    Abstract Background: Myasthenia gravis (MG) patients interact unpredictably with anesthetic agents, including neuromuscular blocking agents. Here, we investigate the effectiveness of general anesthesia without muscle relaxants using either propofol via target-controlled infusion systems (TCI) or sevoflurane in MG patients undergoing thoracoscopic thymectomy.
    Methods: This prospective, open-label, observational study was conducted in a university hospital. We included 90 myasthenic patients undergoing thoracoscopic thymectomy with general anesthesia. Patients received induction and maintenance anesthesia with propofol TCI (group P, n = 45) or induction with propofol 2-3 mg.kg
    Results: All patients achieved good intubation conditions. Hemodynamic instability was more frequent in group S than in group P, mostly in the induction stage, and was controllable. The reduction in the intraoperative train-of-four ratio from baseline at 30 min, 60 min, and 90 min in group S was 10.3%, 14.2%, and 14.3%, respectively, significantly higher than that in group P (6.8%, 7.2%, and 8.4%, respectively), which completely recovered at the end of the surgery. All patients were extubated in the operating room without complications. No other significant differences between the groups were observed.
    Conclusions: Anesthesia with propofol TCI or sevoflurane without muscle relaxants in MG patients offered safe and effective conditions for thoracoscopic thymectomy. Sevoflurane achieved higher levels of intraoperative muscular relaxation than propofol TCI. Postoperative neuromuscular function was not affected by these anesthetics.

    Abstract Multiple complicated concurrent hernias with obturator hernia and paraesophageal hernia unusually occur in clinical settings. The obturator hernias belong to a rare pelvic hernia that accounts for a minority of all abdominal hernias. Besides, paraesophageal hernias occur commonly in elderly female patients. Clinical manifestations of these hernias are usually unspecific and the diagnosis is based on computed tomography (CT). In this paper, we presented a case of multiple complicated hernias in an 81-year-old woman. She was admitted to our hospital due to intestinal obstruction that was caused by a simultaneous obturator and paraesophageal hernia. She was successfully treated by laparoscopic hernia repair. Postoperative progression was favorable. She was then discharged from the hospital after four hospital days.

    Abstract Aerosolized microorganisms have become an important factor in assessing air quality. To determine the characteristics of bacterial bioaerosols in air and rainwater, as well as calculate the recovery rate of bacteria after rains in Ho Chi Minh City, our study was performed using the SKC Biostage sampler for airborne bacteria and Plate Count Agar (PCA) medium for bacterial concentration. Subsequently, the study determined the bacterial community composition at the phylum and order levels using the 16S rRNA (16S metabarcoding) method.     Abstract Objective: Minimally invasive cardiac surgery has not been widely applied in repairing sinus venosus atrial septal defect (SV-ASD). Most were minithoracotomy using the single-patch technique for patients with anomalous pulmonary veins (APVs) connecting to the superior vena cava-right atrium (SVC-RA) junction. It is unclear whether patients with APVs draining high to the SVC can be repaired safely and effectively through port access.
    Methods: From May 2019 to October 2022, 11 consecutive SV-ASD patients with APVs connecting high to the SVC were enrolled in this prospective study. One 12 mm port and 2 trocars (5.5 mm and 10 mm) were established. The pleural and pericardial spaces were filled with CO
    Results: There were no early or late deaths and no reoperations. The concomitant procedures included 5 patients (45.5%) with patent foramen ovale closure, 2 with ASD extension, and 3 with tricuspid valve repair. No endoscopic failure was recorded. The average cardiopulmonary bypass and operative times were 96 (23) min and 190 (30) min, respectively. No cases of venous stenosis or sinus node dysfunction were noted during follow-up of 16.4 ± 12.2 months.
    Conclusions: SV-ASD with the APVs draining high to the SVC could be repaired safely and effectively through port access with a double-patch technique.

    Abstract The current paradigm of training deep neural networks for classification tasks includes minimizing the empirical risk that pushes the training loss value towards zero, even after the training error has been vanished. In this terminal phase of training, it has been observed that the last-layer features collapse to their class-means and these class-means converge to the vertices of a simplex Equiangular Tight Frame (ETF). This phenomenon is termed as Neural Collapse (NC). To theoretically understand this phenomenon, recent works employ a simplified unconstrained feature model to prove that NC emerges at the global solutions of the training problem. However, when the training dataset is class-imbalanced, some NC properties will no longer be true. For example, the class-means geometry will skew away from the simplex ETF when the loss converges. In this paper, we generalize NC to imbalanced regime for cross-entropy loss under the unconstrained ReLU feature model. We prove that, while the within-class features collapse property still holds in this setting, the class-means will converge to a structure consisting of orthogonal vectors with different lengths. Furthermore, we find that the classifier weights are aligned to the scaled and centered class-means with scaling factors depend on the number of training samples of each class, which generalizes NC in the class-balanced setting. We empirically prove our results through experiments on practical architectures and dataset.

    Abstract In this study, the gamma ray-induced Maillard reaction method was carried out for chitosan (CTS) and glucosamine (GA) to improve the water solubility and antibacterial activity.
