    Abstract Objective To investigate the value of cystatin C-based estimated glomerular filtration rate (eGFR-CysC) combined with thromboelastogram (TEG) to predict clinical prognosis of patients with acute-on-chronic liver failure (ACLF). Methods 34 patients with ACLF (ACLF group), 26 patients with chronic hepatitis (chronic hepatitis group) and 30 patients with cirrhosis (cirrhosis group) were selected. Laboratory parameters within 24 h after admission, TEG parameters and eGFR-CysC value were collected and compared among three groups. All patients were divided into different groups according to the incidence of complications and prognosis. The risk factors affecting clinical prognosis of ACLF patients were analyzed by binary logistic regression analysis. The predictive value of these risk factors for clinical prognosis of ACLF patients was assessed by the receiver operating characteristic (ROC)curve. Results There were significant differences in the white blood cell count (WBC), alanine aminotransferase (ALT), total bilirubin (TBIL), albumin (ALB), C-reactive protein (CRP), prothrombin time activity (PTA), prothrombin time (PT), international normalized ratio (INR), reaction time of TEG (R), maximum amplitude (MA) and eGFR-CysC among different groups (all P < 0.05). ACLF intra-group analysis showed that MA and eGFR-CysC were significantly different between patients with or without acute renal injury (both P < 0.05), R and MA were significantly different between patients with or without upper gastrointestinal bleeding (both P < 0.05), MA and eGFR-CysC significantly differed between the death and survival groups (both P < 0.05). Binary logistic regression analysis suggested that MA (OR=0.439, 95%CI 0.231-0.833, P = 0.012) and eGFR-CysC (OR=0.931, 95%CI 0.878-0.988, P = 0.018) were the risk factors affecting short-term prognosis of ACLF. MA combined with eGFR-CysC yielded similar prognostic value in patients with ACLF (AUC=0.933, 95%CI 0.792-0.990) compared with Model for End-Stage Liver Disease (MELD) ...

    Abstract Background: Coronavirus disease 2019 (COVID-19) is currently a global pandemic. The pathogenesis of severe COVID-19 has been widely investigated, but it is still unclear. Human leukocyte antigen (HLA) plays a central role in immune response, and its variants might be related to COVID-19 progression and severity.
    Objective: To investigate the hypothesis that individual HLA variations could alter the course of COVID-19 and might be associated with the severity of COVID-19.
    Methods: In this study, we conducted an HLA targeted capture enrichment and sequencing of severe COVID-19 patients matched to mild cases. A total of 16 COVID-19 patients, confirmed by SARS-CoV-2 viral RNA polymerase-chain-reaction (PCR) test and chest computed tomography (CT) scan, were enrolled in this study. The HLA targeted capture enrichment and sequencing were conducted. HLA typing was performed by comparing contigs with IPD-IMGT/HLA Database.
    Results: In this study, 139 four-digit resolution HLA alleles were acquired. The results showed that HLA-DRB3*01:01 allele was significantly associated with the severity of COVID-19 (odds ratio [OR] = 27.64, 95% confidence interval [CI] = 1.35-560.50, P = 0.0064). And HLA-K*01:01 might be a potential risk factor for COVID-19 severity (OR = 0.11, 95% CI = 0.017-0.66, P = 0.019), but HLA-K*01:02 might be a protective factor (OR = 7.50, 95% CI = 1.48-37.92, P = 0.019).
    Conclusion: Three non-classical HLA alleles, including HLA-DRB3*01:01, HLA-K*01:01, HLA-K*01:02 were identified to be associated with the severity of COVID-19 by comparing mild and severe patients. The current findings would be helpful for exploring the influence of HLA gene polymorphisms on the development and severity of COVID-19.

    Abstract Methazolamide is used to treat patients with glaucoma. However, as a sulfonamide derivative, methazolamide shares the same adverse reaction profile as other sulfa-based medications. Stevens-Johnson syndrome (SJS) and toxic epidermal necrolysis (TEN) are rare delayed-type hypersensitivity cutaneous reactions with high morbidity and mortality. Here, we report a severe SJS/TEN overlap syndrome in an 85-year-old Chinese male patient who received methazolamide 25 mg twice daily for his left eye glaucoma. The causal relationship between SJS/TEN and methazolamide was categorized as "highly likely" on the algorithm for assessing drug causality for epidermal necrolysis. In addition to the treatments with methylprednisolone and immunoglobulin, we used a special electromagnetic spectrum therapeutic apparatus to provide skin wound care. The patient had a thoroughly satisfying recovery. This is the first case report to use electromagnetic field therapy in a patient with SJS/TEN. We share our experience here and suggest that electromagnetic field therapy can provide advanced skin wound care and facilitate the recovery of SJS/TEN.

    Abstract Objectives: To examine the role of ferroptosis on the pathogenesis and progression of COVID-19.
    Materials and methods: A total of 127 patients who were hospitalized for COVID-19 were categorized into two groups according to the intensity of oxygen therapy (high-flow or low-flow). Clinical characteristics, laboratory parameters, plasma markers, and peripheral blood mononuclear cell (PBMC) markers were measured at baseline and one or two weeks after treatment. Telephone follow-up was performed 3 months after discharge to assess long COVID.
    Results: Patients receiving high-flow oxygen therapy had greater levels of neutrophils; D-dimer; C reactive protein; procalcitonin; plasma protein levels of tumor necrosis factor-alpha (TNF-α), interleukin 6 (IL-6), IL-17, and acyl-CoA synthetase long-chain family member 4 (ACSL4); and PBMC mRNA level of TNF-α; but had lower levels of lymphocytes and plasma glutathione peroxidase 4 (GPX4). There were negative correlations of plasma GPX4 and cystine/glutamate transporter-11 (SLC7A11) with TNF-α, IL-6, and IL-17, and positive correlations of ACSL4 with inflammatory markers in plasma and PBMCs. The plasma levels of TNF-α, IL-6, IL-17, and ACSL4 were significantly lower after treatment than at baseline, but there were higher post-treatment levels of lymphocytes, GPX4, and SLC7A11. Patients with long COVID had a lower baseline level of plasma SLC7A11.
    Conclusion: Ferroptosis is activated during the progression of COVID-19, and a low baseline level of a ferroptosis marker (SLC7A11) may indicate an increased risk for long COVID-19. Ferroptosis has potential as a clinical indicator of long COVID and as a therapeutic target.

    Abstract Background: Field-of-view optimized and constrained undistorted single-shot imaging (FOCUS) is a new sequence that shows enhanced anatomical details, improving the diffusion-weighted (DW) images.
    Purpose: To investigate the value of FOCUS diffusion-weighted imaging (DWI) in the evaluation of nasopharyngeal carcinoma (NPC) and compare it with the single-shot echo planner imaging (SS-EPI) DWI approach.
    Material and methods: A total of 87 patients with NPC underwent magnetic resonance imaging, including FOCUS and SS-EPI DWI sequences. The signal-to-noise ratio (SNR), signal-intensity ratio (SIR), contrast-to-noise ratio (CNR), and apparent diffusion coefficient (ADC) values of the nasopharyngeal lesions were measured and compared. According to the clinical stages of patients, T and N were divided into early and advanced stage groups, respectively. The mean ADC values of the two techniques were computed, and the area under the curve (AUC) was estimated to calculate the diagnostic efficiency.
    Results: Subjective and objective image qualitative values of FOCUS were significantly higher than those of SS-EPI. The ADC values for FOCUS of early T and N stages were significantly lower than those of the advanced stages.
    Conclusion: FOCUS provides significantly better image quality in NPC compared to SS-EPI, with lower ADC values for early-stage disease than late-stage disease.

    Abstract Background: The current study aimed to construct a computed tomography (CT)-based decision tree algorithm (DTA) model to predict the epidermal growth factor receptor (EGFR) mutation status in synchronous multiple primary lung cancers (SMPLCs).
    Methods: The demographic and CT findings of 85 patients with molecular profiling for surgically resected SMPLCs were reviewed retrospectively. Least absolute shrinkage and selection operator (LASSO) regression was used to select the potential predictors of EGFR mutation, and a CT-DTA model was developed. Multivariate logistic regression analysis and receiver operating characteristic (ROC) curve analysis were performed to assess the performance of this CT-DTA model.
    Results: The CT-DTA model was applied to predict the EGFR mutant that had ten binary split, of which eight parameters to accurately categorize the lesions as follows: the presence of bubble-like vacuole sign (19.4% importance in the development of the model), presence of air bronchogram sign (17.4% importance), smoking status (15.7% importance), types of the lesions (14.8% importance), histology (12.6% importance), presence of pleural indentation sign (7.6% importance), gender (6.9% importance), and presence of lobulation sign (5.6% importance). The ROC analysis achieved an area under the curve (AUC) of 0.854. Multivariate logistic regression analysis demonstrated that this CT-DTA model was an independent predictor of EGFR mutation (P<0.001).
    Conclusions: CT-DTA model is a simple tool to predict the status of EGFR mutation in SMPLC patients and could be considered for treatment decision-making.

    Abstract Introduction and objective: Decompensated cirrhosis (DCC) is a more advanced stage of liver cirrhosis (LC). It is important to identify biomarkers to predict DCC progression. The aim of this study was to analyze microRNA (miRNA) profiles of whole blood involved in the DCC process to gain a better understanding of the molecular mechanisms underlying its development.
    Materials and methods: RNA-Seq analysis of blood samples from a discovery set, including four DCC patients and four LC individuals, was performed to identify differentially expressed miRNAs. The selected differentially expressed miRNAs were validated by using an independent validation set.
    Results: In this study, a total of 1,036 miRNAs were identified in whole blood samples. Forty differentially expressed miRNAs were identified, including 24 upregulated and 16 downregulated miRNAs. The expression levels of three upregulated miRNAs (hsa-miR-20b-5p, hsa-miR-421, and hsa-miR-1307-3p) and two downregulated miRNAs (hsa-miR-139-5p and hsa-miR-150-5p) were validated by quantitative reverse transcriptase polymerase chain reaction. The receiver operator characteristic curve for the logistic regression model based on hsa-miR-20b-5p, hsa-miR-421, and hsa-miR-150-5p could distinguish DCC patients with excellent diagnostic accuracy (area under the curve: 0.981, p < 0.01).
    Conclusion: The miRNA expression profiles in patients with DCC and LC controls suggested that miR-20b-5p, miR-421, and miR-150-5p could be potential biomarkers and therapeutic targets for this condition.

    Abstract Objective To investigate the effects of YAP on the occurrence and progression of acute liver failure by regulating the ferroptosis pathway and its underlying mechanism. Methods A total of 20 8-week-old C57BL/6 mice were randomly divided into four groups: a control group, an acute liver failure model group, a YAP agonist XMU-MP-1 treatment group and a YAP inhibitor verteporfin treatment group, five mice for each group. HE staining was used to observe the pathological changes of hepatic inflammation and necrosis. Plasma alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels were detected by liver biochemistry. Iron (Fe), malondialdehyde (MDA), glutathione (GSH) determination kits were used to measure their levels in liver tissues of each group. The changes of hepatocyte mitochondrial in each group were observed by electron microscopy. Real time PCR and Western blot analysis were used to detect the mRNA and protein expressions of YAP, glutathione peroxidase 4 (GPX4) and 5-lipoxygenase (5-LOX). Results Compared with the control group, mice in the acute liver failure model group and the YAP inhibitor verteporfin treatment group showed severe liver tissue congestion with inflammatory cell infiltration and structural damage to hepatic lobules. Liver injury was alleviated in the XMU-MP-1 treatment group. With the occurrence of liver failure, plasma ALT and AST levels significantly increased, and liver function was improved in XMU-MP-1 treatment group. Electron microscopy showed that mitochondria in hepatocytes of mice with liver failure became smaller and bilayer membrane density increased, while mitochondria changes in the XMU-MP-1 group were alleviated. In addition, the acute liver failure model group showed an increase in Fe and MDA contents, decreased protein expressions of GPX4, and enhanced expression of 5-LOX, suggesting that ferroptosis was involved in acute liver failure in C57BL/6 mice. Ferroptosis was inhibited by activation of YAP. Conclusion Activation of YAP may ameliorate liver injury by inhibiting ferroptosis.
