    Abstract Background: An increasingly large proportion of clinical trials is being conducted at non-traditional geographic regions such as Latin America. However, concerns have been raised that hosting countries may lack adequate research regulations and that clinical trials may not address local health needs. In this context, Chile has been hosting a relatively large proportion of clinical trials and has introduced new regulatory protections.
    Aim: To study trends and characteristics of clinical trials in Chile, including the effects of regulatory protections and whether clinical trials are aligned with the local burden of diseases.
    Material and methods: Data from clinical trials on pharmaceutical products registered over the last decade in Chile's Institute of Public Health was reviewed. Clinical trials were analyzed according to sponsorship, phase, disease studied, and whether distribution of trials according to diseases was aligned with the local burden of diseases measured in disability-adjusted life years.
    Results: Most of the 876 clinical trials analyzed were funded by external pharmaceutical companies and corresponded to late-phase trials. The most commonly studied disease groups were neoplasms, musculoskeletal disorders, other noncommunicable diseases, chronic respiratory diseases, diabetes and kidney diseases, neurological disorders, and circulatory diseases. The distribution of clinical trials was partially aligned with the distribution of major causes of disease burden. The introduction of new regulatory protections was followed by changes in the number of trials studying certain disease groups associated with a high burden.
    Conclusions: Clinical trials conducted in Chile over the last decade are largely funded by external pharmaceutical companies. Their distribution is partially aligned with local disease burden. The introduction of regulatory protections was followed by changes in the distribution of diseases studied.

    Abstract The current COVID-19 pandemic has the potential to overwhelm the capacity of hospitals and Intensive Care Units in Chile and Latin America. Thus local authorities have an ethical obligation to be prepared by implementing pertinent measures to prevent a situation of rationing of scarce healthcare resources, and by defining ethically acceptable and socially legitimate criteria for the allocation of these resources. This paper responds to recent ethical guidelines issued by a Chilean academic institution and discusses the main moral principles for the ethical foundations of criteria for rationing during the present crisis. It argues that under exceptional circumstances such as the current pandemic, the traditional patient-centered morality of medicine needs to be balanced with ethical principles formulated from a public health perspective, including the principles of social utility, social justice and equity, among others. The paper concludes with some recommendations regarding how to reach an agreement about rationing criteria and about their implementation in clinical practice.

    Abstract Hip subspine impingement (SSI) is a condition located in the extra-articular area of the hip and is characterized by irregular contact between the anterior inferior iliac spine (AIIS) and the distal femoral neck. Recently, it also has been described that SSI can occur in a normal AIIS, without or in combination with femoroacetabular impingement (FAI), which has redirected the focus of the research. In clinical practice, SSI has no characteristic symptoms/signs and shares similar clinical findings with FAI, making it an important cause of revision hip arthroscopy due to the increased risk of being underdiagnosed and untreated. Consequently, interest in studying SSI and FAI simultaneously has grown to determine whether injuries to the labrum, chondrolabral junction, and acetabular cartilage can be aggravated by dynamic dysfunction or abnormal morphology of the AIIS.
