    Abstract Background: The use of fluids is the most frequently used therapy for circulatory shock. Its inadequate use has adverse effects, requiring hemodynamic goals, highlighting the use of pulse pressure variation due to its high accuracy. One of the problems related to this method is the difficulties in measuring the pulse pressure variation (PPV) in most monitors for clinical use. We assessed the qualitative aspects of perceived usability of a smartphone application (app), which, based on a photograph of the patient's arterial pulse wave, can help measure PPV and help in the diagnosis and management of shock cases.
    Methods: To assess the software perceived usability, we used the System Usability Scale (SUS) applied to 30 physicians in 2 tertiary hospitals in Brazil. The software accuracy was measured using a sequence of 3 images with different values ​​of pulse pressure variation, comparing the obtained values ​​ with the gold standard. The educational interface of the app was evaluated qualitativelyfrom the spontaneous testimonies of the selected test participants.The project was approved by the Research Ethics Committee of Centro Universitário Christus.
    Results: The analysis showed an average SUS of 86.3 points on a scale of 1-100 (above 80.3 is considered the best in terms of interface). The assessment of the application's accuracy when evaluating pulse pressure variation showed that the average variation of the measurements taken by the participants was small, with a good measure of repeatability and reproducibility. The app's educational interface was qualitatively evaluated, being praised by the users.
    Conclusions: It can be concluded that the developed mobile application showed excellent qualitative aspects of perceived usability results. More studies with this app will be required to evaluate the potential to help professionals with hemodynamic evaluation in emergency and intensive care settings.

    Abstract Infection with SARS-CoV-2 causes critical disease in approximately 5% of affected patients, particularly the elderly, hypertensive, obese and immunocompromised. Patients with haematological cancer, including chronic lymphocytic leukaemia (CLL), are particularly at risk of complications. Very rarely, patients with extreme leukocytosis may develop spurious hypoxemia, or pseudohypoxemia, which confuses the diagnosis of complications and can lead to intervention errors. We report the case of a patient with CLL, severe infection with SARS-CoV-2 and pseudohypoxemia.
    Learning points: Patients with haematological neoplasms are susceptible to viral, bacterial and fungal infections, and are thus at risk of COVID-19.Patients with chronic lymphocytic leukaemia may rarely, due to a high lymphocyte count, present with spurious, or in vitro, hypoxemia.The clinician must identify and properly treat such cases to prevent any unnecessary treatments and their complications.

    Abstract Although tuberculosis (TB) is relatively common in pregnancy and puerperium, its disseminated form is a rare event, with potential lethal maternal and fetal outcomes. Due to the infrequency and lack of knowledge of most physicians about the various manifestations of the disease, the diagnosis is often overlooked, and treatment postponed, resulting in a high death rate. We report a fatal case of disseminated tuberculosis in the puerperium, with lung, brain, liver and uterine involvement. After, we briefly review the clinical manifestations of TB in the gestational period.

    Abstract We report a case of hemoglobin (Hb) Willamette (β51 Pro → Arg) in the Hematology Department of a tertiary hospital in Fortaleza, Northeast of Brazil. A literature review of the cases described in health sciences databases using as a descriptor Hb Willamette was performed, revealing 12 reported cases, of which only one presented with anemia. Herein, we describe a case of a female 29 years old, with hemoglobinopathy Willamette presenting clinically with anemia, having the lowest hemoglobin rate of the published cases. The relatives of the patient were evaluated andthe patient's mother corresponded to the first description of the association between Hb Willamette and HbC. Among the hemoglobinopathies, hemoglobin Willamette is an extremely rare disease; therefore it is important to analyze its clinical and laboratory manifestations for accurate diagnosis and assessment of potential interactions with other genetic variants.

    Abstract This study aimed to assess the impact of different land uses on the spore density and richness of arbuscular mycorrhizal fungi (AMF) in a Red Latosol of a tropical savanna biome in Brazil (“Cerrado”). Ten soil samples and roots were obtained from a riparian forest, a pasture, an annual crop production system with no tillage, and a coffee plantation in dry and wet seasons. Spores were extracted, counted, and identified from field soils, and roots were stained to assess mycorrhizal colonization. A total of 42 AMF species were detected in all four land uses. The genus Acaulospora had the largest number of species (18), followed by Glomus (6) and Gigaspora (5). Gigaspora decipiens and Gigaspora margarita were present in all areas, regardless of the sample period. Gigaspora decipiens was the species most frequently recovered and contributed to the majority of spores in coffee plantations and riparian forest. Cetraspora pellucida was dominant in the area of no tillage and Acaulospora tuberculata in the pasture. Low species richness was detected in soils under the coffee plantation, with AMF communities dominated almost exclusively by members of Gigasporaceae. We conclude that the area under the coffee plantation caused a reduction in the richness of AMF species and promoted a dominance of a single family AMF compared with the pasture area.

    Abstract Gastric cancer during pregnancy is a rare event and difficult to diagnose, as the symptoms can be confused with those of an ordinary pregnancy. We report a case of a 25-year-old patient with a 29-week gestation, with asthenia complaint, vomiting and weight loss. During the investigation of wasting syndrome endoscopy was performed with infiltrative ulcerative lesions in pre-pyloric region with biopsy revealing carcinoma with signet ring, undifferentiated type. It was held on a strict control of fetal vitality, and pregnancy was interrupted via the abdominal delivery at 34 weeks. Soon after the cesarean section was performed, exploratory laparotomy was performed to perform inventory of the abdominal cavity, being observed the presence of carcinomatous implants in the peritoneum. In the face of irresectability clinical conduct was adopted and the patient was sent to chemotherapy, ensuring nutrition via a jejunostomy. The article reviews the gastric carcinoma association with pregnancy, discussing the initiation of treatment and continuity of pregnancy.
