    Abstract ABSTRACT Estimation of soil loss susceptibility is of great importance for the management of watersheds. Thus, several models for soil loss prediction have been proposed. This study estimated the total annual soil loss for the Santa Rita watershed, located in southern Brazil, using the Revised Universal Soil Loss Equation. In addition, a classification to soil loss index (ISL) was proposed to identify regions with critical soil loss values. Altitude, slope, land use, and soil class data were applied to the model, in addition to spatial information for 78 soil samples collected within the study area. It was found that there is an average annual loss of 35.94 Mg ha-1 year-1, with the most substantial loss occurring in areas with predominantly exposed soil and annual cropping. Furthermore, the ISL values indicated that approximately 50% of the study area is experiencing erosion estimates above the tolerable limit. Results emphasize the need for changes in conservation and application practices present in the watershed, considering land use and soil bearing capacity.

    Abstract Embora o conhecimento sobre a florística dos fragmentos de florestas estacionais semideciduais tenha crescido nos últimos anos, ainda sabe-se pouco sobre a comunidade de lianas (lenhosas ou herbáceas) nesses fragmentos. Assim, foi realizado o levantamento florístico de lianas na gleba Maravilha, pertencente ao Parque Estadual de Vassununga (Santa Rita do Passa Quatro, SP), a fim de colaborar com o conhecimento dessa comunidade e subsidiar futuros trabalhos que envolvam essa forma de vida. A área de estudo compreende 127,08 ha, com inverno seco e temperatura média anual de 22 ºC. Para a coleta do material, percorreu-se mensalmente toda a borda do fragmento e três trilhas no interior da mata, de agosto/2002 a setembro/2003. Foram identificadas 120 espécies de lianas, pertencentes a 30 famílias e 71 gêneros, das quais 51% das espécies são volúveis, 42% apresentam gavinhas e apenas 7% são escandentes. As famílias mais representativas em número de espécies foram: Bignoniaceae (26), Malpighiaceae (14), Sapindaceae (12) e Asteraceae (9). Houve baixa similaridade florística entre as espécies de lianas presentes na gleba Maravilha em relação a outras áreas de florestas estacionais semideciduais do interior paulista.

Although the knowledge about the floristic composition of the fragments of seasonal semidecidual forest had grown in the last few years, little is known about the liana communities (woody vines and herbaceous vines) in those fragments. To collaborate with the knowledgement of the lianas and subsidize future works involving this life form, a floristic survey of the liana species occurring at the fragment Maravilha of the State Park of Vassununga (Santa Rita do Passa Quatro - SP) was carried out. The study area comprised 127.08ha, with average temperature of 22 ºC. The whole border of the forest fragment and three tracks inside the forest were surveyed monthly between August 2002 and September 2003. One hundred and twenty species of lianas, belonging to 30 families and 71 genera were identified where 51% of them were twiners, 42% tendrilous and 7% scandents. The most representative families in number of species were: Bignoniaceae (26), Malpighiaceae (14), Sapindaceae (12) and Asteraceae (9). Fragment Maravilha given low similarity with other fragments of semidecidual forest in São Paulo State.

    Abstract Background: Post-traumatic vertebral necrosis and pseudarthrosis represents one of the most concerning and unpredictable challenges in spinal traumatology. The evolution of this disease at the thoracolumbar transition usually courses with progressive bone resorption and necrosis, leading to vertebral collapse, retropulsion of the posterior wall and neurological injury. As such, the therapeutic goal is the interruption of this cascade, seeking to stabilize the vertebral body and avoid the negative consequences of its collapse.
    Case description: We present a clinical case of a pseudarthrosis of T12 vertebral body with severe posterior wall collapse, treated with removal of intravertebral pseudarthrosis focus by transpedicular access, T12 armed kyphoplasty with VBS® stents filled with cancellous bone autograft, laminectomy and stabilization with T10-T11-L1-L2 pedicle screws. We present clinical and imaging detailed results at 2-year follow-up and discuss our option for this biological minimally invasive treatment for vertebral pseudarthrosis that mimics the general principles of atrophic pseudarthrosis therapeutic and allows to perform an internal replacement of the necrotic vertebral body, avoiding the aggression of a total corpectomy.
    Conclusions: This clinical case demonstrates a successful outcome of the surgical treatment of pseudarthrosis of vertebral body (mobile nonunion vertebral body) in which expandable intravertebral stents allow to perform an internal replacement of the necrotic vertebral body by creating intrasomatic cavities and filling them with bone graft, obtaining a totally bony vertebra with a metallic endoskeleton, which is biomechanically and physiologically more similar to the original one. This biological internal replacement of the necrotic vertebral body technique can be a safe and effective alternative over cementoplasty procedures or total vertebral body corpectomy and replacement for vertebral pseudarthrosis and may have several advantages over them, however long-term prospective studies are needed in order to prove the effectiveness and advantages of this surgical option in this rare and difficult pathological entity.

    Abstract Thymomas and thymic carcinomas are rare mediastinal neoplasms arising from thymic epithelial cells, and the presence of synchronous or metachronous primary thymic neoplasms in a single patient is an extremely rare event. Thymoma patients appear to have an inherent predisposition toward developing additional neoplasms. This additionally presents a diagnostic challenge, revealing the importance of multidisciplinary expertise to the management of these patients. This is a case report of a patient with a thymoma and thymic carcinoma, submitted to surgical resection and postoperative radiotherapy.

    Abstract Juvenile idiopathic arthritis (JIA) is a term that collectively refers to a group of chronic childhood arthritides, which together constitute the most common rheumatic condition in children. The International League of Associations for Rheumatology (ILAR) criteria define seven categories of JIA: oligoarticular, polyarticular rheumatoid factor (RF) negative (RF-), polyarticular RF positive (RF+), systemic, enthesitis-related arthritis, psoriatic arthritis, and undifferentiated arthritis. The ILAR classification includes persistent and extended oligoarthritis as subcategories of oligoarticular JIA, but not as distinct categories. JIA is characterized by a chronic inflammatory process affecting the synovia that begins before the age of 16 and persists at least 6 weeks. If not treated, JIA can cause significant disability and loss of quality of life. Treatment of JIA is adjusted according to the severity of the disease as combinations of non-steroidal anti-inflammatory drugs (NSAIDs), synthetic and/ or biological disease modifying anti-rheumatic drugs (DMARDs). Although the disease etiology is unknown, disturbances in innate and adaptive immune responses have been implicated in JIA development. B cells may have important roles in JIA pathogenesis through autoantibody production, antigen presentation, cytokine release and/ or T cell activation. The study of B cells has not been extensively explored in JIA, but evidence from the literature suggests that B cells might have indeed a relevant role in JIA pathophysiology. The detection of autoantibodies such as antinuclear antibodies (ANA), RF and anti-citrullinated protein antibodies (ACPA) in JIA patients supports a breakdown in B cell tolerance. Furthermore, alterations in B cell subpopulations have been documented in peripheral blood and synovial fluid from JIA patients. In fact, altered B cell homeostasis, B cell differentiation and B cell hyperactivity have been described in JIA. Of note, B cell depletion therapy with rituximab has been shown to be an effective and well-tolerated treatment in children with JIA, which further supports B cell intervention in disease development.

    Abstract Lymphangioleiomyomatosis (LAM) is a rare systemic disease that typically presents like cystic lung disease. High-resolution computed tomography (CT) is the recommended imaging technique, with cysts being the hallmark: typically multiple, well-circumscribed, thin-walled, with a variable diameter (usually <2 cm) and widespread in distribution. The gold standard for diagnosis is a biopsy. LAM should be considered in the differential diagnosis of cystic lung diseases. The authors report a case of LAM presenting with a pneumothorax, which due to its atypical imaging characteristics, mimicked another uncommon cystic disease. A multidisciplinary approach is crucial when dealing with presentations of rare diseases.

    Abstract Rheumatoid arthritis (RA) is a chronic, systemic immune-mediated inflammatory disease that can lead to joint destruction, functional disability and substantial comorbidity due to the involvement of multiple organs and systems. B cells have several important roles in RA pathogenesis, namely through autoantibody production, antigen presentation, T cell activation, cytokine release and ectopic lymphoid neogenesis. The success of B cell depletion therapy with rituximab, a monoclonal antibody directed against CD20 expressed by B cells, has further supported B cell intervention in RA development. Despite the efficacy of synthetic and biologic disease modifying anti-rheumatic drugs (DMARDs) in the treatment of RA, few patients reach sustained remission and refractory disease is a concern that needs critical evaluation and close monitoring. Janus kinase (JAK) inhibitors or JAKi are a new class of oral medications recently approved for the treatment of RA. JAK inhibitors suppress the activity of one or more of the JAK family of tyrosine kinases, thus interfering with the JAK-Signal Transducer and Activator of Transcription (STAT) signaling pathway. To date, there are five JAK inhibitors (tofacitinib, baricitinib, upadacitinib, peficitinib and filgotinib) approved in the USA, Europe and/ or Japan for RA treatment.
