    Title translation Social, demographic and morbimortality characteristics of the cases treated for COVID-19 at the Ignacio Chávez National Institute of Cardiology. A descriptive cross-sectional study.
    Abstract Introduction: The COVID-19 pandemic brought with it a large number of adverse consequences for public health with serious socioeconomic repercussions. In this study we characterize the social, demographic, morbidity and mortality conditions of individuals treated for COVID-19 in one of the SARS-CoV-2 reference hospitals in Mexico City.
    Method: A descriptive cross-sectional study was carried out in 259 patients discharged from the Instituto Nacional de Cardiología Ignacio Chávez, between April 11, 2020 and March 14, 2021. A multivariate logistic regression model was used to identify the association between sociodemographic and clinical variables. An optimization was performed using maximum likelihood calculations to choose the best model compatible with the data. The maximum likelihood model was evaluated using ROC curves, goodnessof-fit estimators, and multicollinearity analysis. Statistically significant patterns of comorbidities were inferred by evaluating a hypergeometric test over the frequencies of co-occurrence of pairs of conditions. A network analysis was implemented to determine connectivity patterns based on degree centrality, between comorbidities and outcome variables.
    Results: The main social disadvantages of the studied population are related to the lack of social security (96.5%) and the lag in housing conditions (81%). Variables associated with the probability of survival were being younger (p < 0.0001), having more durable material goods (p = 0.0034) and avoiding: pneumonia (p = 0.0072), septic shock (p < 0.0001) and acute respiratory failure (p < 0.0001); (AUROC: 91.5%). The comorbidity network for survival cases has a high degree of connectivity between conditions such as cardiac arrhythmias and essential arterial hypertension (Degree Centrality = 90 and 78, respectively).
    Conclusions: Given that among the factors associated with survival to COVID-19 there are clinical, sociodemographic and social determinants of health variables, in addition to age; It is imperative to consider the various factors that may affect or modify the health status of a population, especially when addressing emerging epidemic phenomena such as the current COVID-19 pandemic.

    Abstract Objectives: Congenital vascular malformations of the major arteries in the chest have been classified into 5 groups: 1) double aortic arch; 2) right aortic arch with left ligament or persistent ductus arteriosus; 3) aberrant subclavian artery; 4) aberrant left pulmonary artery, and 5) anomalous innominate artery. We reviewed the patients with aberrant right subclavian artery and their treatment.
    Methods: We studied retrospectively the records of 29 patients with aberrant right subclavian artery in childhood, from January 1992 to December 2012, analyzing the following variables: age at onset, clinical manifestations, associated cardiovascular defects, diagnosis and surgical approach method.
    Results: We found that most patients have an asymptomatic course, only 31% of them course with symptoms during the first year of life, with an incidental diagnosis of 35% during catheterization or other imaging studies. Patent ductus arteriosus was the most frequently associated congenital malformation, with 13%. Down's syndrome was found in 21%. The most common treatment was surgical section of the aberrant subclavian artery to release the esophagus.
    Conclusions: This vascular abnormality must be suspected in those patients with dysphagia, dyspnea, chest pain during feeding or breathing difficulties. A significant number of patients are not diagnosed in time, some reach adulthood without a diagnosis. This malformation is often found in imaging studies when evaluating the aorta or in a gastroesophageal reflux study, in which the barium bolus reveals the extrinsic compression of the esophagus.

    Abstract Introducción: Las enfermedades cutáneas son frecuentes en los pacientes trasplantados renales, por lo cual el monitoreo de la piel es necesario para realizar un diagnóstico precoz e iniciar tratamiento temprano. Objetivo: Determinar la prevalencia de las manifestaciones cutáneas en pacientes con trasplante renal. Materiales y métodos: Se evaluaron en consulta de dermatología 86 pacientes del programa de trasplante renal del Hospital Universitario San Ignacio. Resultados: Las infecciones de piel fueron las manifestaciones más prevalentes, y dentro de estas las más frecuentes fueron las micosis superficiales presentes, en un 48 % de los pacientes; seguidas por las infecciones virales, en un 20 %, y las bacterianas, en un 15 %. Se encontraron dos casos de cáncer de piel: un carcinoma basocelular y un sarcoma de Kaposi. Doce pacientes tuvieron queratosis actínicas consideradas lesiones premalignas. Conclusión: Los resultados del estudio evidencian la necesidad de realizar un examen dermatológico rutinario en los pacientes trasplantados para detectar tempranamente la presencia de lesiones cutáneas y, de esta manera, iniciar un manejo oportuno. Los hallazgos más comunes fueron las micosis superficiales.

    Title translation Synthetic membrane coronary stent graft in coronary disease.
    Abstract The use of a synthetic membrane-covered stent has been suggested to overcome some specific lesions in coronary arteries and to solve some potential complications that could arise in any procedure of coronary intervention. A synthetic membrane coronary stent graft (JOSTENT-JOMED) is constituted by a membrane of polytetrafluoroethylene (PTFE), fixed between two thin metallic stents. In this clinical review of our laboratory's experience, the Jostent graft was used in several angiographic circumstances in coronary arteries or in vein grafts. Our clinical experience consists of 17 patients that were treated with placement of PTFE stents for different indications: coronary rupture in two patients; thrombus and degenerative vein grafts in eight patients, coronary aneurysms in four patients, and thrombus in coronary arteries in three more. Our clinical results with this device suggest that its implant is feasible and safe with a clinical success of 100% without complications during in-hospital stay; during the clinical 11 +/- 3 months follow-up, only three patients exhibited major cardiac events. The indication for PTFE stent in vein grafts with degenerative and thrombus lesions has not been determined yet; however, recently, some randomized trials have shown little benefit in this circumstance. On the other side, coronary vessel rupture and coronary aneurysms are currently the most definitive indications.

    Abstract Our objective was to analyze the information in Spanish on YouTube about the influenza vaccine. In August 2020, a search was conducted on YouTube using the terms "Vacuna gripe", "Vacuna influenza", and "Vacuna gripa". Associations between the type of authorship, country of publication, and other variables (such as tone, hoaxes, and vaccination recommendations) were studied via univariate analysis. A total of 100 videos were evaluated; 57.0% were created in Mexico (24.0%), Argentina (17.0%), and Spain (16.0%), and 74.0% were produced by mass media or health professionals. Positive messages were detected in 65.0%. The main topics were the benefits of the vaccine (59.0%) and adverse effects (39.0%). Hoaxes were detected in 19 videos. User-generated content, compared to that of health professionals, showed a higher probability of hoaxes (odds ratio (OR) = 15.56), a lower positive tone (OR = 0.04), and less evidence of recommendations to vaccinate pregnant individuals (OR = 0.09) and people aged 60/65 or older. Videos published in Spain, in comparison with those from Hispanic America, presented significant differences in the positive tone of their messages (OR = 0.19) and in the evidence of the benefits of vaccination (OR = 0.32). A higher probability of hoaxes was detected in videos from Spain and the USA. Information in Spanish about the influenza vaccine on YouTube is usually not very complete. Spanish health professionals are urged to produce pro-vaccination videos that counteract hoaxes, and users in Hispanic America should be advised to consult videos produced in Hispanic American countries by health professionals to obtain reliable information.

    Abstract Pneumococcal vaccination is recommended for chronic kidney disease (CKD) patients. We evaluated the impact of implementing a hospital vaccine consultation on their vaccination coverages. A sample was drawn from consecutive cases referred to our clinic between November 2014 and June 2018. 101 patients were analyzed: 4 had received the pneumococcal conjugated vaccine 13-valent at the time of the first appointment, whereas 93 had received it after being seen (RR (95%-CI) = 23.3 (8.88-60.85)). Moreover, 5 had received the pneumococcal polysaccharide vaccine 23-valent at the time of the first appointment, whereas 90 had received it after being seen (RR (95%-CI) = 18.0 (7.64-42.42)). Implementing a hospital-based vaccine consultation represents an effective intervention to improve pneumococcal vaccination coverage in these patients.
