    Abstract A fistulous communication between the appendix and any viscus is rare. Such fistula is most often acquired due to recurrent appendicitis, cystic fibrosis, Crohn's disease, tuberculosis, and malignancy. Here in, we report a rare case of an appendico-ileal fistula incidentally detected during laparotomy for adhesive small bowel obstruction. The fistula was divided, the ileal opening was sutured, and appendectomy was performed. Postoperative recovery was uneventful, with no evidence of malignancy, tuberculosis, or inflammatory bowel disease on the histopathological examination of the appendix.

    Abstract Metastatic gastrointestinal neuroendocrine tumors classically appear as contrast-enhancing lesions on computed tomography. However, in a small percentage of patients, these lesions can be cystic in nature, leading to false diagnoses of benign or infectious lesions such as echinococcosis. Hence, every cystic lesion of the liver must be carefully investigated before making the treatment plan. We report a patient with hematemesis caused by a large gastric ulcer with multiple cystic lesions in the left lobe of the liver abutting the stomach. The liver lesions were misdiagnosed as hepatic echinococcosis, and the patient was started on medical therapy. However, when medical therapy failed, the patient underwent surgical excision and the histopathology showed cystic metastases of a gastric neuroendocrine tumor.

    Abstract This paper examines the employment effects of an emergency assistance package by the Indian government, the Garib Kalyan Rozgar Abhiyaan that had the sole objective of providing employment to returning migrants. It was targeted to 116 districts that had seen returning migrants in excess of 25,000, was limited in duration to 4 months, and was directed at top-up funding to public works and 25 other target sectors in rural areas. Using a sharp RD approach, we find that the intervention had substantive impacts on employment and in reducing rationing in public works and that it did so in a cost-effective manner. In contrast to the widespread impression of a slow-moving bureaucracy, these results point to an administrative machinery that was able to successfully implement this project within a relatively short period of time.

    Abstract Background: Rapunzel Syndrome associated with Trichobezoar secondary to a psychiatric illness Trichotillomania, is often a rare diagnosis of Gastrointestinal symptoms, occurring predominantly in young & adolescent females. Clinical Description: We present here a case of such disease in a 12-year-old female who presented to us with recurrent vomiting & pain abdomen for the last 2 months. Management & Outcome: On thorough examination & investigations, she was suspected to be a case of trichobezoar on ultrasound abdomen, which was confirmed by upper GI endoscopy. Mass was surgically removed from the patient’s abdomen with a long tail of Rapunzel. Conclusion: Although trichobezoar is a rare condition, it must be considered in patients with trichotillophagia & abdominal symptoms. Symptomatic trichobezoars are often so large that they are palpable on simple clinical examination as in this case. USG may be inconclusive and endoscopy is confirmative. Undiagnosed or untreated trichobezoar continues to grow in size & weight, resulting in devastating complications, such as gastric ulcers, perforation, or even death.

    Abstract Myelocystoceles, also known as syringoceles, syringomyeloceles, and lipomeningomyelocystoceles, represent 5% of all lumbosacral skin-covered masses. Terminal myelocystocele (TMC) is a rare variant of spinal dysraphism. At times, patients with TMCs can present with a huge lumbosacral mass (giant TMC). A 14-month-old female child presented with progressive increasing swelling in the lumbosacral region (35 cm × 35 cm × 30 cm) since birth with inability to move both lower limbs. Magnetic resonance imaging revealed a dilated terminal central canal herniating through a spina bifida defect (from L4 to S2) into a huge meningocele, suggestive of a giant TMC. The girl was operated on successfully and was doing well at 6 months follow-up, although neurologically unchanged. Giant TMC is a rare entity and only a few case reports are available in literature. Herein we present a case with TMC with the largest dimensions reported till date.

    Abstract Using a survey of 1,500 farmers, we identify farmer-level constraints to adoption of modern cultivars in Rajasthan, India, and decompose the overall elasticity into the elasticities of adoption probability and use intensity. We find that access to credit and intensity of extension services received drive adoption of modern cultivars, but both factors are more important for determining adoption probability than for determining use intensity. Moreover, the study assess the roles that key traits of individual cultivars play in their adoption. Varietal traits significant for the adoption of modern cultivars vary by crop and geography. However, the main driver for continuing old cultivars across crops is taste attributes.

    Abstract Agriculture sector in India is a primary source of livelihood for a majority of the population. Low and stagnant income in the sector remains a focal point of policy debate in India. The most prominent pathways to enhance farmers’ income is the adoption of improved agricultural technologies. This study documents the current state of agriculture technologies in India. The main objectives are: (a) What are the adoption levels of improved technologies and their impact on farmers’ income, agricultural production, natural resources and environment? (b) What are the constraints in up-scaling improved technologies and the conditions for success of their adoption? (c) What are the rate of return on agriculture research and extension system? and (d) What can be learnt from the global perspective on agriculture research and extension services?
