Abstract Sir William Osler (1849-1919), who became Regius Professor of Medicine at Oxford in 1905, first drew attention in 1909 to the painful nodes in subacute bacterial endocarditis, which now carry his eponym, and he published an account in the Quarterly Journal of Medicine, which he helped establish. Attention is drawn to the often overlooked fact that it was a Dr John Alexander Mullin (1835-1899) of Hamilton, Ontario, Canada, who first drew the attention of Sir William Oster to their occurrence. Confusion arose over the relationship between Osler's nodes and the skin lesions described by Theodore Caldwell Janeway (1872-1917), which are generally non-tender and found in acute bacterial endocarditis. The evidence is that there is essentially no difference since their pathogenesis and histological findings are identical.

Abstract: ... A. Atlantic salmon from the St. John River aquaculture strain was generated using PacBio long reads. Information ...

Abstract Atlantic salmon (Salmo salar) in Northeastern US and Eastern Canada has high economic value for the sport fishing and aquaculture industries. Large differences exist between the genomes of Atlantic salmon of European origin and North American (N.A.) origin. Given the genetic and genomic differences between the 2 lineages, it is crucial to develop unique genomic resources for N.A. Atlantic salmon. Here, we describe the resources that we recently developed for genomic and genetic research in N.A. Atlantic salmon aquaculture. Firstly, a new single nucleotide polymorphism (SNP) database for N.A. Atlantic salmon consisting of 3.1 million putative SNPs was generated using data from whole-genome resequencing of 80 N.A. Atlantic salmon individuals. Secondly, a high-density 50K SNP array enriched for the genic regions of the genome and containing 3 sex determination and 61 putative continent of origin markers was developed and validated. Thirdly, a genetic map composed of 27 linkage groups with 36K SNP markers was generated from 2,512 individuals in 141 full-sib families. Finally, a chromosome-level de novo genome assembly from a male N.A. Atlantic salmon from the St. John River aquaculture strain was generated using PacBio long reads. Information from Hi-C proximity ligation sequences and Bionano optical mapping was used to concatenate the contigs into scaffolds. The assembly contains 1,755 scaffolds and only 1,253 gaps, with a total length of 2.83 Gb and N50 of 17.2 Mb. A BUSCO analysis detected 96.2% of the conserved Actinopterygii genes in the assembly, and the genetic linkage information was used to guide the formation of 27 chromosome sequences. Comparative analysis with the reference genome assembly of the European Atlantic salmon confirmed that the karyotype differences between the 2 lineages are caused by a fission in chromosome Ssa01 and 3 chromosome fusions including the p arm of chromosome Ssa01 with Ssa23, Ssa08 with Ssa29, and Ssa26 with Ssa28. The genomic resources we have generated for Atlantic salmon provide a crucial boost for genetic research and for management of farmed and wild populations in this highly valued species.

  6. Article: The 'John Thomas' sign and pelvic fractures-Fact or humorous myth?: A systematic review and meta-analysis.

Abstract: ... view is that of the John Thomas Sign (JTS), which is defined as positive, when the penile shadow points ...

Abstract Objectives: An incidental and sometimes humorous finding on an Antero-Posterior (AP) pelvis X-ray view is that of the John Thomas Sign (JTS), which is defined as positive, when the penile shadow points towards the side of a hip or pelvic fracture in male patients. Despite previous research reports and studies performed on the JTS sign, uncertainty with regards to its clinical relevance still remains. The objective of this review was to assess the clinical validity and accuracy of this sign by performing a systematic review and meta-analysis.
Methods: In accordance with the Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) guidelines, a systematic search was performed (May 2017), using the following databases: BMJ best practice, Cochrane Database of Systematic Reviews, EMBASE, PubMed and Scopus. Studies comparing the predictability of the JTS were critically appraised. The summary effect (and 95% CI) was estimated using a random effect model and the Restricted Maximum Likelihood Method. The pooled sensitivity as well as the summary effect log odds ratio was determined. Selected studies were further ranked for quality and relevance using the Critical Appraisal Skills Program (CASP) (Oxford 2013) tool.
Results: In total, nine articles were isolated and assessed. A total of 1942 participants were included. The mean sample size amongst these included studies was 243 participants with a median of 198 participants. Since the necessary data needed for the Meta-analysis was only present in the 6/9 studies assessed, these 6 were analyzed further. The JTS was positive in 1089 out of 1439 patients with a pooled sensitivity of 75.7% (95%CI, 73.4%-77.9%). There was a large variation in the sensitivity and specificity amongst studies, accounting for a non-significant summary Odds Ratio effect of -0.03.
Conclusions: Based on this review, there is insufficient statistical evidence to support the reliability of the JTS to predict the laterality in the case of a hip fracture on the standard AP pelvis x-ray.

Abstract "Well-written single best answer (SBA) questions are currently held to be the format of choice for knowledge assessments in high stakes undergraduate and postgraduate examinations. They may also be useful adjunct to learning. This chapter contains over 100 SBA questions. Each SBA consists of a clinical scenario, associated question and a list of five plausible options. Of the five options, the most likely option is the correct answer, whilst the other four are incorrect options. In some of the more challenging SBAs one of the four incorrect answers may be partially correct but does not answer the question as well as the correct response"--

Abstract Research has shown that spontaneous visual coupling supports frequency entrainment, phase attraction, and intermittent interpersonal coordination when co-actors are switched from a no-vision (NV) to vision (V) context. In two experiments, co-actors started in a NV context while producing the same or different amplitude movements. The same amplitude resulted in similar self-paced frequencies, while different amplitudes resulted in disparate frequencies. In experiment 1, co-actors were instructed to maintain amplitude while receiving no instructions to coordinate their actions. Frequency and phase entrainment was limited in the V context even when co-actors started the NV context with the same amplitude. In experiment 2, co-actors were instructed to maintain amplitude and intentionally coordinate together, but not at a specific pattern. Significant frequency modulations occurred to maintain amplitude as the co-actors sought to coordinate their actions. With the open-ended instructions, co-actors produced in-phase and anti-phase coordination along with intermittent performance exhibited by shifts between a variety of stable relative phase patterns. The proposed hypotheses and findings are discussed within the context of a shared manifold representation for joint action contexts, with the coordination dynamics expressed by the HKB model of relative phase serving to conceptualization the representations in the shared manifold.
