    Abstract The diploid A. glutinosa (L.) Gaertn. is widespread throughout the European continent, except in the Iberian and Balkan Peninsulas where tetraploid populations have been discovered. We focused on the tetraploid species described as A. rohlenae Vít, Douda and Mandák that occupies the western part of the Balkan Peninsula, where it has likely completely replaced the diploid species. While the distribution range of the diploid A. glutinosa s. str. is well known, the exact distribution range of the tetraploid A. rohlenae is unknown. Here, we report the first exact distribution of the tetraploid A. rohlenae and the anticipated hybrid zones in which it is in contact with diploid populations using flow cytometry and morphometrics. Tetraploids are located primarily in the mountainous parts of the study area and towards the lowlands are gradually being replaced by diploids, forming a contact zone. We compare the main morphological characteristics of both species. Due to the geographical proximity of the study species, the morphological differences between them are clear outside the contact zones. However, within the contact zones, we recorded hybridisations that obscure the morphological differences between species, probably due to the presence of triploid hybrids.

    Abstract The family Buthidae represents an early-diverging and most species-rich lineage of extant scorpions, but its internal phylogenetic relationships are still poorly understood. The family is traditionally divided into six morpho-groups; however, the monophyly of some of them remains unclear. We combined multilocus sequence data with extensive taxon sampling to reconstruct the phylogenetic relationships among Buthidae and assess the validity of the morphology-based groupings. We recovered a monophyletic Buthus group as a sister clade to all the remaining Buthidae. We also found support for the monophyly of the Tityus group, but the remaining morpho-groups were recovered as para-/polyphyletic. Our results also suggest that some genera are in need of a taxonomic revision.

    Abstract Aims: Refractory out-of-hospital cardiac arrest (r-OHCA) in patients with pulmonary embolism (PE) is associated with poor outcomes. The role of extracorporeal cardiopulmonary resuscitation (ECPR) in this patient group is uncertain. This study aims to analyse clinical course, outcomes, and the effect of an invasive procedure, including ECPR, in a randomized population.
    Methods and results: A post hoc analysis of a randomized controlled trial (Prague OHCA study) was conducted to evaluate the effect of ECPR vs. a standard approach in r-OHCA. A subgroup of patients with PE-related r-OHCA was identified, and procedural and outcome characteristics, including favourable neurological survival, organ donation, and complications, were compared to patients without PE. Pulmonary embolism was identified as a cause of r-OHCA in 24 of 256 (9.4%) enrolled patients. Patients with PE were more likely to be women [12/24 (50%) vs. 32/232 (13.8%); P < 0.001] and presented more frequently with an initial non-shockable rhythm [23/24 (95.8%) vs. 77/232 (33.2%); P < 0.001], as well as more severe acidosis at admission [median pH (interquartile range); 6.83 (6.75-6.88) vs. 6.98 (6.82-7.14); P < 0.001]. Their favourable 180-day neurological survival was significantly lower [2/24 (8.3%) vs. 66/232 (28.4%); P = 0.049], but the proportion of accepted organ donors was higher (16.7 vs. 4.7%, P = 0.04).
    Conclusion: Refractory out-of-hospital cardiac arrest due to PE has a different presentation and inferior outcomes compared to other causes but may represent an important source of organ donations. The ECPR method did not improve patient outcomes.

    Abstract The surgical treatment of intraretinal juxtapapillary retinal hemangioblastomas (JRHs) was previously contraindicated because of the significant risk of collateral damage to the macula and optic nerve. This case report discusses the effectiveness and safety of a novel surgical technique using intraocular bipolar diathermy forceps to coagulate feeder and draining blood vessels of an intraretinal JRH. The patient suffered from bilateral retinal hemangioblastomas with loss of visual function in one eye and the development of an intraretinal JRH in the other eye. Despite intensive treatment with intravitreal bevacizumab and subconjunctival triamcinolone acetonide, growth of the intraretinal JRH continued, macular exudation worsened, and visual acuity decreased. Surgical treatment was undertaken in which, first, the feeder and draining vessels of the JRH were identified by comparing the retinal imaging of the JRH with the imaging before the emergence of the JRH 4 years earlier. Then, retinal incisions were made above the blood vessels and parallel to the nerve fibers during a pars plana vitrectomy. Lastly, these vessels were lifted above the retinal surface and coagulated using intraocular diathermy forceps. Postoperatively, macular edema reduced, and visual acuity increased and remained stable for about 6 months. Using intraocular diathermy forceps, this case report demonstrates effective and safe intraretinal JRH blood vessel coagulation above the retinal surface. This novel surgical approach was able to delay the deterioration of visual acuity due to tumor growth and exudation in this patient. This suggests that coagulation with intraocular diathermy forceps can be considered an additional surgical treatment option for JRHs, especially those with an intraretinal growth pattern.
