    Abstract SARS-CoV-2 is the pathogen responsible for COVID-19, an infectious disease that can evolve from a mild viral illness to multiple organ failure and death. This disease is characterized by a high transmissibility rate, which has lead to its spread throughout the world. There are no clear prognostic markers to guide the severity of the condition; however, some clinical elements could be considered possible predictors of severity. Knowing its viral structure and pathogenesis has allowed to recognize specific molecular pathways candidates as therapeutic targets for various drugs, which are still under investigation and will set the guidelines for future protocols.

    Abstract Resumen El SARS-CoV-2 es el agente patógeno responsable del COVID-19, enfermedad infecciosa que puede evolucionar desde un cuadro viral leve hasta la falla multiorgánica y muerte. Esta enfermedad se caracteriza por tener una tasa de transmisibilidad elevada, lo que ha permitido su diseminación por el mundo. No existen marcadores pronósticos claros que guíen la gravedad del cuadro; no obstante, hay elementos clínicos que podrían considerarse posibles predictores de gravedad. Conocer su estructura viral y patogenia ha posibilitado objetivar los pasos moleculares específicos que pueden ser blancos terapéuticos de variados fármacos, los cuales se mantienen en investigación y marcarán las directrices de futuros protocolos.

    Abstract Acute kidney injury is a syndrome characterized by a sudden, sustained, and potentially reversible decrease in glomerular filtration rate and tubular function, which globally impacts renal function. It comprises of a series of events starting with the presence of risk factors, then evolving towards acute kidney injury progression, characterized by stress, injury, and renal failure, culminating with either the use of renal replacement therapy or death. Currently, the use of biomarkers that differentiate between the initial functional deterioration and late-onset structural damage of the kidney enables the clinician to perform an early diagnosis and indicate treatment before the stages of acute kidney injury progression are established, thus increasing survival rates.

    Title translation Essential aspects for the administration of blood components in hospitalized patients: A narrative literature review.
    Abstract Introduction: Transfusion medicine develops and disseminates guidelines that govern the optimal conditions for transfusion. The purpose of this article is to review the current evidence on the use of blood components.
    Methods: We searched PubMed, Scholar Google, ScienceDirect, SciELO and Cochrane web portals, as well as official documents published in the Chilean Society of Hematology. Articles from the last ten years were included, of which 42 were appropriate for this narrative literature review.
    Conclusion: First of all, there is a controversy between two types of strategies regarding the practice of red blood cell transfusion: a liberal strategy and a restrictive strategy. Second, for the management of coagulopathies, clotting times do not reflect the true ability of patients to clot. Third, to reverse the effect of coumadin, the administration of vitamin K would suffice over the use of fresh frozen plasma. Fourth, the use of physiological triggers could help define the best time for a transfusion.

    Title translation Presentación atípica y diagnóstico tardío de disección aórtica aguda sin tratamiento quirúrgico oportuno: reporte de un caso y revisión de la literatura.
    Abstract Acute aortic syndrome includes a group of diseases that have clinical similarity in their natural history, the most important characteristic being their association with a high vital risk. The diagnosis and management of aortic dissection depends on the degree of aortic involvement according to the location of the lesion, as defined by the Stanford classification. In this syndrome, chest pain is considered the cardinal symptom; however, there are situations where clinical feedback is difficult. We present the case of a patient who debuted with a Stanford A aortic dissection, with an indication for surgical resolution in the acute phase, but who unexpectedly presented unspecific clinical manifestations. An opportune diagnosis was not obtained. After repeated consultations for changes in his symptoms, the definitive diagnosis was determined through imaging study, evolving favorably with ambulatory therapy.

    Abstract Resumen El síndrome aórtico agudo incluye un grupo de enfermedades que poseen similitud clínica en su historia natural, siendo la característica más importante su asociación a un alto riesgo vital. Entre estas patologías, el diagnóstico y manejo de la disección aórtica depende del grado de compromiso aórtico según la ubicación de la lesión definida bajo la clasificación de Stanford. Dentro de las manifestaciones clave se considera al dolor torácico como el síntoma cardinal. Sin embargo, existen situaciones que por ambigüedad clínica retrasan el diagnóstico. Se presenta el caso de un paciente que debutó con una disección aórtica Stanford A, con indicación de resolución quirúrgica en fase aguda pero que dado lo inespecífico de su cuadro clínico, no se logró un diagnóstico oportuno. Posterior a reiteradas consultas por cambios en su sintomatología, se determinó el cuadro definitivo a través de estudio imagenológico, evolucionando de forma favorable con terapia ambulatoria.
