    Abstract Most work on social interactions studies a single, composite effect of interactions within a group. Yet in the case of sexual initiation, there are two distinct social mechanisms-peer-group norms and partner availability-with separate effects and different potential interventions. Here I develop an equilibrium search and matching model for first sexual partners that specifies distinct roles for these two mechanisms as part of demand and supply. I estimate the model using a national sample of high school students, with data over time on individual virginity status. The results indicate that peer-group norms have a large effect on the timing of sexual initiation for both boys and girls. Changes in opposite-gender search behavior (i.e., partner availability) also have a large impact on initiation rates for boys, but not for girls.

    Abstract In the article "Antithrombotic therapy in atrial fibrillation: new oral anticoagulants" (Meditsinskiy Sovet, 2014, P. 38--48), the focus was on direct thrombin inhibitors and factor Xa inhibitors. This is quite justified. Currently, there is no doubt that direct oral anticoagulation (AC) for the prevention of thromboembolic events (TE) in general and in particular for ischemic stroke in patients with atrial fibrillation (AF) has a number of significant advantages over the "ancient" vitamin K antagonists. They are well known and listed in the latest Russian guidelines for the treatment of AF. These advantages include comparable efficacy in the prevention of stroke and TE with greater safety against the risk of "large" hemorrhage, particularly hemorrhagic stroke; a trend towards lower mortality; fewer interactions with other drugs and foods; no need for continuous monitoring of blood coagulation parameters. However, no one doubts that in the long-term, we will have to use the old, indirect oral AC, if only because of their low cost compared to the new drugs. However, this is not the only reason why we cannot give up the old ones in the near future. For example, new oral AC can be used only in non-valvular AF. Vitamin K antagonists as supported by stronger evidence base, should be preferred in the treatment of cancer patients. The new oral AC, definitely, "are offensive on all fronts." It is already possible to have cardioversion of AF of 48 or more hours against a background of their administration, and interventional treatment of AF is an option in the near future. In a little while, antidotes to direct oral AC will appear. But patients need treatment right now, today. Therefore, we should not forget how to use the "old" drugs.

    Abstract Background: Interatrial block (IAB) is an ECG indicator of atrial fibrosis related to atrial remodeling and thrombus formation thus leading to embolic stroke and increasing mortality. We aimed to assess weather IAB predicted all-cause mortality during 10 years after ischemic stroke.
    Methods: The study sample comprised 235 patients (median age 74 (interquartile range 25-75% 65-81) years, 95 female) included in the Lund Stroke Register in 2001-2002, who had sinus rhythm ECGs at stroke admission. IAB was defined as a P-wave duration ≥120 ms without = partial IAB (n = 56) or with = advanced IAB (n = 41) biphasic morphology (±) in the inferior ECG leads. All-cause mortality was assessed via linkage with the Swedish Causes of Death Register.
    Results: During follow-up 126 patients died (54%). Advanced IAB, but not partial, was associated with all-cause mortality in univariate Cox regression analysis (hazard ratio (HR) 1.98, 95% CI 1.27-3.09, p = 0.003). After adjustment for age, gender, severity of stroke measured by NIHSS scale and smoking status in patients without additional comorbidities advanced IAB independently predicted all-cause mortality (HR 7.89, 95% CI 2.01-30.98, p = 0.003), while in patients with comorbidities it did not (HR 1.01 95% CI 0.59-1.72, p = 0.966).
    Conclusion: Advanced IAB predicted all-cause mortality after ischemic stroke, but mostly in patients without additional cardiovascular comorbidities.

    Abstract Sudden cardiac death (SCD) is an unexpected death due to cardiac causes that occurs in a short time period (generally within 1 h of symptom onset) in a person with known or unknown cardiac disease. Patients with cardiomyopathies, myocarditis, ischemic heart disease and cardiac channelopathies are at risk of SCD. However, a certain percentage of autopsy-negative cases of SCD in the young (<35 years) remain unexplained even after a post-mortem genetic testing. Autoantibodies against cardiac proteins may be potentially involved in the pathogenesis of different heart diseases and in the occurrence of unexplained SCD. In this review we analyze clinical and animal studies that elucidate the prevalence of these autoantibodies in patients with different cardiac diseases and their pathophysiological relevance. We propose a classification of the autoantibodies associated with heart diseases and focus on their molecular and cellular effects. Anti-beta adrenergic receptor antibodies and anti-muscarinic acetylcholine receptor antibodies affect myocardial electrophysiological properties and were reported to be the independent predictors of SCD in patients with different heart diseases. Autoimmune mechanism is proposed for cardiac-related adverse reactions following human papillomavirus (HPV) vaccination. The pentapeptid sharing between HPV's antigens, adrenergic receptors and muscarinic acetylcholine receptors supports this assumption. The dysregulating effects of the autoantibodies against calcium and potassium ion channels can be the basis for autoimmune phenocopies of genetic cardiac channelopathies, which are also associated with SCD.
