    Abstract Introduction: Anal fissure is a tear in the lining of the anus and is a common problem throughout the world. It is one of the most common painful and benign conditions of the anal region, which leads to morbidity and decrease in quality of life. Aim: To prospectively observe the outcomes of management of acute anal fissure, both surgically and conservative modalities of treatment. Materials and Methods: This longitudinal study constituted patients clinically presenting with features of acute anal fissure such as pain, bleeding, constipation attending Out Patient Department (OPD) in College of Medicine and JNM Hospital, West Bengal, India, from January 2020 to June 2021. Patients were grouped according to modalities of treatment given (surgical/conservative), after that regular follow-up was done at regular intervals to find out outcome of treatment, with respect to relief of pain, bleeding, healing rate, fissure relapse and incontinence. Chi-square test was used to compare the proportions and independent t-test was used to compare means. The p-value of <0.05 was considered to be statistically significant. All the analysis was done in IBM Statistical Package for the Social Sciences (SPSS) version 22.0. Results: In the current study, 9 (8.8%) patients had anterior anal fissure and 93 (91.2%) patients had posterior anal fissure. Fifty one patients (50.0%) underwent medical management {(nifedipine 2% gel locally, diltiazem (2%) locally)} and 51 patients (50.0%) were treated with surgical management (lateral internal sphincterectomy). Significant difference was noted at 15th, and 30th and 45th days post-treatment when surgically managed patients were compared with medically managed patients p-value <0.05*. Four patients in the medically managed group, had bleeding during the entire period.     Abstract Context (Background): Cognitive dysfunctions as the unexplored complications in diabetics need to be systematically studied. Aims: To assess the correlation between type 2 diabetes mellitus (DM) and associated cognitive dysfunctions with varying age groups and duration. Settings and Design: A hospital-based cross-sectional study was conducted in a tertiary care teaching institute in eastern India. Materials and Methods: In this study the participants were allocated into five groups according to duration of diabetes, for example, less than 3 months, 3 months-5 years, 6-10 years, 11-20 years, and more than 20 years. Each of these groups was then classified into following six subgroups according to age: group A (20 years or less), group B (21-30 years), group C (31-40 years), group D (41-50 years), group E (51-60 years), and group F (more than 60 years). A predesigned, pretested, semi-structured questionnaire was used to find the cognitive functions based on the Global Deterioration Scale (GDS) in diabetics and age-matched nondiabetic participants. Statistical Analysis Used: Statistical analysis using Statistical Package for the Social Sciences (SPSS) version 11.0 to calculate proportions and significance test were done in this study. Results: There was noticeable attenuation of cognitive functions in diabetics in all age groups irrespective of duration of the morbidity. Further, a distinct pattern of incremental cognitive dysfunctions was also noticed as the age progressed in diabetics and in healthy controls. Conclusion: Diabetics are more at risk of cognitive dysfunctions than age-matched controls, which needs more elaborative studies.

    Abstract Objective: To study leprosy patients in tribal concentrated Bastar district of Chhattisgarh, India in terms of their knowledge, attitudes and the stigma they have experienced. Method: This cross-sectional study included 101 registered Leprosy patients, in a single leprosy treatment centre between April, 2012 - June, 2013. The data collection tool (a pre tested close-ended questionnaire) was based on leprosy related socio-demographic variables, knowled geregarding different kinds of problems and issues experienced by the patients/participants. The investigators collected the data in face to face interviews and house hold visit. Results: Overall the majority of the respondents (85.1%) were between 16 and 60 years of age. 74.3% were males, 80.2% were married, and 54.5% were literate. The majority (67.3%) articulated positive knowledge about transmission of the disease, 75.3% knew that numbness of hands is an early symptom, 88.12% that it is curable, 91.1% had untreated deformities. Experiences reported included disturbed marital relationships (90.1%) or social life (94.1%), loss of employment (54.5%), isolation with the sitation to talk to people (29.7%), family members not sharing food (94.1%); being forced to leave the family (54.45%). Health education interventions improved the knowledge of 91.1% of participants. Multi-Bacillary leprosy was higher in newly registered cases with higher disabilities in the hands and feet (60.4%), eyes (29.7%). 9.9% of WHO grade-2 disabilities were due to a delayed diagnosis. 67.5% of the patients/participants with some form of disability had experienced a delay in diagnosis up to 12 months. Conclusion: The study noted that the tribal people were affected by leprosy not only in terms of the physical problems, but also by the stigmatization that affects their social participation.     Abstract Diabetes has become one of the major global public health problems of this new millennium. There is dire need for truthful "Continuing Medical Education" for healthcare professional of all shades and spectrum to be aware of their historical role to provide necessary advices on Diabetes to their stakeholders at every intersection. Let us not place the fate of average Indians at the mercy of health care providers who are too busy to get time to be updated regarding preventive measures to reduce prevalence and downstream complications of this "silent" killer. The unremitting progressions of the concepts of diet and nutrition issues amid lifestyle management need to be internalized and advocated in the light of holistic approaches to the management to the diabetics on one hand and caregivers as well as nondiabetics on the other hand. The authors wish to emphasize on the importance of philosophy of the voluntary and perpetual updating of knowledge, on diet and appropriate lifestyle, of the health care providers of all shades and all levels for our mission and vision of health promotion with the example of diabetes at this juncture.

    Abstract A 40-year-old man presented with a progressively increasing swelling over the ventral aspect of the right wrist of 23 days duration following a laceration. The swelling was soft, pulsatile and was intermittently bleeding, with poor distal capillary filling and cold distal extremity. Clinically it was diagnosed as pseudoaneurysm of the radial artery and was sent for a duplex scan for confirmation. The lesion ruptured in the radiology department before the scan was done, and the patient went into hypovolaemia. In view of this, the patient underwent exploration of the pseudoaneurysm under brachial block, and the radial artery was found to be transected just proximal to the dorsal branch. It was decided to repair the artery as there was no evidence of adequate circulation supply or the patency of the ulnar artery preoperatively. Doppler study at 15 and 45 days postoperatively showed good flow through, with no evidence of thrombus.

    Abstract Background: The contemplation for the salvage operations and the nonoperative treatment for the pediatric splenic injuries had increasingly been suggested as the standard case management.
    Objectives: The study was carried out to identify the risk factors, the presentations, the severities and outcome of the interventions of blunt splenic injuries in the children and adolescents.
    Materials and methods: This retrospective review was carried out in a tertiary care hospital in Sikkim on the children and adolescents admitted with splenic injury from January 2005 to December 2009. Splenic injuries were graded with the American Association for the Surgery of Trauma Splenic Injury Scale followed by the operative and nonoperative managements (NOM).
    Results: Overall 147 cases with the abdominal trauma were diagnosed with splenic injury. Of them, males reported in higher numbers; three-fourths were adolescents with preponderance above 16 years of age. Majority of the cases [n=91(61.90%)] were due to fall from heights and others from road traffic accidents. Immediate surgical interventions was instituted in the hemodynamically unstable cases (n=87) NOM failed in 27 patients; of them eight cases underwent splenectomy, and 19 underwent surgical salvage; 33 were closely followed up by conservative approach with both clinical and CT criteria. Total number of cases in grade III and above was significantly higher than with lower grades of injury.
    Conclusions: In total 95(64.63%) of the cases were managed with total splenectomy; 19 cases in the initial nonsurgical group underwent salvage operation and 33 cases received NOM.

    Abstract Background: The pbreakdown of phospholipids lead to accumulation of malondialdehyde (MDA) that is preferred as a surrogate marker of oxidative stress in diabetics.
    Objective: To compare serum MDA levels along with other biochemical parameters between type 2 diabetic patients with and without complications.
    Materials and methods: This cross-sectional study was carried out in the Department of Biochemistry of a tertiary care teaching hospital in Sikkim on 60 type 2 diabetics and compared with 100 non-diabetic participants. The un-hemolyzed blood samples were used for estimation of biochemical parameters; MDA was estimated in serum by using trichloroacetic acid (TCA) and 1% thiobarbituric acid (TBA). Whole blood was used for estimation of glycated hemoglobin. The comparison of different parameters between cases and control was calculated by using Student's t test.
    Results: In the study and control groups, no significant difference was noted with regard to independent variables. The fasting and postprandial serum glucose along with glycated hemoglobin from whole blood and the lipid profile differed significantly between the study and control groups. Overall, tmean serum MDA level in the study group was significantly higher thanin the controls. Male sex, addiction to tobacco (smoking and smokeless inclusive), longer duration of diabetes (≥5 years), and presence of complications (both microvascular and macrovascular) significantly increased the MDA level.
    Conclusion: To sum up, the serum MDA level was observed to be significantly high in diabetics with and without complication along with other parameters.

    Abstract A full term newborn baby presented at the age of 2 days with a pedunculated mass arising from the mandibular gingiva and having difficulty in feeding. The mass was excised under general anaesthesia. Histopathology proved it to be a case of congenital epulis. The baby is doing well during follow-up one year after surgery.
