    Abstract Die klinische Symptomatik und die Diagnostik der Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) werden dargestellt. Einleitend wird die ADHS-Kernsymptomatik anhand der DSM-5-Kriterien verdeutlicht. Des Weiteren werden weitere ADHS-typische Symptome sowie psychosoziale Folgen der Erkrankung aufgezeigt. Außerdem wird auch auf besondere Fähigkeiten und Eigenschaften bei der ADHS (etwa Spontaneität, Kreativität, Impulsivität) eingegangen. Anschließend wird die Diagnostik der Störung behandelt. Hierzu werden Fragenkataloge für die Anamneseerhebung sowohl bei Kindern und Jugendlichen als auch bei Erwachsenen vorgelegt sowie verschiedene Fragebögen skizziert. Außerdem werden klinische Beobachtungen sowie neuropsychologische Testdiagnostik berücksichtigt. Im Folgenden werden Differenzialdiagnose und komorbide Störungen (affektive Störungen, Angst- und Zwangsstörungen, Persönlichkeitsstörungen, psychotische Störungen, posttraumatische Belastungsstörung, Essstörungen, Tic-Störung und Tourette-Syndrom sowie somatische Differenzialdiagnosen) betrachtet.

    Abstract Ätiopathogenese und Neurobiologie der Aufmerksamkeitsdefizit-/Hyperaktivitätsstörung (ADHS) werden beschrieben. Nach einleitenden Anmerkungen zu verschiedenen Einflussfaktoren und Teilaspekten der ADHS werden neurochemische und neuroanatomische Aspekte der ADHS erläutert. Hierzu werden zunächst die Grundlagen (Aufbau des Gehirns, Neurotransmission) verdeutlicht und anschließend die pathologischen Veränderungen bei ADHS dargelegt. Dabei wird festgehalten, dass die Neurotransmitter Dopamin und Noradrenalin wesentlich in die Pathologie der ADHS involviert sind, ebenso der präfrontale Cortex und das Striatum. Des Weiteren werden genetische Auffälligkeiten bei der ADHS erörtert und hierbei auf folgende Punkte eingegangen: (1) Grundlagen, (2) Genetik des Dopamin- und Noradrenalinstoffwechsel, (3) andere Neurotransmittersysteme, (4) weitere "Kandidatengene" sowie (5) Epigenetik.

    Abstract Background: The negative effect of autism spectrum disorders (ASD) persists into adulthood, with impacts on social interactions and occupational development. This article reviews the current status of clinical aspects of ASD in adulthood on the basis of ICD-11.
    Methods: A selective search of PubMed and other relevant publications on ASD focused on changes in ICD-11 and on prevalence, symptoms, diagnosis, treatment, and prognosis.
    Results: The global lifetime prevalence of ASD is around 1%. A number of recent studies have concentrated on behaviors termed "camouflaging" (disguising symptoms typical of autism) and "stimming" (use of repetitive self-stimulation), which affect the mental health of those concerned. Standardized diagnostic instruments are available, but the data on validity in adulthood and on applicability are limited. Gestures, eye contact, facial expressions, social closeness, and reciprocity play particularly important roles in diagnosis. German-language treatment manuals lack randomized controlled studies: a validated treatment strategy is currently available only in English. Investigation of the prognosis showed limitation of occupational perspectives (54% unemployment in the study sample) and 2.9 times higher mortality compared to the general population.
    Conclusion: Individual treatment goals should be set, taking account of any coexisting psychiatric disorders. The process of diagnosis remains clinical, using standardized instruments. Further improvement of diagnostic and therapeutic instruments for adult ASD is desirable.

    Abstract Episodic memories are encoded by experience-activated neuronal ensembles that remain necessary and sufficient for recall. However, the temporal evolution of memory engrams after initial encoding is unclear. In this study, we employed computational and experimental approaches to examine how the neural composition and selectivity of engrams change with memory consolidation. Our spiking neural network model yielded testable predictions: memories transition from unselective to selective as neurons drop out of and drop into engrams; inhibitory activity during recall is essential for memory selectivity; and inhibitory synaptic plasticity during memory consolidation is critical for engrams to become selective. Using activity-dependent labeling, longitudinal calcium imaging and a combination of optogenetic and chemogenetic manipulations in mouse dentate gyrus, we conducted contextual fear conditioning experiments that supported our model's predictions. Our results reveal that memory engrams are dynamic and that changes in engram composition mediated by inhibitory plasticity are crucial for the emergence of memory selectivity.

    Abstract Background: The incidence of serious complications during vaginal delivery with a passive second stage in women with medical conditions is unknown.
    Methods: Our retrospective cohort study with matched groups (pairing 1 passive with 2 active second stage) included women who had a medical delivery plan from the high risk obstetric team at our center. The primary outcome was a composite of major maternal and neonatal complications.
    Results: The primary outcome occurred in 50% (12/24) of women in the passive group versus 35.4% (17/48) (
    Conclusion: The higher proportion of complications in women who had a passive second stage should encourage physicians to make this recommendation only in selected cases.
