    Abstract The Institut Curie initiated a therapeutic patient education activity in 2011 by creating several programs. Coordinated and animated by different multidisciplinary teams, these functioned autonomously, with a different initial educational assessment for each one. This organization broke up the patient's educational pathway, and the educational offer, which sometimes proved to be redundant, most often corresponding to the needs shared by all cancer patients. The transversal therapeutic education unit allowed the creation of a single educational pathway per patient. It was necessary to imagine a common initial educational assessment. How can we move from eight specific educational assessments to a single educational assessment? After the harmonization of the different workshops, we moved from seven programs with several workshops to one program with eight themes and 26 workshops. Then several working groups led to the creation of a common, unique framework for the initial educational assessment. In seven months, a total of 119 unique educational assessments were carried out. The majority of those who wanted to undertake an educational process were women (96%). 7% had localized breast cancer, and 66% were undergoing treatment. Each workshop chosen corresponded to a need identified during the assessment. The themes mostly requested were nutrition, communication, and pain. Several advantages have been felt in the implementation of this educational assessment: single entry for the patient reinforced therapeutic alliance, transversal reading of the patient's educational needs. What remains to be done is to train therapeutic education practitioners about this global vision.

    Title translation L-thyroxine pseudomalabsorption: a factitious disease.
    Abstract Introduction: Thyroxine supplementation of patients with hypothyroidism is usually simple. A few patients, however, continue to present elevated TSH levels despite large doses of L-thyroxine.
    Case: We report the case of a 71-year-old women who had had a thyroidectomy 10 years earlier and had since been hospitalized repeatedly for profound hypothyroidism. Despite her consistent claims of good adherence to her treatment regimen, we considered the diagnosis of L-thyroxine pseudomalabsorption and confirmed it by thyroid hormone absorption tests.
    Discussion: L-thyroxine pseudomalabsorption due to concealed poor treatment adherence should be considered after ruling out drug or dietary interference and true organic malabsorption. Diagnosis of this factitious disease can be confirmed by L-thyroxine absorption tests.

    Abstract Background: Constitutional mismatch repair deficiency (CMMRD) is a rare and extraordinarily penetrant childhood-onset cancer predisposition syndrome. Genetic diagnosis is often hampered by the identification of mismatch repair (MMR) variants of unknown significance and difficulties in PMS2 analysis, the most frequently mutated gene in CMMRD. We present the validation of a robust functional tool for CMMRD diagnosis and the characterization of microsatellite instability (MSI) patterns in blood and tumors.
    Methods: The highly sensitive assessment of MSI (hs-MSI) was tested on a blinded cohort of 66 blood samples and 24 CMMRD tumor samples. Hs-MSI scores were compared with low-pass genomic instability scores (LOGIC/MMRDness). The correlation of hs-MSI scores in blood with age of cancer onset and the distribution of insertion-deletion (indel) variants in microsatellites were analyzed in a series of 169 individuals (n = 68 CMMRD, n = 124 non-CMMRD).
    Results: Hs-MSI achieved high accuracy in the identification of CMMRD in blood (sensitivity 98.5% and specificity 100%) and detected MSI in CMMRD-associated tumors. Hs-MSI had a strong positive correlation with whole low-pass genomic instability LOGIC scores (r = 0.89, P = 2.2e-15 in blood and r = 0.82, P = 7e-3 in tumors). Indel distribution identified PMS2 pathogenic variant (PV) carriers from other biallelic MMR gene PV carriers with an accuracy of 0.997. Higher hs-MSI scores correlated with younger age at diagnosis of the first tumor (r = -0.43, P = 0.011).
    Conclusions: Our study confirms the accuracy of the hs-MSI assay as ancillary testing for CMMRD diagnosis, which can also characterize MSI patterns in CMMRD-associated cancers. Hs-MSI is a powerful tool to pinpoint PMS2 as the affected germline gene and thus potentially personalize cancer risk.

    Abstract We present a 42-year-old woman with primary central nervous system lymphoma (PCNSL) and strong F-FDOPA PET uptake. F-FDOPA PET has high diagnostic accuracy in gliomas and brain metastases. The L-type amino acid transporter 1, targeted by F-FDOPA and C-MET PET, is a cell-type transporter usually upregulated in malignant tumors, including PCNSL. In this line, strong uptake was already shown with C-MET in PCNSL. We report the same findings with F-FDOPA. Consequently, PCNSL is a possible differential neoplastic diagnosis of F-FDOPA uptake among neoplastic lesions.
