Abstract Purpose: Macular telangiectasia (MacTel) type 2 is observed in patients in their 5th-8th decades of life. The clinical and imaging findings in younger patients is unknown in larger cohorts. The study purpose is to report prevalence, baseline clinical and spectral domain optical coherence tomography (SDOCT) findings in young MacTel patients below 40 years.
Methods: This hospital-based, multicentre, retrospective, cross-sectional study included patients between 2011 and 2023. Retinal photographs from multiple imaging techniques were evaluated to diagnose and stage type 2 MacTel and describe their SDOCT findings. Imaging characteristics were correlated with clinical stages and visual acuity.
Results: Among all MacTel patients seen in hospital, prevalence of young MacTel cases less than age 40 was 1.77% (32/1806; 62 eyes). Youngest participant was 34 years, while mean age was 38.44 ± 1.795 years. Sixteen patients (50%) were diabetics. Perifoveal greying (n = 56, 90%) and perifoveal hyperreflective middle retinal layers (n = 47, 76%) were the most prevalent clinical and SDOCT imaging finding respectively. Less than 10% (n = 6) eyes had proliferative disease. Presence of retinal pigment clumps (RPC) (7% vs. 67%; p = 0.002) coincided with proliferative MacTel. Poor vision was associated with presence of outer retinal layer SDOCT findings like outward bending of inner retinal layers (p = 0.047), RPC (p = 0.007), subfoveal neurosensory detachment (p = 0.048) and subretinal neovascular membrane (p = 0.001).
Conclusion: Type 2 MacTel before age 40 is rare, common in women and diabetics, and affects vision in advanced stage. Disease symmetry, comparison with older cases, and longitudinal SDOCT changes in such patients require further study.

Abstract Patients suffering from diseases that occur due to spreading of virus like fever and cold will have decrease in body temperature. They feel cold in the normal body and room temperature conditions. For the comfort of these patients, an electric under blanket is designed which warms up the patient to maintain the normal body temperature. The heated under body supports include a heater assembly and a layer of compressible support material. The heater assembly includes a flexible heating element, multiplex polyester, and a temperature sensor. The flexible heater element may include a fabric, which coated with a conductive or semiconductive polymer. The heated under body support may also include a water resistant shell, whereas it may encase the heater assembly and the compressible support material. The material used for outer shell and inner heating element has simulated in COMSOL tool for analyzing the heat transfer between them. The proto type model has simulated in PROTEUS software, which includes Arduino UNO and thermistor. This analysis will give the result whether the material can be used as the under garment for warming the patient.

Abstract We report a rare case of co-existence of dominant dystrophic epidermolysis bullosa (EB) and juvenile idiopathic arthritis (JIA) in an 18-year-old male patient, who is born out of consanguineous marriage. He presented with blistering skin lesions all over the body since the 2nd day after his birth, anonychia of all the toes from childhood and symmetric polyarticular inflammatory arthritis for 4 years. Erythrocyte sedimentation rate and C-reactive protein were elevated. Skin biopsy of induced vesicle was done, and histopathology showed epidermis with basket-weave hyperkeratosis, basal pigmented layer and sub-epidermal free bulla, while dermis revealed a few thin-walled proliferating capillaries with sparse lymphocytes surrounding them. Immunofluorescence showed immunoglobulin G along the basement membrane confirming the diagnosis of EB. He was diagnosed to have polyarticular rheumatoid factor-negative JIA. Treatment with oral methylprednisolone, disease-modifying anti-rheumatic drugs and intra-articular triamcinolone injections resulted in symptomatic relief of the joint pains.

Abstract : Iliotibial band syndrome (ITBS) is the most common injury of the lateral side of the knee in runners. Runners typically complain of persistent lateral knee pain not associated with swelling, usually it occurs due to one to two miles of running and further worsening of the pain during running on the downhill. The popularity of running is still growing and, as participation increases, the incidence of running-related injuries will also increase. The Iliotibial track (ITT) or the band is an anatomical structure of the lateral upper leg that recently has been highly published as an overused structure during sports. A friction syndrome has been attributed to excessive distance running, inappropriate running regimens and worn footwear [1] . Hence we have taken up this study to study about the effect of cryotherapy and kinesio taping technique with stretching exercise in patients with iliotibial band friction syndrome in long distance runners.

Abstract Jacob's disease is a rare condition consisting of pseudojoint formation between enlarged coronoid process and the inner surface of the zygoma. The authors report a 45-year-old female patient who presented with limited mouth opening (5 mm) and swelling of the left zygoma. No temporomandibular joint disease was diagnosed. 3D computed tomography images showed a mushroom-shaped coronoid growth with pseudojoint formation medial and superior to the zygomatic arch. Under general anaesthesia, coronoidectomy was carried out through a combined extraoral and intraoral approach. Histopathological examination revealed normal trabecular bone covered with hyaline cartilage. To the best of the authors' knowledge this case is the fifth typical mushroom-shaped Jacob's disease; only four similar cases have been reported. A review of the literature revealed only 39 histologically proved cases of Jacob's disease cases up to 2010. Aggressive physical treatment was given to improve function and maximum mouth opening, which had increased to 40 mm at 3 months follow-up. The literature concerning this condition's aetiology, pathogenesis, clinical characteristics, diagnosis, and treatment is reviewed.
