no difference in NPRS pain scores between the two groups (p=0.691); pain decreased in both groups at six weeks and at 12 months. There was no significant difference in the functional outcome (qDASH score) in both groups. Both groups were equally satisfied with the treatment they had received.
    Conclusion: The study concluded that there is no difference between outcome and efficacy of both treatment modalities used for the treatment of tennis elbow in alleviating pain at 12 months.

    Abstract Real-time gait event detection (GED) system can be utilized for gait analysis and tracking fitness activities. GED for various types of terrains (e.g., stair-walk, uneven surfaces, etc.) is still an open research problem. This study presents an inertial sensor-based approach for real-time GED system that works for diverse terrains in an uncontrolled environment. The GED system classifies three types of terrains, i.e., flat-walk, stair-ascend and stair-descend, with an average classification accuracy of 99%. It also accurately detects various gait events, including, toe-strike, heel-rise, toe-off, and heel-strike. It is computationally efficient, implemented on a low-cost microcontroller, works in real-time and can be used in portable rehabilitation devices for use in dynamic environments.

    Abstract Chemical graph theory is a multidisciplinary field where the structure of the molecule is analyzed as a graphical structure. Chemical descriptors are one of the most important ideas employed in chemical graph theory; this is to associate a numerical value with a graph structure that often has correlation with corresponding chemical properties. In this paper, we investigate another very important closed-packed usual crystal structure defined as HCP (Hexagonal Close-Packed) crystal structure and its lattice formed by arranging its unit cells in a dimension for topological descriptors based on a neighborhood degree, reverse degree, and degree.     Abstract Gastroparesis is a disorder with few available treatments and delayed stomach emptying. Gastric electrical stimulation (GES) has shown promise in treating the signs and symptoms of gastroparesis as well as gastric emptying by stimulating the stomach with high-frequency electrical impulses. In this case, a 43-year-old lady with refractory gastroparesis had a GES device laparoscopically implanted. Even though GES seems promising, more study is necessary to improve patient choice, technique, and long-term results. Patients with refractory gastroparesis who have not responded to traditional therapy should be considered for GES, with treatment decisions being made individually depending on clinical presentation and patient preferences.

    Abstract The novel coronavirus infection (COVID-19) caused by the SARS-CoV-2 virus is a relatively recent infection that has resulted in a global pandemic, appearing first at the end of 2019. While initially presenting as a predominantly respiratory disease, with a classical picture of fever, dry cough, dyspnea and, in some cases anosmia and ageusia, recent cases have shown increasingly atypical and more systemic manifestation of the disease. A precise understanding of the extent and pathophysiology of COVID-19 remains underway to this day, particularly concerning its behavior in the pediatric population. Moreover, there has been an increasing number of COVID-19 reports with neurological complications and manifestations, prompting inquiry into neuroinvasion. Postulations include indirect invasion through a surge of inflammatory mediators "cytokine storm" and subsequent widespread endothelial injury; and direct neural tropism. We report the case of a previously healthy 12-year-old male presenting with acute right-sided hemiparesis, new-onset seizures and a generalized petechial rash. Laboratory tests revealed elevated inflammatory markers and radiological investigations confirmed an evolving left middle cerebral artery (MCA) infarct and large vessel vasculitis. Testing for SARS-CoV-2 infection was positive.
