    Abstract In the care of adolescents, health care providers often face situations raising ethical concerns or dilemmas, such as refusal of a treatment or hospitalization, or request of confidentiality while engaging in risky behaviors or facing unplanned pregnancy. This position paper provides concrete avenues as how to assess the adolescent's capacity for autonomous decision making, e.g. the patient's competence in a specific situation, and how to elicit informed choice or consent. To do so, professionals need to be sensitized and trained as how to assess the cognitive and socio-psychological development of the young patient. Another challenge for the health professionals is to balance the needs to support patient's autonomy while offering secure guidance and protection if needed. To optimize such a process, they establish a climate of trust and empathy that will allow the patient to participate freely in the decision. In addition, especially when the decisions have potentially important consequences on the health and life, the professionals include, with the adolescent's permission, parents, caregivers or other significant adults, as well as they may request the opinion of other members of the health care team or expert colleagues such as ethicists.

    Abstract Background: Patients with hepatocellular carcinoma (HCC) have been overprioritized in the deceased donor liver allocation system. The United Network for Organ Sharing adopted a policy in May 2019 that limited HCC exception points to the median Model for End-Stage Liver Disease at transplant in the listing region minus 3. We hypothesized this policy change would increase the likelihood to transplant marginal quality livers into HCC patients.
    Methods: This was a retrospective cohort study of a national transplant registry, including adult deceased donor liver transplant recipients with and without HCC from May 18, 2017, to May 18, 2019 (prepolicy) to May 19, 2019, to March 1, 2021 (postpolicy). Transplanted livers were considered of marginal quality if they met ≥1 of the following: (1) donation after circulatory death, (2) donor age ≥70, (3) macrosteatosis ≥30% and (4) donor risk index ≥95th percentile. We compared characteristics across policy periods and by HCC status.
    Results: A total of 23 164 patients were included (11 339 prepolicy and 11 825 postpolicy), 22.7% of whom received HCC exception points (prepolicy versus postpolicy: 26.1% versus 19.4%; P = 0.03). The percentage of transplanted donor livers meeting marginal quality criteria decreased for non-HCC (17.3% versus 16.0%; P < 0.001) but increased for HCC (17.7% versus 19.4%; P < 0.001) prepolicy versus postpolicy. After adjusting for recipient characteristics, HCC recipients had 28% higher odds of being transplanted with marginal quality liver independent of policy period (odds ratio: 1.28; confidence interval, 1.09-1.50; P < 0.01).
    Conclusions: The median Model for End-Stage Liver Disease at transplant in the listing region minus 3 policy limited exception points and decreased the quality of livers received by HCC patients.

    Abstract Adolescents aged 10 to 19 live a period of their life marked by opportunities and vulnerabilities during which the issue of mental health is of prime importance. Since several decades, and especially since the start of the COVID pandemic, mental health problems and disorders among adolescents have increased around the world. Depression, self-harm and suicidal behavior are common during this period of life, and pediatricians can play a pivotal role in identifying affected or vulnerable youngsters. This article describes risk factors for self-harm and suicidal behavior and reviews how primary care pediatricians and health professionals can respond to such situations. This scoping review is based on existing evidences as well as the authors clinical experience. It suggests concrete actions that can be taken to secure the life of at risk teenagers, and discusses how to organize the transfer to mental health professionals when needed.

    Abstract Während der COVID-19-Pandemie waren audiologische Untersuchungen im Labor nur unter sehr strengen Hygienemaßnahmen bzw. gar nicht möglich. Um dieses Hindernis zu umgehen, wurde ein Online-Experiment mit dem synthetischen Sprachmaterial des Oldenburger Satztests (OLSA) implementiert. Dadurch sollte geprüft werden, ob online mit synthetischer Sprache vergleichbare Ergebnisse erzielt werden wie in einem vorhergehenden Experiment im Labor. Mithilfe der Plattform Gorilla (https://gorilla.sc/) wurde ein Online-Experiment erstellt und durch einen Link über verschiedene Kommunikationskanäle (z. B. in Social Media) weitergegeben. Aufgerufen wurde dieser Link 182 Mal, was zu 81 vollständigen Datensätzen führte. Die ermittelten Sprachverständlichkeitsschwellen von 69 Probanden sind vergleichbar mit den Ergebnissen aus der Laborsituation. Allerdings wichen die Steigungen der Diskriminationsfunktionen signifikant voneinander ab. Grundsätzlich konnte gezeigt werden, dass der synthetische OLSA auch als Online-Experiment durchführbar ist. Hürden im Vergleich zur Labormessung zeigten sich jedoch bei Rekrutierung und Motivation der Probanden.

    During the COVID-19 pandemic, audiological tests in the laboratory were only possible under very strict hygiene measures or not at all. To circumvent this obstacle, an online experiment was implemented using the synthetic speech material of the Oldenburg Sentence Test (OLSA). The aim was to test whether comparable results could be achieved online with synthetic speech as used in a previous experiment in the laboratory. With the help of the platform Gorilla (https://gorilla.sc/), an online experiment was created and shared through a link via various communication channels (e.g., in social media). This link was accessed 182 times, resulting in 81 complete data sets. The speech-recognition thresholds established for 69 participants were comparable to the results from the laboratory situation. However, the slopes of the discrimination functions differed significantly. In principle, it was shown that the synthetic OLSA is also feasible as an online experiment. Compared to the laboratory measurement, greater hurdles were encountered in the recruitment and motivation of the test participants.
