    Abstract Bartonellosis (Carrion's Disease) during pregnancy is associated with high rates of maternal and perinatal mortality. We report the immunological patterns in two cases of human bartonellosis during pregnancy. One patient had an uncomplicated course while the second patient developed life threatening anasarca and cardiac tamponade. The patient with a complicated course had a Th1 response with a higher elevation of IL-10. This elevation has been associated with poor outcome pregnancies during bacterial infections.

    Abstract Bartonellosis, or Carrion's Disease, is an endemic and reemerging disease in Peru and Ecuador. Carrion's Disease constitutes a health problem in Peru because its epidemiology has been changing, and it is affecting new areas between the highland and the jungle. During the latest outbreaks, and previously in endemic areas, the pediatric population has been the most commonly affected. In the pediatric population, the acute phase symptoms are fever, anorexia, malaise, nausea and/or vomiting. The main signs are pallor, hepatomegaly, lymphadenopathies, cardiac murmur, and jaundice. Arthralgias and weight loss have also commonly been described. The morbidity and mortality of the acute phase is variable, and it is due mainly to superimposed infections or associated respiratory, cardiovascular, neurological or gastrointestinal complications. The eruptive phase, also known as Peruvian Wart, is characterized by eruptive nodes (which commonly bleed) and arthralgias. The mortality of the eruptive phase is currently extremely low. The diagnosis is still based on blood culture and direct observation of the bacilli in a blood smear. In the chronic phase, the diagnosis is based on biopsy or serologic assays. There are nationally standardized treatments for the acute phase, which consist of ciprofloxacin, and alternatively chloramphenicol plus penicillin G. However, most of the treatments are based on evidence from reported cases. During the eruptive phase the recommended treatment is rifampin, and alternatively, azithromycin or erythromycin.

Bartonelosis (Enfermedad de Carrión) durante el embarazo esta asociado a una alta tasa de mortalidad maternal y perinatal. Reportamos el perfil inmunológico de dos casos de Bartonelosis humana en el embarazo. Una paciente tuvo un curso sin complicaciones, mientras la segunda presento complicaciones severas de anasarca y tamponamiento cardiaco. La paciente con curso complicado tuvo un patrón de repuesta Th1, con una elevación de IL-10, que se ha asociado a mal pronóstico en infecciones durante embarazo.

    Abstract Several epidemiological evidence have been correlated with the effect of environmental factors over infectious diseases. Since the El Niño phenomenon in 1973, this event has been incriminated in the onset or trigger of epidemic changes and outbreaks. El Niño, which is responsible for the Ecuatorial Pacific Ocean water warming, produce a global effect, and as a consequence atmospheric humidity and environmental temperature are higher than usual. Currently, there are evidence that those changes have a direct effect on the biological life cycle of infectious diseases vectors and some microorganisms with a correlation in the epidemiology of those diseases. Recently, the molecular biology and the mathematical model analysis have been improving our understood about biological explanations, allowing the opportunity to predict outbreaks, risky areas, or epidemiological changes. We review the current evidence that may affect infectious diseases, as Malaria, Leishmaniasis, Bartonellosis, Cholera and others. (Rev Med Hered 2004;15:218-224).

    Abstract Human Bartonellosis has an acute phase characterized by fever and hemolytic anemia, and a chronic phase with bacillary angiomatosis-like lesions. This cross-sectional pilot study evaluated the immunology patterns using pre- and post-treatment samples in patients with Human Bartonellosis. Patients between five and 60 years of age, from endemic areas in Peru, in the acute or chronic phases were included. In patients in the acute phase of Bartonellosis a state of immune peripheral tolerance should be established for persistence of the infection. Our findings were that elevation of the anti-inflammatory cytokine IL-10 and numeric abnormalities of CD4(+) and CD8(+) T-Lymphocyte counts correlated significantly with an unfavorable immune state. During the chronic phase, the elevated levels of IFN-γ and IL-4 observed in our series correlated with previous findings of endothelial invasion of B. henselae in animal models.

    Abstract Cat-Scratch Disease (CSD) is a benign lymphadenitis that may progress to severe or recurrent forms, and it is occasionally associated with morbidity. Between January of 1998 and March of 1999, forty-three suspected CSD patients were assessed in the Hospital Cayetano Heredia and the Instituto de Salud del Niño, in Lima, Peru. Twelve patients had a confirmed diagnosis, 8 of whom were women, and the mean age was 10 years old. The majority (53%) of the cases were encountered in the summer. All patients reported having had contact with cats. Fever, malaise, lymphadenopathy and skin lesions were the most frequent clinical features. Twelve patients had indirect immunofluorescence antibody test titers of between 1/50 and 1/800 for Bartonella henselae and Bartonella clarridgeiae. Two lymph node biopsies were histologically compatible with CSD. No positive blood cultures could be obtained. This is the first Peruvian prospective study able to identify B. henselae and B. clarridgeiae in pediatric patients.

    Abstract Objetivo: Determinar la influencia de factores climáticos y del fenómeno El Niño 1997 - 1998 en la epidemiología de la bartonelosis en Ancash y Cusco, Perú. Materiales y Métodos: Se desarrolló un estudio ecológico. Se obtuvo información mensual climática de Urubamba y Carhuaz del SENAMHI, así como reporte de casos mensuales confirmados de la OGE, entre 1996 y 1999. Resultados: En los departamentos de Ancash y Cusco los coeficientes de correlación para los parámetros ambientales locales (temperatura máxima, media y mínima mensual, y precipitación total) no fueron estadísticamente significativos, mientras que la Temperatura Superficial del Mar fue el mejor parámetro que correlacionó con la tasa de incidencia de la bartonelosis, tanto a nivel local como regional. La tasa de incidencia de bartonelosis fue estadísticamente superior durante El Niño 1997 - 1998 en Ancash y Cusco, de manera simultanea. Conclusiones: Los incrementos significativos en la tasa de incidencia de bartonelosis durante El Niño en departamentos tan distantes como Ancash y Cusco de manera simultanea, evidencian que los factores climatológicos y el fenómeno El Niño modifican la epidemiología de la Bartonelosis humana.
