    Abstract Phanuphak et al. compared three strategies for first-line antiretroviral therapy in 150 Thai patients: initiating therapy with zidovudine (AZT), tenofovir disoproxil fumarate (TDF), or a 24-week lead-in phase with stavudine (d4T) followed by a switch to AZT. Those taking d4T had higher haemoglobin levels and CD4(+) T-cell counts without an increase in neuropathic symptoms, peripheral neuropathy or lipoatrophy compared with those on AZT. Because AZT is associated with more short-term side effects and toxicity than d4T, and because most d4T toxicity occurs only after long-term use, this approach may have advantages over initial use of AZT. However, TDF-based regimens, while more expensive, are more effective, better tolerated, less toxic, less likely to lead to cross-resistance, and possibly more cost-effective. The goal in resource-limited settings should be to move away from use of thymidine analogues in first-line regimens.

    Abstract Despite recent improvements in the efficacy, safety, tolerability, and convenience of antiretroviral therapy for patients, the management of HIV infection remains complex for clinicians. Multiple studies have shown better clinical outcomes and lower cost of care when HIV-infected patients are managed by experts. However, generalists are frequently involved in the care of patients with HIV infection, in many cases providing primary care in collaboration with an HIV expert. Generalists also play a critical role in the diagnosis and prevention of HIV infection. Generalists managing HIV-infected patients should be aware of the components of the initial patient evaluation. They should be familiar with the general principles of antiretroviral therapy and opportunistic infection prevention. They should be able to recognize antiretroviral toxicity and should be aware of common drug-drug interactions involving antiretroviral agents.

    Abstract Individual health benefits of antiretroviral therapy (ART) are becoming clearer. In resource-rich countries, side effects of current ART regimens are minimal. US guidelines recommend ART regardless of CD4 count or viral load. Maintaining an undetectable viral load with ART comes close to eliminating the risk of HIV transmission, leading the US guidelines to recommend universal ART to reduce HIV transmission. Achieving population-level control through treatment as prevention (TasP) may be feasible, but requires considerable investment of resources devoted to HIV testing, linkage to care, ART accessibility, and retention in care. Ongoing studies of TasP will provide insight into achieving meaningful ART coverage.
