    Title translation Validation of a symptom scale for COVID-19 patients in ambulatory care.
    Abstract Background: A symptom scale can be useful for the standardization of clinical evaluations and follow-up of COVID-19 patients in ambultaroy care. Scale development should be accompanied by an assessment of its reliablility and validity.
    Objective: To develop and measure the psychometric characteristics of a COVID-19 symptom scale to be answered by either healthcare personnel or adult patients in ambulatory care.
    Material and methods: The scale was developed by an expert panel using the Delphi method. We evaluated inter-rater reliability, where we defined a good correlation if Spearman's Rho was ≥ 0.8; test-retest, where we defined a good correlation if Spearman's Rho was ≥ 0.7; factor analysis using principal component methodology; and discriminant validity using Mann-Whitney's U test. A p < 0.05 was considered statistically significant.
    Results: We obtained an 8 symptom scale, each symptom is scored from 0-4, with a total minimum score of 0 and a maximum of 32 points. Inter-rater reliability was 0.995 (n = 31), test-retest showed correlation of 0.88 (n = 22), factor analysis detected 4 factors (n = 40) and discriminant capacity of healthy versus sick adults was significant (p < 0.0001, n = 60).
    Conclusions: We obtained a reliable and valid Spanish (from Mexico) symptom scale for COVID-19 ambulatory care, answerable by patients and health care staff.

    Abstract The outbreak of the new COVID-19 disease is a serious health problem that has affected a large part of the world population, especially older adults and people who suffer from a previous comorbidity. In this work, we proposed a classifier model that allows for deciding whether or not a patient might suffer from the COVID-19 disease, considering spatio-temporal variables, physical characteristics of the patients and the presence of previous diseases. We used XGBoost to maximize the likelihood function of the multivariate logistic regression model. The estimated and observed values of percentage occurrence of cases were very similar, and indicated that the proposed model was suitable to predict new cases (AUC = 0.75). The main results revealed that patients without comorbidities are less likely to be COVID-19 positive, unlike people with diabetes, obesity and pneumonia. The distribution function by age group showed that, during the first and second wave of COVID-19, young people aged ≤20 were the least affected by the pandemic, while the most affected were people between 20 and 40 years, followed by adults older than 40 years. In the case of the third and fourth wave, there was an increased risk for young individuals (under 20 years), while older adults over 40 years decreased their chances of infection. Estimates of positive COVID cases with both the XGBoost-LR model and the multivariate logistic regression model were used to create maps to visualize the spatial distribution of positive cases across the country. Spatial analysis was carried out to determine, through the data, the main geographical areas where a greater number of positive cases occurred. The results showed that the areas most affected by COVID-19 were in the central and northern regions of Mexico.

    Abstract Allergic rhinitis (AR) has considerable impact on the general health of individuals. Therefore, treatment trials should include an evaluation of quality of life. We aimed to determine changes in the quality of life of moderate/severe AR patients treated with standard treatment in addition to dialyzable leukocyte extract (DLE), a peptide-based immunomodulator. In a prospective, non-controlled trial, DLE was added to the standard treatment regimen for patients with moderate/severe AR. DLE was administered orally at 2 mg per day for 5 days, followed by 4 mg per week for 5 weeks, and then 2 mg per week for 5 weeks. The primary endpoints were overall improved Standardized Rhinoconjunctivitis Quality of Life Questionnaire (RQLQ) scores, domain scores, and individual item scores of 0.5 points or higher. Statistical significance was defined as P < .05. Thirty patients (50% female) aged 14 to 60 years old (33.4 ± 11.9) were enrolled in this study. The mean overall basal quality of life score was 3.41 ± 1.22. After 11 weeks, the mean RQLQ score was 1.74 ± 1.09 ( P < .0001; 95% confidence interval [CI], 1.05-2.33), and all the domain scores improved (daily activities P < .001, 95% CI 0.91-2.15, sleep P < .001, 95% CI 0.9-2.26, non-hay fever symptoms P = .001, 95% CI 0.51-1.82, practical problems P < .001, 95% CI 1.55-2.85, nasal symptoms P < .001, 95% CI 1.36-2.67, ocular symptoms P < .001, 95% CI 1.05-2.17, emotional P < .001, 95% CI 1.23-2.55). Each of the 28 individual item scores on the RQLQ showed clinical (minimal important difference [MID] ≥ 0.5) and statistical ( P < .05) improvements. DLE might be a beneficial adjuvant treatment for AR. Our results provide preliminary data for future research. Clinical trials registration ID: NCT02506998.

    Abstract Objective: Determine good recovery practices for ambulation of octogenarian women after hospital discharge after being operated on for hip fracture.
    Methods: Prospective study during the second half of 2019, with 192 women (85.95 ± 5.1 years) with hip fracture. A medical history, fracture types, complications, surgical treatment, and assessment of the level of ambulation were recorded before and after six months of hospital discharge.
    Results: 100 patients lived in the family home and 92 in an institutional center, 68.2% provided pertrochanteric fracture and a total of 3.7 comorbidities, all of them received spinal anesthesia and were admitted an average of 11.4 days. After six months, the patients showed a significant loss of functional independence with respect to the situation prior to the fracture, both for the ability to wander and for activities of daily living. It is noteworthy that the worst prognosis in the recovery of ambulation has to do with intermediate levels of ambulation and that the functional level of departure influences to a lesser extent than the place where they perform the recovery.
    Conclusions: Age is a factor that influences the recovery of hip fracture, but there are other influential factors since patients who remain in the family home have a better functional prognosis than those who recover in institutionalized centers, after six months of hospital discharge.

    Abstract Resumen Objetivo: Identificar las principales características clínicas, funcionales y cognitivas de las mujeres octogenarias hospitalizadas con diagnóstico de fractura de cadera en el Hospital Universitario de León (España). Además, determinar la influencia del domicilio, al que se trasladan los pacientes al recibir el alta hospitalaria en la recuperación de la deambulación. Método: Estudio piloto prospectivo descriptivo transversal que analiza las características fisiológicas de mujeres mayores hospitalizadas con diagnóstico de fractura de cadera en el Hospital de León. Resultados: Se incluyeron 96 mujeres con una edad media de 85,95 ± 5,1 años. El número total de comorbilidades presentadas por las pacientes fue de 3,7. El tipo de fractura más frecuente fue la pertrocantérica. Todos los pacientes empeoraron respecto a la capacidad de deambulación basal, encontrando las mayores diferencias en los que fueron institucionalizados frente a los que permanecieron en su residencia habitual. Conclusiones: El domicilio al que se trasladan los pacientes al recibir el alta hospitalaria es un factor determinante para la recuperación de la movilidad e independencia de los mismos. En este sentido, es importante señalar que la práctica habitual de institucionalización de las personas mayores después de una fractura de cadera se asocia con una peor recuperación.

    Abstract Introduction: Since delirium is a major complication that can arise after a patient with a hip fracture has been hospitalized, it is considered to be one of the most common geriatric conditions. Therefore, its prevention and early detection are essential for reducing both the length of the patient's stay in the hospital and complications during the hospitalization process.
    Objective: To identify and analyze the predictors for developing delirium in octogenarians who were admitted to hospital for a hip fracture.
    Methodology: A prospective study conducted with a sample of 287 patients aged 80 years and older (mean age 87.2 ± 3.2 years; 215 women, 72 men), recruited from the Trauma Unit of the University Hospital of León (Spain). Further, 71.1% of the patients lived in a family member's home, while the other 28.9% lived in a nursing home. After observing each patient's interactions with their doctor in a clinical setting, the data for this study were obtained by reviewing the selected patients' charts. The variables analyzed were sociodemographic information (age, sex, and place of residence), medical information (type of hip break and surgical intervention), cognitive impairment (MMSE score), functional level (Barthel Index score), and clinical information (pharmacological, comorbidities, complications, and the diagnosis and assessment of the severity of delirium in a patient). The univariate and multivariate logistic regression analysis showed a significant relationship between acute confusional state and the following variables: anemia, American Society of Anesthesiologists (ASA) III and IV patients, state of cognitive frailty and functional level, a urinary tract infection, changes in the visual field, renal arterial occlusion, and the type and dosage of drugs administered (this variable was identified in the multivariate model). The inverse relationship between anemia and acute confusional state is surprising.
    Conclusion: This research shows that clinical observation of acute confusional state is necessary but not sufficient for addressing this condition early and adequately in older adults who have been hospitalized for a hip fracture.

    Abstract Thyroid abscess is a rare entity, commonly experienced by immunocompromised patients, or those who have anatomical abnormalities or a pre-existing thyroid disease. An early diagnosis continued by treatment with antibiotics and drainage of the abscess is the recommended therapeutic strategy for such cases. The present study describes a clinical case of this rare event, and also provides a brief literature review. The present study describes the case of a 48-year-old healthy male with no medical antecedents, apart from acute prostatitis treated with antibiotics for 6 days prior, who visited the Emergency Department of the authors' hospital with neck pain and progressive swelling of the mass. Diagnostic imaging confirmed the authors' suspicion of an abscess and revealed the lesion displacing the airway to the contralateral side. This restricted the mobility of the neck of the patient. As an emergency measure, the patient was then taken to the operating room for a neck examination. A hemithyroidectomy was finally performed. Following a prolonged hospital duration, he was discharged from the hospital and his recovery was uneventful without any voice alterations, hypocalcemia or recurrence.
