    Abstract This work aimed to analyze the reliability of a steel hall that was recently erected in central Poland subjected to dynamic wind excitation using the stochastic finite element method. Reliability analysis was completed using the relative entropy concept delivered by Bhattacharyya and contrasted with the first-order reliability method recommended by the engineering design codes. Bhattacharyya probabilistic relative entropy was additionally rescaled in this study to fit the demands and recommended admissibility intervals given in Eurocode 0. The finite element method study was carried out thanks to a discrete model created in the system ABAQUS 2019, while all further statistical and probabilistic computations were programmed and completed in the symbolic environment of MAPLE 2019. Contrary to most engineering analyses in steel structure areas, this study included the important warping effect while designing the hall ridges and the purlins. Dynamic structural responses were determined via the Hilber-Hughes-Taylor algorithm and their series were numerically obtained for a series of input uncertainty parameters representing several mechanical and environmental quantities. The generalized 10th order iterative stochastic perturbation technique was contrasted in this context with statistical estimators from the Monte Carlo simulations and numerical integration resulting from the semi-analytical approach. The key research finding of this study was an extremely good coincidence between the FORM indices and the rescaled relative probabilistic entropies for the given stochastic excitations, which additionally did not depend on a choice of one of the three proposed numerical approaches.

    Abstract The COVID-19 pandemic disrupted cancer screening programs and care for individuals with malignancies. We aimed to analyze the effects of the COVID-19 pandemic on the interest of Google users in cancers and their screenings. We collected data from Google Trends (GT) from 1 January 2015 to 31 December 2021 worldwide for nine topics representing cancer screening and the HPV vaccine and for 33 topics representing malignancies. We performed a secular analysis comparing the prepandemic (2015-2019) and pandemic (2020-2021) period. We performed forecasting analysis on the prepandemic timeline to assess interest in the analyzed topics if the pandemic hadnot occurred.The actual interest in most of the analyzed topics was significantly lower than in the forecasted trend. Interest in 6 of the 9 topics representing cancer screening and 3 of the 33 topics representing cancer was higher during the pandemic than in the prepandemic period. The interest of Google users in cancer screenings increased in 2020-2021 compared to 2015-2019, but the growth was less dynamic than expected. The interest in many cancers during the pandemic was significantly lower than in the prepandemic period, especially during March and April 2020. The lower interest in cancers and their screenings may delay the diagnosis and worsen the long-term outcomes.

    Abstract Injuries to the articular cartilage of the human knee joint, commonly resulting from trauma, inflammation, or age- and activity-related wear and tear, have significant implications, primarily leading to osteoarthritis (OA). Conservative treatments for such injuries often yield suboptimal clinical outcomes. Surgical interventions using current methods may not consistently provide satisfactory results, largely due to the formation of low-quality scar tissue lacking the biomechanical properties of hyaline cartilage. In this retrospective study, we compared the results of two promising methods for regenerating cartilage defects in the knee joints using scaffolds soaked with stem cells of different origins: bone marrow aspirate concentrate mesenchymal stem cells (BMAC-MSCs) and human umbilical cord-derived mesenchymal stem cells (hUCB-MSCs). We evaluated 39 patients (39 knees, hUCB-MSCs: 20 knees, BMAC: 19 knees) at the 12-month follow-up using VAS, KOOS, Lysholm scales and radiologically with M-MOCART 2.0 score. The analysis demonstrated substantial overall improvement in both groups, notably reflected in enhanced quality of life for the patients. Interestingly, the final scores in the hUCB-MSCs group were comparable to those in the BMAC-MSCs group, with no statistically significant differences observed, despite variations in preoperative age and lesion size. Notably, the hUCB-MSCs group consisted of significantly older individuals with larger lesion sizes. Both procedures were found to be safe, and improvement was observed in both groups, which holds promise for future clinical investigations.

    Abstract Modern anti-seizure drug development yielded benefits in terms of improved pharmacokinetics, safety and tolerability profiles, but offered no advances in efficacy compared to previous older generations of anti-seizure drugs. Despite significant advances in our understanding of the genetic bases to epilepsy, and a welcome renewed interest on the severe monogenic epilepsies, modern genetics has yet to directly inform more effective or disease-modifying anti-seizure drugs. Here, we describe a new approach to the identification of novel disease modifying anti-epilepsy drugs. The systems genetics approach aims to first identify pathophysiological mechanisms by integrating polygenic risk with cellular gene expression profiles and then to relate these molecular mechanisms to druggable targets using a gene regulatory (regulome) framework. The approach offers an exciting and flexible framework for future drug discovery in epilepsy, and is applicable to any disease for which appropriate cell-type and disease-context specific data exist. This article is part of the special issue entitled 'New Epilepsy Therapies for the 21st Century - From Antiseizure Drugs to Prevention, Modification and Cure of Epilepsy'.

    Abstract Temporomandibular joint disorders are a heterogenic group of clinical conditions, which impair physiological functioning of the masticatory system. Arthrocentesis of the temporomandibular joint has become a widely approved method for non-invasive treatment, bridging the gap between conservative and surgical approaches. Regardless of technique, treatment is based upon joint lavage and lysis of the inflammatory fibrous tissue adhesions, which, in turn, improves joint mobility and reduces pain and closed lock. Recently, approaches for intra-articular injections have been proposed as adjuvant or replacement therapy. The aim of this study was to assess the most efficient technique of arthrocentesis. A systematic search based on PRISMA guidelines, including a computer search with specific keywords, a reference list search and a manual search, was performed. Relevant articles were selected after three search rounds for final review. The studies pulled for the analysis presented information about the relevant predictors, including the technique of arthrocentesis (single- or two-needle method), fluid used for lavage (Ringer lactate or saline), volume of the fluid, application of the injectable, number of interventions, pain (VAS) and mouth opening scores (MMO) and follow-up. All cohorts showed improvement in mouth opening, but significant pain reduction was observed only in cohorts treated either by arthrocentesis alone or arthrocentesis followed by intra-articular injectables. Intra-articular injectables used alone failed to reduce pain post-operatively when compared to other cohorts. We concluded that both double-needle and single-puncture arthrocentesis techniques are equally efficient. Application of the adjuvant injectable did not improve the outcomes of arthrocentesis performed alone. The volume of the fluid used for joint lavage and its chemical composition were not significant in clinical outcomes. However, due to the lack of homogeneity in the study settings, a meta-analysis could not be applied and a systematic review was conducted. We still, however, state that there is a knowledge gap in the current literature regarding the use of injectables alone, as well as a longitudinal follow-up, which provides information about treatment efficiency. More high-quality and randomized controlled trials are required to shed light on this subject.

    Abstract In this study, we present a conformational landscape of 5000 AlphaFold2 models of the Histidine kinases, Adenyl cyclases, Methyl-accepting proteins and Phosphatases (HAMP) domain, a short helical bundle that transduces signals from sensors to effectors in two-component signaling proteins such as sensory histidine kinases and chemoreceptors. The landscape reveals the conformational variability of the HAMP domain, including rotations, shifts, displacements, and tilts of helices, many combinations of which have not been observed in experimental structures. HAMP domains belonging to a single family tend to occupy a defined region of the landscape, even when their sequence similarity is low, suggesting that individual HAMP families have evolved to operate in a specific conformational range. The functional importance of this structural conservation is illustrated by poly-HAMP arrays, in which HAMP domains from families with opposite conformational preferences alternate, consistent with the rotational model of signal transduction. The only poly-HAMP arrays that violate this rule are predicted to be of recent evolutionary origin and structurally unstable. Finally, we identify a family of HAMP domains that are likely to be dynamic due to the presence of a conserved pi-helical bulge.
