Abstract This is a case series report of 19 palliative care patients where 23 acupuncture sessions were performed which included Yintang (EX-HN 3) acupuncture or acupressure for the relief of terminal restlessness, anxiety or psychological distress present during the dying process. There was an observable relief from the restlessness, anxiety and distress both in sessions where only Yintang (EX-HN 3) point acupuncture or acupressure was performed (observed in 10 out of 11 sessions) as well as in sessions where Yintang (EX-HN 3) point acupuncture or acupressure was performed together with additional interventions, such as other points acupuncture, ear acupuncture or benzodiazepine treatment (observed in 10 out of 12 sessions). In total relief was observed in 20 out of 23 sessions (86.9%). A hypothesis that might worth further testing is whether Yintang (EX-HN 3) acupuncture or acupressure has an anxiolytic, tranquillising or sedative effect in dying patients. If confirmed this could be potentially useful in the fields of palliative care or disaster/triage medicine.

Abstract Multi-temporal Land use and Land cover (LULC) monitoring is a crucial parameter for assessing an area’s landscape ecology regime. LULC changes can be effectively used to describe dynamics of both urban or rural environments and vegetation patterns as an important indicator of ecological environments. In this context, spatial land use properties can be quantified by using a set of landscape metrics. Landscape metrics capture inherent spatial structure of the environment and are used to enhance interpretation of spatial pattern of the landscape. This study aims to monitor diachronically the LULC regime of the island of Crete, Greece with the use of Landsat satellite imageries (Landsat 5, Landsat-7 and Landsat-8) in terms of soil erosion. For this reason, radiometric and atmospheric corrections are applied to all satellite products and unsupervised classification algorithms are used to develop detail LULC maps of the island. The LULC classes are developed by generalizing basic CORINE classes. Following, various landscape metrics are applied to estimate the temporal changes in LULC patterns of the island. The results denote that the diachronic research of spatial patterns evolution can effectively assist to the investigation of the structure, function and landscape pattern changes.

Abstract An adult female presented with acute watery diarrhoea and severe dehydration during a cholera outbreak in Mae La refugee camp, Thailand in 2007. Her stools had the classic appearance of cholera ''rice water stools'', which is similar to water where rice was boiled. Stool are watery, greyish in colour, cloudy, with flecks of mucus, with a non offensive fish-like odour (Panels A, B, C and D from this patient). The patient was isolated, treated initially with Ringers's lactate intravenously and then with oral rehydration solution. Tetracycline per os was given for three days according to local protocol and the patient recovered completely. A culture of stools ob-tained by rectal swab upon admission subsequently tested positive for Vibrio Cholerae, 01 Inaba serotype, which was also the cause of the outbreak. Cholera is transmitted through contami-nated food or water, can cause epidemics and may also infect travellers. Clinical spectrum ranges from asymptomatic to severe disease with massive watery diarrhoea often fatal if untreated. Treatment includes rapid fluid replacement with the WHO's reduced osmolarity oral rehydration solution or intravenous rehydration (Ringer's lactate) for severe cases. Antibiotics (azithromycin, erythromycin, tetracycline or ciprofloxacine) decrease disease duration.

Abstract A 29 years old male from Bangladesh presented in the emergency department with severe abdominal pain and symptoms of gastric outlet obstruction. Epigastric tenderness, tachycardia, low grade fever, leucocytosis (WBC: 14900/microL) with neutrophilia (Neu:10900/microL) and eosinophilia (Eos:600/microL) were noted, as well as features of obstructive jaundice (increased AST, ALT, and total bilirubin). Stomach dilation was noted on a chest X-ray (A), while in a plain abdominal erect X-ray (C) tubular curvilinear soft tissue densities in a jejunal loop were identified (D). An abdominal CT scan showed stomach dilation and gave the impression of thickening of the pyloric wall and superior part of duodenum (B). In addition, a distended gallbladder, a common bile duct of one cm in diameter and borderline distention of intrahepatic ducts were observed. Subsequently, a nasogastric tube was placed which drained 2 litres of food containing liquid. Antibiotics, opioid analgesics and intravenous fluids were also given. During gastroduodenoscopy a bundle of several entangled Ascaris Lumbricoides roundworms was identified obstructing the duodenum lumen and some of them were removed endoscopically. Following albendazole treatment for three days and passing several dead worms per rectum, the patient recovered and radiological findings resolved completely.

Abstract Complex brain disorders, including Alzheimer's dementia, sleep disorders, and epilepsy, are chronic conditions that have high prevalence individually and in combination, increasing mortality risk, and contributing to the socioeconomic burden of patients, their families and, their communities at large. Although some literature reviews have been conducted mentioning the available methods and tools used for supporting the diagnosis of complex brain disorders and processing different files, there are still limitations. Specifically, these research works have focused primarily on one single brain disorder, i.e., sleep disorders or dementia or epilepsy. Additionally, existing research initiatives mentioning some tools, focus mainly on one single type of data, i.e., electroencephalography (EEG) signals or actigraphies or Magnetic Resonance Imaging, and so on. To tackle the aforementioned limitations, this is the first study conducting a comprehensive literature review of the available methods used for supporting the diagnosis of multiple complex brain disorders, i.e., Alzheimer's dementia, sleep disorders, epilepsy. Also, to the best of our knowledge, we present the first study conducting a comprehensive literature review of all the available tools, which can be exploited for processing multiple types of data, including EEG, actigraphies, and MRIs, and receiving valuable forms of information which can be used for differentiating people in a healthy control group and patients suffering from complex brain disorders. Additionally, the present study highlights both the benefits and limitations of the existing available tools.

Abstract Climate change and land use/cover (LULC) changes are considered as main agents of soil erosion by water. Hence, evaluating both the past and future trends in rainfall and LULC patterns is crucial to seek their multi-temporal impacts on the erosion. In this line, the main objective of the present study was to assess the past and project the future variabilities of water-induced soil erosion under changes in rainfall and LULC patterns for the Mediterranean island of Crete (Greece). They were based on the estimation of soil loss rate by the GIS-based empirical model of RUSLE, and referred to three past periods (1991–2000, 2001–2010 and 2011–2020) and four periods of near and far future (2021–2030, 2031–2040, 2041–2050 and 2051–2100) under three different climate change scenarios (RCP2.6, RCP4.5 and RCP8.5). The relevant dynamics of R and C-factors incorporated in RUSLE were determined by using rainfall multi-historical observations and future simulations for the former, and LULC outputs derived from satellite images and CA-Markov model for the latter. The RUSLE outputs showed a gradual increase of mean soil loss during the past periods as a result of continuously increasing rainfall erosivity and bare land extent. However, the generally decreased levels projected for them in the future are expected to lead to a subsequent decrease of mean soil loss. In all cases, the mountainous parts of Crete and especially in the west were found to be more affected by erosion. Compared to the more recent past period of 2011–2020, the future period of 2041–2050 was projected to be the most notable in terms of potential risk, indicating the only scenario of increase (+0.4) in mean soil loss as well as the highest coverage (a total of 8 %) by areas of high and extreme loss. The RCP8.5 and RCP2.6 were projected to be the most negative climate change scenarios for the near (until 2040) and far (2040 onwards) future, respectively, presenting the smallest decreases (about −16 % and −5 %, respectively) in mean soil loss. The study's findings can offer knowledge indispensable for future sustainable policy-making and spatial planning.
