    Abstract Introduction: Obesity is a growing public health problem associated with many comorbid diseases. The aim of this study was to evaluate the relationship between body mass index and complications of thyroidectomy.
    Methods: Patients who underwent total thyroidectomy between January 2015 and December 2018 were enrolled. Patients were divided into two groups, i.e., BMI <25 (group A) and BMI≥ 25 (group B). Demographics, operative time, and complications were retrospectively reviewed.
    Results: The study included 145 patients (66 in Group A and 79 in Group B). There was no significant difference between the two groups in terms of age (p=0.330) and gender (p=0.055). No surgical site infection and bleeding complications were observed in any patients. The mean operative time was 148.4 minutes (90-235) in Group A and 153.4 minutes (85-285) in Group B (p=0.399). Transient hypocalcemia was observed in 25 (37.9%) patients in group A, and 23 (29.1%) patients in Group B (p = 0.291). Permanent hypocalcemia was not observed in any patient in group A, and in 2 patients in Group B (2.5%) (p = 0.501). Transient recurrent nerve palsy was observed in 1 (1.5%) patient in Group A and in 3 (3.8%) patients in Group B (p=0.626). None of the patients had permanent recurrent nerve palsy. Parathyroid autotransplantation was performed on 1 patient (1.5%) in group A and on 7 (8.9%) patients in Group B (p=0.055).
    Conclusion: We think there is no relationship between a high BMI and thyroidectomy complications, and surgery can also be performed safely in this patient group.

    Abstract Objective: The aim of this study was to examine the relationship between risky alcohol use, burnout, and suicide in physicians. Methods: An online questionnaire (SurveyMonkey®) including the Sociodemographic Data Form, Maslach Burnout Inventory, Addiction Profile Index Risk Screening (APIRS), and Suicide Probability Scale (SPS) was created. Results: Two hundred eighty-five physicians took the survey. It was determined that 49 of the physicians (17.2%) were high-risk alcohol users. SPS scores of residents (p=0.014), physicians without children (p=0.019), physicians with a history of suicide attempt (p=0.034) and psychiatric treatment (p=0.001) were found to be significantly higher. Suicide probability scores were weakly correlated with APIRSalcohol scores (rho: 0.138, p=0.02) and highly correlated with burnout scores (r=0.718, p=0.001). Conclusion: Structural and organizational measures should be taken to decrease burnout in physicians, and preventive public health services should be increased to reduce alcohol use in physicians.

    Abstract Objective: Pituitary abscess is a rare condition, with approximately 300 cases reported in the literature. Primary pituitary abscess, the most common type, occurs in previously healthy normal glands. Secondary pituitary abscess is secondary to pre-existing lesions in the pituitary region, such as pituitary adenoma, Rathke's cleft cysts, or craniopharyngioma and infections.
    Material and methods: A total of 2281 patients underwent pituitary surgery via endoscopic transsphenoidal approach in Kocaeli University Pituitary Gland Research Center between 1997 and 2018. Among this cohort 9 patients (4 female and 5 male) were diagnosed with primary pituitary abscess based on both intraoperative findings and postoperative histopathological evidence.
    Results: Primary pituitary abscess incidence was obtained 0.39% in our center. Mean age of the patients was 50 years old. There was no history of pituitary surgery, radiotherapy and infection diseases in our patients. Visual symptoms were prominent in two patients, hypopituitarism was found in 5 out of the 9 patients. All patients have typical pituitary lesion on pituitary magnetic resonance imaging. Staphylococcus species were the most commonly isolated organisms in the culture. A few weeks of antibiotic therapy were administered after surgery.
    Conclusion: Presentation of fever, headache, diabetes insipidus, hypopituitarism and a sellar cystic mass with an enhanced rim after gadolinium contrast on pituitary magnetic resonance imaging may be suggestive of a pituitary abscess. Transsphenoidal endoscopic surgery, proper antibiotics and appropriate hormone replacement therapy when necessary are the keys of pituitary abscess treatment.

    Abstract Tiroid nodülleri tiroid bezinin sık görülen hastalığıdır. Prevalansı yaşla ve iyot eksikliği ile artmakla birlikte son yıllarda başka nedenlerle yapılan görüntüleme tekniklerinde saptanması farkındalığı da arttırmıştır. Tiroid nodüllerinin tanınıp araştırılmasındaki gereklilik bunların malignite potansiyellerinin olmasıdır. Hastalar genellikle boyunda fark ettikleri şişlik nedeniyle başvururlar. Tiroid fonksiyonları değerlendirilmeli beraberinde ultrasonografi ile nodülün özellikleri (boyutu, sayısı, ekojenitesi, kenar özellikleri, kanlanma düzeni) ortaya konulmalıdır. Nodülün klinik ve morfolojik özelliklerine göre tiroid ince iğne aspirasyon biyopsisi (TİİAB) gerekliliği değerlendirilir. İhtiyaç halinde diğer görüntüleme tekniklerinden (Elastosonografi, Bilgisayarlı Tomografi, Magnetik Rezonans, Radyonüklid görüntüleme) faydalanılabilir. Klinik, radyolojik ve TİİAB sonucu birlikte değerlendirilerek nodülün tedavisine ya da izlenmesine karar verilir.

    Abstract Purpose: Metabolic syndrome (MS) is the most important clinical situation which predisposes the endotelial dysfunction and atherosclerosis. Nitric oxid (NO) loss is an important finding in the early stages of endothelial dysfunction. NO deficiency results erectile dysfunction (ED) signs and symptoms. We aim in this study to assess the frequency of the ED in MS patients. Method: This study includes 43 MS patients who fullfill NCEP-ATP III criteria and 29 healthy subjects. A signed permission form was taken from all of the participants. Biochemical and hormonal parameters such as fasting bood glucose (FBG), postprandial glucose (PPG), total testosterone (TT), insulin, high sensitive CRP, total cholesterol (TC), triglyceride (Tg), low density lipoprotein (LDL), high density liporotein (HDL) were measured from the blood which was taken after 12 hours fasting period. ED was evaluated by the questionnaire of International Index of Erectile Function–5 (IIEF–5) which was answered by the included subjects. Insulin resistance was calculated by HOMA-IR method by the formula of FBGxinsulin/405. Results: ED was detected in 65% of the MS group and in 13,8% of the control group. ED was divided into subgroups according to the severity. In MS patients; 18,6% had mild ED, 14% had mild-moderate ED, 14% had moderate ED and 14% had severe ED but in control group only 13,8% of them had mild ED. Although HOMA-IR value was 3,56 ± 3,45 in MS patients with ED, it was 2,23±1,72 in patients without ED (p=0,034). In MS patients with and without ED testosterone levels were measured as 4,5±1 and 6,1±1,56, respectively (p<0,001). Conclusion: ED is highly prevelant in patients with MS and MS patients with ED have low testosterone levels and high HOMA-IR levels which shows insulin resistance.

    Abstract Background: A limited number of human and animal studies suggest that a relationship exists between phthalates and obesity, although this is not supported by all research. The purpose of this study was to investigate the relationship between Body Mass Index (BMI) and the levels of phthalates in human blood and urine samples.
    Methods: Sixty-four overweight or 132 obese individuals (total=196) of different ages (min-max, 17-62; mean ± SD, 42.07±11.3) and genders (F:M 97:99) enrolled in the study. BMI and waist circumference were measured to diagnose obesity. Venous blood samples were taken after overnight fasting. To compare the urine phthalates among participants, single spot urine (at least 10 mL) was collected from the subject after blood samples were taken. Urine and blood phthalate concentrations were measured using gas chromatography.
    Results: Total blood/urinary phthalate levels significantly increased in proportion to the degree of obesity. There was a high correlation between the level of total phthalates in serum and BMI (ρ=0.697, P<0.001), and between total urinary phthalate levels and BMI (ρ=0.707, P<0.001).
    Conclusions: This is the first study to have shown that both blood and urinary phthalates increased in proportion to BMI. The results show a strong association between obesity and phthalates.
