    Abstract Background: Although care of Parkinsonism (PKM) is assumed to be optimally provided by movement disorder neurologists within an interdisciplinary clinic model, there is a paucity of published data to support this.
    Objectives: To investigate the impact of movement disorder neurologist care of individuals with Parkinsonism (PKM).
    Methods: A retrospective exposure design was adopted using administrative data. Incident PKM individuals were identified in billing claims. A nine-year exposure period to movement disorder neurologist, general neurologist and non-neurologist care was calculated based on the billing codes. Regression models were used to test the association of provider exposure on time to death and long-term care (LTC) admission. Linear models were used to test varying provider exposure and hospital admissions, hospital days and emergency department visits.
    Results: 1914 incident individuals were identified. There was no difference in PKM mortality, emergency visits, hospital admissions, or hospital days between providers, however exposure to general neurology and non-neurology care was associated with a significantly higher risk of admission to LTC compared to movement disorder neurologist care (HR 1.43; 95% CI 1.09-1.87 for general neurology (p-value = 0.0089); HR 1.61; 95% CI 1.25-2.05 for non-neurology (p-value = 0.0002), respectively.
    Conclusion: Movement disorder neurologist care is associated with a lower risk of admission to LTC over general neurologist care in individuals with PKM.

    Abstract In this treatise, we will address one of the higher-risk procedures, subclavian vein cannulation, that a practitioner may undertake in the care of complex patients. All cardiothoracic surgeons and their trainees will need, on occasion, to put in central lines in a variety of circumstances, including in the operating room, in the intensive care unit, in emergency circumstances, and, occasionally, when other practitioners have been unsuccessful in their attempts to place a central line. We will describe, in detail, the anatomy of the subclavian vein, the preparation of the patient for subclavian vein cannulation, the infraclavicular approach to cannulation of the vein, and a few notes about the supraclavicular approach to the subclavian vein. It is self-evident that the priorities of central venous cannulation include safety of insertion, minimizing clot formation, and avoiding infection. We will dwell primarily on the principles of safe subclavian line insertion.

    Abstract Aminoacyl-tRNA synthetase (aaRS)/tRNA cognate pairs translate the genetic code by synthesizing specific aminoacyl-tRNAs that are assembled on messenger RNA by the ribosome. Deconstruction of the two distinct aaRS superfamilies (Classes) has provided conceptual and experimental models for their early evolution. Urzymes, containing ~120-130 amino acids excerpted from regions where genetic coding sequence complementarities have been identified, are key experimental models motivated by the proposal of a single bidirectional ancestral gene. Previous reports that Class I and Class II urzymes accelerate both amino acid activation and tRNA aminoacylation have not been extended to other synthetases. We describe a third urzyme (LeuAC) prepared from the Class IA

    Abstract Aminoacyl-tRNA synthetase (aaRS)/tRNA cognate pairs translate the genetic code by synthesizing specific aminoacyl-tRNAs that are assembled on messenger RNA by the ribosome. Deconstruction of the two distinct aaRS superfamilies (Classes) has provided conceptual and experimental models for their early evolution. Urzymes, containing ~120–130 amino acids excerpted from regions where genetic coding sequence complementarities have been identified, are key experimental models motivated by the proposal of a single bidirectional ancestral gene. Previous reports that Class I and Class II urzymes accelerate both amino acid activation and tRNA aminoacylation have not been extended to other synthetases. We describe a third urzyme (LeuAC) prepared from the Class IA Pyrococcus horikoshii leucyl-tRNA synthetase. We adduce multiple lines of evidence for the authenticity of its catalysis of both canonical reactions, amino acid activation and tRNA Leu aminoacylation. Mutation of the three active-site lysine residues to alanine causes significant, but modest reduction in both amino acid activation and aminoacylation. LeuAC also catalyzes production of ADP, a non-canonical enzymatic function that has been overlooked since it first was described for several full-length aaRS in the 1970s. Structural data suggest that the LeuAC active site accommodates two ATP conformations that are prominent in water but rarely seen bound to proteins, accounting for successive, in situ phosphorylation of the bound leucyl-5′AMP phosphate, accounting for ADP production. This unusual ATP consumption regenerates the transition state for amino acid activation and suggests, in turn, that in the absence of the editing and anticodon-binding domains, LeuAC releases leu-5′AMP unusually slowly, relative to the two phosphorylation reactions.

    Abstract The Primate Research Institute (PRI) dedicated a colony of 81 proven breeders (60 females, 21 males) to the National Chimpanzee Breeding and Research Program (NCBRP). When possible, infants were left with their mothers for a minimum of 18 months. Programs to define and reduce fetal wastage and neonatal mortality were implemented. Pregnancies were diagnosed both by RIA for serum chorionic gonadotropin and by ultrasound prior to day 30 of pregnancy. Of 65 pregnancies detected by ultrasound and RIA, 15 (23%) resulted in fetal loss. Without the use of ultrasound and RIA, only 8 of these losses would have been detected. Infant mortality (7 of 53 live births; 13%) was primarily due to maternal abuse or problems related to premature delivery. The incidence of losses due to maternal abuse has been reduced by placing females in single cages 2 weeks prior to their estimated delivery date so endangered infants can be recognized and removed. The new system provided the first opportunity for most of the mothers to care for their infants; 25 succeeded (in 2 instances in the second pregnancy only) and 12 did not (infants from 2 mothers were removed due to delivery of twins). During this 26-month period the colony grew by 46 infants (58%), half of which will be retained as future breeders. Thus, the NCBRP goal of achieving a self-sustaining population appears viable.

    Abstract Recent studies of children have argued that children are suffering from a deficiency in nature experience. Some argue that a lack of experience leads to poor affective relations which for wooded environments may be manifested as fear. This study investigates a geographical knowledge gap in understanding children’s relationships with woodland. This interactive qualitative study included 21 junior age children living in a rural setting in Derbyshire, England, UK. Most were found to visit local woodlands regularly, though unsupervised visits were usually limited to woods adjacent to housing. The children demonstrated good levels of practical knowledge though explicit knowledge, such as tree names, was generally poor. The majority of children had positive attitudes towards woodland, especially those with the greatest experience. Adventure, calm and freedom were identified as major themes. Fear was widespread but rarely dominated and was often associated with exhilaration linked to cultural imaginaries such as computer games and films.
