Abstract Introduction: The vasculitis diagnosed specifically in the post-partum period are less well known. We report here such a case followed by a descriptive review of the literature.
Case report: A 25 year-old French nurse reported abrupt-onset musculoskeletal pain 15 days after delivery of her first infant. Her first pregnancy was uneventful. The physical examination yielded only bilateral conjunctivitis and purpuric eruption of lower limbs, and complementary investigations evidenced pulmonary renal syndrome in connection with the diagnosis of Granulomatosis with Polyangiitis.
Methods: We screened previous articles in Medline database using keywords (i) "post-partum" or "puerperium" (ii)"peripartum" (iii) "pregnancy" associated with "vasculitis". Full texts were obtained from case reports or cases series whose title or abstract included keywords of interest (or synonyms). These references were secondarily excluded if the diagnosis of vasculitis was not confirmed or made before or more than 6 months after delivery and if vasculitis occurred only in the new born or affected only the cerebral vasculature or the retina.
Results: Fifty-six cases of vasculitis diagnosed in the post-partum period were included, 50 (89.3%) were secondary to an immunological process and 44 corresponded to primary vasculitis, 4 were secondary to Systemic Lupus Erythematosus, 1 to cryoglobulinaemia and 1 to cryoglobulinaemia associated with inflammatory bowel disease. The main primary vasculitis diagnosed were Takayasu Arteritis (n = 10), Eosinophilic granulomatosis with polyangiitis (n = 9), Granulomatosis with Polyangiitis (n = 7), Periarteritis Nodosa (n = 5) and Behcet's disease (n = 4). The first symptom occurred before delivery in 26 (59.1%) and maternal death occurred in 4 (9.1%) out of the 44 primary vasculitis cases.
Conclusion: The vasculitis diagnosed in the post-partum period were mainly primary vasculitis among patients in whom the diagnosis had not been made during pregnancy. In less than half of cases and as described in ours, there were no reported symptoms before delivery. Therefore, the physicians should pay attention to symptoms and keep a high degree of suspicion for vasculitis before as well as after delivery.

Abstract In this study, we focused on the rime priming effect and examined whether the frequency of rimes influences the size of the effect. Using the lexical decision task, we observed that the rime priming effect interacted with rime frequency with a stronger priming effect when the target words contained low-frequency rimes, in comparison with target words containing high-frequency rimes. Moreover, for both target words with low- and high-frequency rimes, a priming effect of similar magnitude was observed whether the primes and targets were pronounced by the same speaker or by different speakers. All together, these findings provide additional support for all the studies that plead for the existence of prelexical units and, in a more general way, for phonological abstraction prior to lexical access.

Abstract In various medical fields, a change of soft tissue stiffness is associated with its physio-pathological evolution. While elastography is extensively employed to assess soft tissue stiffness in vivo, its application requires a complex and expensive technology. The aim of this study is to determine whether an easy-to-use method based on impact analysis can be employed to determine the concentration of agar-based soft tissue mimicking phantoms. Impact analysis was performed on soft tissue mimicking phantoms made of agar gel with a mass concentration ranging from 1% to 5%. An indicator Δt is derived from the temporal variation of the impact force signal between the hammer and a small beam in contact with the sample. The results show a non-linear decrease of Δt as a function of the agar concentration (and thus of the sample stiffness). The value of Δt provides an estimation of the agar concentration with an error of 0.11%. This sensitivity of the impact analysis based method to the agar concentration is of the same order of magnitude than results obtained with elastography techniques. This study opens new paths towards the development of impact analysis for a fast, easy and relatively inexpensive clinical evaluation of soft tissue elastic properties.

Abstract During herbivory, chewing insects deposit complex oral secretions (OS) onto the plant wound. Understanding how plants respond to the different cues of herbivory remains an active area of research. In this study, we used an herbivory-mimick experiment to investigate the early transcriptional response of rice plants leaves to wounding, OS, and OS microbiota from Spodoptera frugiperda larvae. Wounding induced a massive early response associated to hormones such as jasmonates. This response switched drastically upon OS treatment indicating the activation of OS specific pathways. When comparing native and dysbiotic OS treatments, we observed few gene regulation. This suggests that in addition to wounding the early response in rice is mainly driven by the insect compounds of the OS rather than microbial. However, microbiota affected genes encoding key phytohormone synthesis enzymes, suggesting an additional modulation of plant response by OS microbiota.

Abstract Heart failure remains, despite increasing therapeutic advances, a major burden in terms of public health. It is associated with a significant mortality and dramatically impacts the daily life of these patients with, among other things, repeated and prolonged hospitalizations. This article aims to focus on the therapeutic modalities for the management of patients with heart failure and reduced ejection fraction (HFrEF) recommended by the European Society of Cardiology. A significant change is taking place in pharmacological treatment following the discovery of new drug classes.

Abstract Lao PDR is undergoing a rapid agrarian transformation, underpinned by a growing commercial production of commodity crops such as cassava by smallholder farmers. The global nature of commodity crop markets increases the exposure of potentially vulnerable smallholder farmers to global price dynamics. We apply a cross-scalar and mixed-methods approach to investigate the role of institutional structures and the global-regional-domestic context in local price formation mechanisms at the farm gate. We focus our analysis on policy shocks created by the Chinese government’s temporary purchase and storage policy (TSPS) for maize, and the experience and price risk mitigation strategies of smallholder cassava farmers in Lao PDR during this period. We find that the price level and volatility experienced by farmers are shaped by local-level market structures as much as global price dynamics and regional trade integration, demonstrating the importance of institutions across scales in price formation and transmission.
