    Abstract The medical director is an important member of the healthcare team in a nursing home, and is responsible for overall coordination of care and for implementation of policies related to care of the residents in a nursing home. The residents in nursing homes are frail, medically complex, and have multiple disabilities. The medical director has an important leadership role in assisting nursing home administration in providing quality care that is consistent with current standards of care. This article provides an overview of roles and functions of the medical director, and suggests ways the medical director can be instrumental in achieving excellent care in today's nursing facilities.

    Abstract We report on a chemiresistive gas sensor using boron nanostructures as the sensing layer, to detect methane gas down to 50 ppm. The sensor showed an excellent response of 43.5-153.1% for a methane concentration of 50 ppm to 105 ppm, with linear behaviour and good response and recovery time. The stability, repeatability, reproducibility, and shelf life of the sensor are promising for next generation methane gas detection.

    Abstract: A lead-free (K,Na)­NbO₃-based piezoelectric ceramic is textured along the (001) direction using the NaNbO₃ (NN) seeds. The composition 0.96­(K₀.₅Na₀.₅)­(Nb₀.₉₆₅Sb₀.₀₃₅)­O₃-0.01CaZrO₃-0.03­(Bi₀.₅K₀.₅)­HfO₃ (KNN) is found to provide an excellent ... ...

    Abstract A lead-free (K,Na)­NbO₃-based piezoelectric ceramic is textured along the (001) direction using the NaNbO₃ (NN) seeds. The composition 0.96­(K₀.₅Na₀.₅)­(Nb₀.₉₆₅Sb₀.₀₃₅)­O₃-0.01CaZrO₃-0.03­(Bi₀.₅K₀.₅)­HfO₃ (KNN) is found to provide an excellent combination of electromechanical coefficients at room temperature. The textured composition with 5 wt % NN template (KNN-5NN) exhibits considerably improved electromechanical coefficients, d ₃₃ ∼ 590 pC/N, k ₃₁ ∼ 0.46, and d ₃₁ ∼ 215 ×10-¹² C/N, at room temperature. A flexible piezoelectric energy harvester (F-PEH) is fabricated using the textured KNN-5NN ceramic and tested under cyclic force. F-PEH exhibits enhanced output voltage (V ₒc ∼ 25 V), current (I ∼ 0.4 μA), and power density (P D ∼ 5.5 mW/m²) (R L of 10 MΩ) in the off-resonance frequency regime. In comparison to the random ceramic KNN-0NN-based F-PEH (V ₒc ∼ 8 V and I ∼ 0.1 μA), the textured F-PEH significantly outperformed energy harvesting capability due to the large figure-of-merit value (d ₃₁ × g ₃₁) ∼ 3354 ×10-¹⁵ m³/J. This work provides a methodology for texturing lead-free materials and further implementing them in flexible energy harvesting devices and sensors.

    Abstract Background: Microglial overactivation promotes the production of various second messengers and inflammatory markers in brain tissue, resulting in neuroinflammation and neurodegeneration, which may lead to cognitive decline. The cyclic nucleotides are one of the important second messengers involved in the regulation of neurogenesis, synaptic plasticity, and cognition. The levels of these cyclic nucleotides are maintained by phosphodiesterase enzyme isoforms, particularly PDE4B, in the brain. An imbalance between PDE4B levels and cyclic nucleotides may lead to aggravating neuroinflammation.
    Methods: Lipopolysaccharides (LPS) were administered intraperitoneally on alternate days for 7 days at a dose of 500 μg/kg in mice, which triggered systemic inflammation. This may lead to the activation of glial cells and may activate oxidative stress and neuroinflammatory markers in brain tissue. Furthermore, oral administration of roflumilast (0.1, 0.2, and 0.4 mg/kg) in this model ameliorated oxidative stress markers, neuroinflammation and improved neurobehavioral parameters in these animals.
    Results: The detrimental effect of LPS increased oxidative stress, AChE enzyme levels, and decreased catalase levels in brain tissues, along with memory impairment in animals. Moreover, it also enhanced the activity and expression of the PDE4B enzyme, resulting in a decline in cyclic nucleotide levels. Furthermore, treatment with roflumilast improved the cognitive decline, decreased AChE enzyme level, and increased the catalase enzyme level. Roflumilast also reduced the PDE4B expression in a dose-dependent manner, which LPS up-regulated.
    Conclusion: Roflumilast has shown an anti-neuroinflammatory effect and reversed the cognitive decline in LPS-induced mice model.

    Abstract Anemia has a higher prevalence among residents of long-term care setting. Signs and symptoms of anemia in this group are more insidious and can be overlooked and attributed to other disease manifestations or old age. Available data on the consequences of anemia suggest worse outcomes in heart failure; cognitive and functional decline; and increased rates of falls, hospitalizations, and mortality. Diagnosis and treatment of anemia in long-term care residents should be considered based on cost and benefit to the patient and patient's and/or caregiver's preferences.
