    Abstract Objective: To evaluate the subjective sleep pattern in hospitalized patients.
    Methods: This is a cross-sectional design developed with 230 patients in a university hospital in northeastern Brazil from September 2017 to March 2018. We included patients 18 years old or older, hospitalized for a minimum of 48 hours and a maximum of five days, with stable clinical conditions and preserved guidance. For data collection, a structured questionnaire was used and the subjective sleep pattern was assessed using Visual Analog Sleep Scales. Univariate and bivariate statistics were calculated using IBM® SPSS® software, version 23.0.
    Results: In the hospitalization, the Disorder, Effectiveness, and Supplementation scales reached scores of 208.7 points, 353.8 points, and 62.9 points, respectively. The clinical characteristics that interfered with sleep were the practice of regular physical activity (p = 0.024) and a higher body mass index (p = 0.033). We also observed statistically significant differences between VAS scores and factors influencing sleep.
    Conclusion: There was a certain level of sleep disturbance during the hospitalization period, and consequently the need for sleep supplementation during the day. We also observed that some factors negatively influenced the quality of hospital sleep.

    Abstract Background & aims: Lipid accumulation product (LAP) is emergent predictor of central lipid accumulation linked to diabetes risk and cardiovascular disease. In this study, our aims were (i) to assess the accuracy of the LAP for predicting metabolic syndrome (MS) in subjects without diagnosis of cardiovascular disease (CVD) and type 2 diabetes mellitus compared with other classical anthropometric parameters and (ii) to estimate the optimal LAP cut-off point to predict MS in this population.
    Subjects/methods: We conducted a cross-sectional study with representative undiagnosed subjects aged 20-79 years (n = 201; 37.8% men), selected by simple random sampling. In this study, subjects without previous diagnosis of CVD and type 2 diabetes mellitus, and those who did not make use of continuous medication were included. The independent variables were body mass index (BMI), waist circumference (WC), waist-to-height ratio (WHtR) and waist-to-hip ratio (WHR) and LAP. MS was defined by American Heart Association and the National Heart, Lung, and Blood Institute (AHA/NHLBI); International Diabetes Federation (IDF); and a harmonized criteria between AHA/NHLBI and IDF.
    Results: The prevalence of MS was 78.9% and 69.6% for men and women, respectively. LAP showed better area under the curve (AUC) for MS in three different criteria than those indexes based on body mass index, waist circumference, waist-to-height ratio and waist-to-hip ratio, even after adjusting for age and sex. In the harmonized criteria, the cut-off point of 34.2 cm.mmol/L for LAP showed the highest accuracy for MS (sensitivity 0.90, specificity 0.61, positive likelihood ratio of 2.31 and negative likelihood ratio of 0.17).
    Conclusions: LAP is a simple and accurate predictor tool for MS in undiagnosed adults. Moreover, it has significantly higher predictability than other screening tools commonly used to find subjects at risk of CVD and type 2 diabetes mellitus development, with the best performance at the 34.2 cm.mmol/L cut-off point.

    Abstract Objetivo: verificar a influência do comprometimento cognitivo na adesão ao tratamento anti-hipertensivo em idosos. Método: revisão sistemática, com a inclusão de artigos originais, disponíveis na integra, nos idiomas português, inglês e espanhol, publicados entre os anos de 2007 e 2021. A busca foi realizada nas bases SCIELO, PUBMED, LILACS e MEDLINE, com os descritores: “Cognitive Dysfunction”; “Medication Adherence”; “Hypertension”; “Aged”. Resultados: dos 216 estudos encontrados, cinco foram selecionados. Quatro mostraram relação negativa entre o comprometimento cognitivo e a adesão dos idosos ao tratamento anti-hipertensivo. Evidenciou-se que idosos com déficit cognitivo tinham maior probabilidade de inadequado controle da pressão arterial. Conclusão: observa-se que o comprometimento cognitivo é um importante fator de risco para baixa adesão ao tratamento em idosos hipertensos. Torna-se imprescindível a detecção precoce, visando adequar a assistência às condições cognitivas do idoso e otimizar o gerenciamento da medicação, para atingir o controle adequado dos níveis pressóricos.

    Abstract Objective: To estimate the absolute risk of the public transportation drivers in the city of Teresina, Piauí, to develop coronary heart disease over the course of ten years based on the Framingham risk score.
    Methods: An observational, descriptive, cross-sectional study using the Framingham score was conducted with 107 public transportation drivers in the city of Teresina, Piauí, to assess level of risk and its association with the Framingham predictors that include: age, total cholesterol, HDL cholesterol, systolic blood pressure, diastolic blood pressure, diabetes mellitus and smoking. The significance test used was Chi2. The prevalence rate was used as the association measurement.
    Results: Mean risk was 5% and the greatest majority of the participants were situated in the low risk category (85.05%). Mean values for the variables were: 42 years of age, total cholesterol 200 mg%, HDL cholesterol 49 mg%, systolic blood pressure 130 mmHg and diastolic blood pressure 85 mmHg. The associations between diabetes mellitus, smoking and HDL cholesterol with level of risk were not statistically significant; however other variables had a great influence on the level of risk obtained.
    Conclusion: The mean absolute risk estimate for the public transportation drivers in the city of Teresina to develop coronary heart disease over the next ten years based on the Framingham score is low. The majority of the participants in the study (85.05%) were situated in the low risk category, that is, with a relative risk less than or equal to 10%.

    Abstract OBJETIVO: Estimar o risco absoluto de contração de doença arterial coronariana, nos próximos 10 anos, em motoristas de transportes coletivos urbanos de Teresina, Piauí, segundo o escore de risco de Framingham. MÉTODOS: Foi realizado um estudo observacional, descritivo, transversal, aplicando-se o escore de Framingham, em 107 motoristas de transportes coletivos urbanos de Teresina, Piauí, para avaliação do grau de risco e sua associação com as variáveis previstas no mesmo, que foram: idade, colesterol total, colesterol HDL, pressão arterial sistólica, pressão arterial diastólica, presença de diabete melito e tabagismo. O teste de significância usado foi o c². Utilizou-se a razão de prevalência como medida de associação. RESULTADOS: O risco médio foi de 5%, com a maior parte situando-se na categoria de baixo risco (85,05%). As médias obtidas foram: 42 anos para a idade, colesterol total 200 mg%, colesterol HDL 49 mg%, pressão arterial sistólica 130 mmHg e pressão arterial diastólica 85 mmHg. As associações diabete melito, tabagismo e colesterol HDL, com o risco, não foram estatisticamente significantes, diferente do ocorrido com as outras variáveis, que tiveram grande influência no risco obtido. CONCLUSÃO: O risco absoluto médio estimado para os próximos 10 anos de doença arterial coronariana, em motoristas de transportes coletivos urbanos de Teresina, calculado pelo escore de Framingham, apresentou-se baixo. Uma parte considerável dos participantes da pesquisa (85,05%) situou-se na categoria de baixo risco, ou seja, igual ou inferior a 10%.     Abstract Atherosclerotic coronary heart disease is the leading cause of morbidity and mortality in industrialized countries, and endothelial dysfunction is considered a precursor phenomenon. The nitric oxide produced by the endothelium under the action of endothelial nitric oxide synthase has important antiatherogenic functions. Its reduced bioavailabilty is the beginning of the atherosclerotic process. The addition of two methyl radicals to arginine, through the action of methyltransferase nuclear proteins, produces asymmetric dimethylarginine, which competes with L-arginine and promotes a reduction in nitric oxide formation in the vascular wall. The asymmetric dimethylarginine, which is itself considered a mediator of the vascular effects of the several risk factors for atherosclerosis, can be eliminated by renal excretion or by the enzymatic action of the dimethylarginine dimethylaminohydrolases. Several basic science and clinical research studies suggest that the increase in asymmetric dimethylarginine occurs in the context of chronic renal insufficiency, dyslipidemia, high blood pressure, diabetes mellitus, and hyperhomocysteinemy, as well as with other conditions. Therapeutic measures to combat atherosclerosis may reverse these asymmetric dimethylarginine effects or at least reduce the concentration of this chemical in the blood. Such an effect can be achieved with competitor molecules or by increasing the expression or activity of its degradation enzyme. Studies are in development to establish the true role of asymmetric dimethylarginine as a marker and mediator of atherosclerosis, with possible therapeutic applications. The main aspects of the formation and degradation of asymmetric dimethylarginine and its implication in the atherogenic process will be addressed in this article.

    Abstract Objetivo: Analisar a contribuição dos grupos de convivência e políticas públicas na promoção do envelhecimento saudável. Métodos: Trata-se de uma revisão de literatura realizada por meio de um levantamento bibliográfico, a partir de bancos e bases de dados disponíveis na internet, a exemplo da Biblioteca Virtual em Saúde, Scientific Electronic Library Online e Literatura Latino-Americana e do Caribe em Ciências da Saúde, além de documentos oficiais, de âmbito nacional, que abordem o envelhecimento saudável a partir das políticas públicas e da socialização do idoso em grupos de convivência. Resultados: Inicialmente, foram encontrados 27 artigos; destes, 9 contemplaram os critérios de inclusão e exclusão supracitados. Com relação aos documentos oficiais, foram encontrados 12; destes, 10 se relacionavam com a proposta principal do estudo. Os resultados encontrados apontaram que os idosos participantes dos grupos de convivência e de atividade física são constituídos, em sua maioria, por idosos do sexo feminino e aposentados; além disso, a maioria apresenta menos sintomas depressivos, maior satisfação com a vida e melhor condição física. Conclusão: Os grupos de convivência e de atividade física são reconhecidos como importantes espaços de interação social e socialização das emoções. Observou-se que diversas políticas públicas direcionadas à promoção do envelhecimento saudável são adotadas na realidade brasileira; com isso, foi possível notar a efetivação, nesses grupos, de medidas propostas nos documentos oficiais.
